1.CLINICAL OBSERVATION OF CRANIOPLASTY USING AUTOGENOUS BONE GRAFT.

Calvarium protects the brain, the most important organ. The defect of calvarium results in not only deformity but also fatal injury from the trauma. The cranial bone defects result from 1) removal of bone flap for intracranial decompression or infection 2) fracture 3) excision of tumor 4) craniectomy for craniosynostosis. The goals of cranioplasty are to protect the brain from trauma and make the aesthetically acceptable contour. From 1990 to 1995, we experienced twelve cases of cranioplasty using autogenous bone graft; 5 cases with rib bone, 3 cases with iliac bone, 2 cases with calvarial bone, and 2 cases with rib and calvarial bones. The result was very excellent without any significant complications.

7.Clinical Evaluation of Anesthetic Management for Pheochromocytoma Following Prazosin Pre-treament .

Perioperative management of patients with pheochromocytoma is challenging. Accordingly, proper preoperative preparation is important. Prazosin, a selective alpha I blocker, may offer a potential advantage. This 54-year-old woman was treated with prazosin 2 mg, b.i.d. for 15 days and also with propranolol 20 mg, b.i.d. for a few days intermittently before the proposed surgery. Both symptoms and blood pressure were well controlled effectively. Induction of anesthesia was accomplished with nitrolingual spray, fentanyl 100 ug, 1% idocaine 50 mg, 2.5% thiopental sodium 200 mg, vecuronium 6 mg and 100% O2-enflurane. During the surgical and anesthetic procedure, the patient showed a reduced incidence of excessive blood-pressure variations and no arrhythmia was present except for supraventricular ectopic beats. Conclusively, we believe that careful preoperative preparation is recommended to minimize intraoperative hemodynamic dieturbances and their sequelae.

8.A Case of Ticlopidine-induced Neutropeina Treated with rhG-CSF.

There are many conditions which are associated with neutropenia, such as infections, chemical and physical agents, and hematopoietic diseases. But ticlopidine-induced neutropenis is rarely reported in Korea. We experienced a case of neutropenia which developed after approximately 1 month of ticlopidine administrarion to a stable angina pectories patient. A 59 year-old woman with stable angina pectoris was placed on ticlopidine. Forty days later, she was admitted for high fevers and shaking chills. On admission, leukocyte count was 900/mm (3) (neutrophil 0/mm (3)), hemoglobin was 11.8g/dl, and platelet count was 440.000/mm (3). After confirming ticlopidine-induced neutropenia by bone marrow aspiration and biopsy, we administated rhG-CSF (neutrogen (r), Choongwae. Co. Korea) at a dose of 3-5ug/kg daily. On the 25th day of treatment, leukocyte count reached 2,890/mm (3). She experienced no adverse effects of rhG-CSF treatment and recorved completely. We assume that the rapid recovery of granulocytes was attributable to rhG-CSF, and we suggest that rhG-CSF should be tried in a patients with ticlopidine-induced neutropenia with depletion of myeloid precursors in the hypocelluar bone marrow.

10.Nucleotide Sequence of Internal Transcribed Spacer 1 of Trichophyton mentagrophytes Isolated from Human and Animals.

BACKGROUND: Trichophyton mentagrophytes is a complex species and 3 perfect states, Arthroderma benhamiae, A. vanbreuseghemii, and A. simii have been described. In Korea and Japan, all of the isolates of T. mentagrophytes of which the perfect states determined were known as A. vanbreuseghemii till recently. However, in Japan, one isolate from a monkey was reported as A. simii and several strains from rabbits or patients were A. benhamiae. Therefore, it is necessary to confirm the existence of A. simii or A. benhamiae in Korea. OBJECTIVES: We performed this study to determine the teleomorphs of 19 strains of T. mentagrophytes isolated from Korean patients infected from pets and 4 from the skin lesions of rabbits. METHODS: DNA sequences of internal transcribed spacer 1 (ITS1) area of the 23 isolates and 3 reference strains were determined and matched with the data registered in GenBank nucleotide sequence database. RESULTS: The sequencing data of 22 isolates were very similar to those of A. vanbreuseghemii and 1 to T. mentagrophytes var. interdigitale respectively. CONCLUSION: Although all of the clinical isolates were related with A. vanbreuseghemii in this study, the possibility of the existence of A. simii and A. benhamiae in Korea is still high and further study is needed.
