Even though the nonketotic hyperglycemia is a metabolic disorder, it complicates hemic- horea-hemiballism rarely. Moreover, generalized chorea-ballism associated with nonketotic hyperglycemia in diabetes mellitus is very rare, so it has not been reported in Korean literature. Although the precise pathophysiologic mechanisms of these disorders are still poorly understood, deficiency of gamma aminobutyric acid (GABA) in nonketotic hyperglycemia or reduced GABAnergic inhibition by striatal lesion may increase inhibitory output to subthalamic nucleus. These result loss of pallidal inhibition and produce contralateral hemichorea-hemiballism. The striatal lesions, such as transient ischemia with reactive astrocytosis or small amount of petechial hemorrhage, are related with changes of magnetic resonance image (MRI) findings presumably. We report a diabetic old woman who developed generalized chorea-ballismus as a very rare complication of nonketotic hyperglycemia. Her brain MRI showed high signal intensity in left lentiform nucleus and right pallidum on T1 weighted images and low signal intensity in bilateral putamen on T2 weighted images with highly enhanced corresponding lesions on T1 weighted enhancement images.

The evaluation of urinary calculi was done in 84 patients of paraplegia and quadriplegia due to traumatic spinal cord injury. These patients were diagnosed by KUB and intravenous urography at National Veterans Hospital during 6 years from Jan. 1975 to Dec. 1980. The results were as follows; 1. Overall incidence of urinary calculi was 38.1 %; Incidence of renal calculi was 8.3%, ureteral calculi 4.8%, and urinary bladder claculi 32.1%. 2.Relation of neurological level and incidence of urinary calculi were as follows; Cervical injury in 34.8% upper thoracic injury in 40.0%, lower thoracic injury in 45.0%, and lumbar injury in 36.5%. 3. Laterality was not toplay a role information of urinary claculi. 4. The urinary calculi were developed 62.5% during the first 36 months following spinal cord injury. 5. The recurrence of urinary calculi was 40.6%; True recurrence was 15.6% and pseudo recurrence was 25.05.

Giant lymph node hyperplasia (Castleman's disease) is a rare disease, which represents a peculiar form of lymph node hyperplasia. Generally, it has been considered as benign and localized disease but recently, revealed malignant transformation in some cases of multicentric form. It usually occurs on the mediastinum and occasionally neck, lung, axilla, mesentery, broad ligament, retroperitoneum or soft tissue of extremities. Histopathologically, it is divided into hyaline vascular or plasma cell type and the former is characterized with prominent vascular proliferation and hyalinization in the central portion and tight concentric layering of lymphocytes at the periphery of the follicles (mantle zone) and the latter is characterized by a diffuse plasma cell proliferation in the interfollicular area. From the point of view of clinical presentation, it has been divided into solitary form, which presents as a localized mass located most commonly in the mediastinum, and multicentric form, which occurs multiple location and has systemic manifestation and transformation into malignancy. Herein we report a case of Giant lymph node hyperplasia occurring in the left spermatic cord in a 58-year old male with brief review of literatures.

Purpose: To verify the anti-microbiotic activity and to develop eco-friendly surface disinfectants using the Opuntia ficus-indica extract. Methods: The stem and fruits of the Opuntia cultivated in Jeju Island were extracted with hot water, 70% EtOH and 100% MeOH, respectively. To examine the antimicrobial activity of the extracts, gram positive bacteria (Bacillus subtilis, Staphylococcus aureus), gram negative bacteria (Escherichia coli, Salmonella typhimurium), yeast (Candida albicans), mold (Aspergillus flavus) and antibiotic resistant bacteria (E. coli, S. aureus) were measured to form a clear zone. Results: The antibacterial activity of the fruit extract was higher than that of the stem extract, and the antibacterial activity of the 100% MeOH (FM) extract and the 70% EtOH (FE) extract of the palm cactus fruit was suitable for the surface disinfectant overall, and clear-zone formed even at relatively low concentrations (250 mg/mL). The FM and FE have antibacterial activity against various bacteria, therefore they can be used as disinfectants. Conclusion The antimicrobial activity of Opuntia ficus-indica extract from Jeju Island by stem and fruit was determined at relatively low concentrations (250 mg/mL). The Opuntia ficus-indica extract will be useful for surface disinfectants dissolving the patients’ bath, oral gargling, and powdered powder in water or ethanol in appropriate concentrations.

Malignant lymphoma is a malignant tumor involving the lymphoid cells in lymph node, which also involve the gastrointestineal tract, skin, respiratory system and orbit etc. Malignant lymphoma rarely involves the orbit, furthermore, both orbital involvememt is very rare, whether it is primary or systemic. We experienced a case of systemic malignant lymphoma involving both orbits in a 66-year-old female which was diagnosed with needle aspiration cytology and improved with systemic anticancer drugs. We report this unusual case with literature review.

PURPOSE: We evaluated the structural abnormality of the lacrimal drainage system by using digital subtraction dacryocystography in patients with functional nasolacrimal obstruction. METHODS: From April 2001 to March 2002, digital subtraction dacryocystography was performed in 17 eyes of 11 patients who had patent lacrimal systems on syringing with no known underlying cause of epiphora. RESULTS: Six eyes of 5 patients showed a diffuse incomplete nasolacrimal duct obstruction inferior to the lacrimal sac, and one eye of 1 patient had a filling defect in lacrimal sac. Fifteen eyes of 10 patients were received silicone tube intubation and one eye of 1 patient underwent dacryocystorhinostomy. CONCLUSIONS: There might be anatomical abnormalities in the lacrimal drainage system although it is clinically diagnosed as functional nasolacrimal obstruction. Digital subtraction dacryocystography will provide a better understanding on various causes of epiphora, and give a precise diagnosis and proper treatment.

PURPOSE: Xanthoma in the eyelids is a common dermatologic disorder. Generally these lesions do not affect the function of the eyelids. We report a case of xanthoma that has caused lid edema and ptosis. METHODS: A 46-year-old woman presented with ptosis and mass of the right upper eyelid for several months. We tried oral corticosteroid treatment, but the lesion was not lessened. We performed an excisional biopsy for the mass and ptosis correction. RESULTS: Histopathological examination of the mass revealed xanthoma. Lipid-laden histocytes infiltrated through the subcutaneous tissue, dermis, muscle layer and the preaponeurotic fat tissue. After surgical treatment, the patient was treated with Fenofibrate. There was a mild recurrence after a 18 months of follow-up.
