2.Hydatidiform Mole with a Coexisting Fetus: Report of three cases with nuclear DNA ploidy analysis by flow cytometry or chromosomal analysis.

Three cases of hydatidiform mole with a coexisting fetus are described. In two cases, flow cytometric analysis of nuclear DNA content from paraffin-embedded, molar tissues and normal-appearing placental tissues showed diploid pattern. One case had karyotypes of 46, XX both in fetal skin fibroblasts and in molar tissues. Microscopically, the villi showed diffuse hydropic swelling with circumferential trophoblastic proliferations and consistent with the diagnosis of complete mole. Two patient had persistent gestational trophoblastic neoplasia on the follow-up. Therefore, in cases of hydatidiform mole with a coexisting fetus, dizygotic twin pregnancies composed of a normal pregnancy from one ovum and a complete mole pregnancy from the other ovum must be considered and the patients should be followed with serum beta-hCG measurements.

6.The Association of Perception of Health Status with Lifestyle of Yanbian Koreans and Yanbian Chinese.

To find the association of perception of health status with lifestyle of different ethnic groups living in north-eastern part of China, a cross-sectional questionnaire survey was done by 10 local health workers for 375 Korean immigrants and 217 Chinese whose age was 30 years old or more. Because Korean immigrants showed higher mortality than Chinese, we expected to find significant lifestyle related with perception of health status which was known to be a predictor of mortality by different ethnic groups. The results were as follows: 1. We found that 59.7% of Yanbian Koreans and 42.9% of Yanbian Chinese felt unhealthy (p=0.000). 2. For Yanbian Koreans, significant variables associated with perception of health status were selected through logistic regression analysis and they were sex; female to male with an OR=2.45 (95% confidence interval[CI]: 1.06, 5.64), prevalence of chronic illness with an OR=8.48 (95% CI: 4.62, 15.56), amount of meal; small or moderate to full with an OR=2.67 (95% CI: 1.40, 5.09), preference of spicy food with an OR=1.78 (95% CI: 1.04, 3.04), and less amount intake of vitamin B2 with an OR=2.29 (95% CI: 1.33, 3.93). 3. For Yanbian Chinese, significant variables associated with perception of health status were prevalence of chronic illness with an OR=4.97 (95% CI: 2.11, 11.68), history of taking ginseng with an OR=3.72 (95% CI: 1.33, 10.43), and less intake of vitamin C with an OR=0.18 (95% CI: 0.07, 0.46). In conclusion, sex, presence of chronic illness, dietary habit, and amount of vitamin- B1 intake were associated with perception of health status in Yanbian Koreans. Presence of chronic illness, experience of ginseng intake, and amount of vitamin C intake were associated with perception of health status in Yanbian Chinese. To prove cause-effect relation between perception of health status and lifestyle, further study is needed for these different ethnic groups.

7.The Evaluation of a Health Promotion Program for the Community resident Older Adults.

PURPOSE: This study was conducted to evaluate the process & outcome of a Health promotion program(Growing Younger & More Active) for the community resident older adults from March to December, 2001. METHOD: A quasi experimental research(one group pretest-posttest design) was used in this study. The subjects were 82 older adults(but 40 older adults 4 weeks after the program). Program had 5 sessions(10 hours) once a week. Data were collected before the program, immediately after & 4 weeks after the program and were analyzed with paired t-test. RESULT: The levels of Satisfaction, Interest & Understanding of the Program were high. Significant differences were found in health knowledge, health promoting behaviors, perceived health status and life satisfaction between before program and immediately after program as well as between before program and 4 weeks after program, but no significant differences in Health attitude. Self efficacy has significant difference only between before program and immediately after program , but no significant differences between before program and 4 weeks after the program. CONCLUSION: This results suggest that a Health promotion program for the community resident older adults developed this study is effective. So this program can be recommended as an effective nursing intervention for the health promotion of the older adults living in community.

8.A Case of Caudal Regression Syndrome Diagnosed by Prenatal Ultrasonography.

Caudal regression syndrome is rare malformative syndrome characterized by lower vertebral agenesis, accompanied by abnormalities of the pelvis, lower extremities and urogenital malformation. Although the cause is not clear, hyperglycemia during the organogenesis may be important teratogen. Strict evaluation of diabetes and its control in preconception and early pregnancy are important to prevent this malformation. And ultrasonography in the first trimester should be recommended for early detection of this syndrome. We report a case of caudal regression syndrome detected by prenatal ultrasonography of the gestational diabetic mother.

9.Healing of the bone around pure titanium implants without primary bone contact.

Primary fixation is one of the most important factor in establishing adequate osseointegration between implant and bone. To evaluate the initial healing response of bone around implants without primary bone contact, this study was designed to create considerable space between implant and bone in 5 mongrel dogs, about l-year old. After 3 holes of 6 . h in diameter were prepared at the femur neck of the dogs, commercially pure titanium thread type implants(STERI-OSS^(R)), 8mm in length and 3.8mm, 5.0mm and 6 .0mm in diameter, were inserted. Implants were supported by only nonresorbable membrane(Teflon^(R)), and the penetration of upper soft tissue into the gap was inhibited by it. The each implant was positioned in the center of the drilled hole. 9 implants with different diameters were inserted in 3 dogs for histologic observation, and 12 were inserted in 2 dogs for mobility test and removal torque test. Fluorescent dyes were injected in order of Doxycycline, Arizarin Red S, and Calcein at intervals of 2 weeks. At 4-,8-, and 12-week after placement, 3 dogs were sacrificed for histologic observation, and at 8- and 12-week after placement, 2 dogs were sacrificed for mobility test using Periotest^(R) (Simens AG, Bensheim, Germany) and torque test using Autograph AGS-1000D series^(R)(Japan). The result were as follows: 1. The wider the gap between bone and implant was, the less bone maturity was, and the later osseointegration was occurred. Trabecular direction of new bone around implant was changed from parallel to perpendicular to the implant, and the gap was filled with new bone, over time. 2. There was a decreasing tendency over time in the mobility of all implants, but the wider gap between bone and implant was, the smaller decrease of the mobility was. 3. There was a increasing tendency over time in the removal torque gauge of all implants, and the wider gap was, the smaller increase of the removal torque gauge was. The results suggest that osseointegration in case of implant without primary bone contact may be obtained by guided bone regeneration technique with prolonged healing period, but the time of second surgery should be considered carefully.

10.Immediate One-Stage Breast Reconstruction Including Nipple-Areolar Complex.

As surgical trend for breast cancer changes, the method for breast reconstruction is also changing. Due to improved diagnostic techniques and public awareness, more and more patients with breast cancer are diagnosed at earlier stage and the number of patients who undergo skin sparing mastectomy is also increasing. Skin sparing mastectomy has several advantages for immediate breast reconstruction over modified radical mastectomy. Its preservation of breast skin envelope and inframammary fold made it possible to provide more natural breast mound shape without any evidence of increased local recurrence of malignancy. Furthermore, in case of immediate breast reconstruction after modified radical mastectomy, procedures for nipple-areolar reconstruction was deferred for fear of postoperative deformation and vascular problem of the flap. This interval made patients less satisfied with the result and postoperative adjuvant treatment had to be sometimes postponed. To avoid these limitations, "one-stage" breast reconstruction including nipple-areolar complex was performed after skin sparing mastectomy. From December 1998 to March 2000, a total of 12 patients with breast cancer underwent skin-sparing mastectomies, which were followed by immediate reconstruction using our new technique. After long-term follow-up, no major complication including flap necrosis or asymmetry was found and all patients were satisfied with the results. With careful patient selection and adequate intra-operative flap tailoring, this new technique could offer lots of advantages to both surgeons and patients.
