4.The effect of both oophorectomy on serotonin level in cerebral cortex and hippocampus of the female Sprague-Dawley rat.

OBJECTIVE: The purpose of this study is to investigate the effect of chronic deprivation of estradiol on the serotonin level in Sprague-Dawley rat's cerebral cortex and hippocampus. METHODS: One hundred of female Sprague-Dawley rats were taken care for seven to eight weeks, then they were randomly assigned into two group; fifty rats underwent bilateral oophorectomy & the remaining rats were the control group. Six months after oophorectomy, brain tissue was extracted from cerebral cortex and hippocampus and then homogenated: this was done in the both groups. The upper layer of the homogenated substance was taken and frozen and finally dried up. The next process was marking with PITC, quantification through HPLC and comparing with authentic serotonin. Protein quantity was measured by using Bradford method. RESULTS: Comparison between the bilateral oophorectomy group and the control group for serotonin content in the cerebral cortex was 134.2+/-14.3 nmol/mg protein(mean+/-SEM) and 122.5+/-11.6 nmol/mg protein in each group. As for the serotonin content in the hippocampus was 148.8+/-13.9 nmol/mg protein and 132.9+/-13.7 nmol/mg protein in each group. It shows that, there was no significant difference in the serotonin level, in the two areas mentioned above. CONCLUSION: The effect of estradiol on the serotonin level may be dependent on the hormonal change duration. Chronic effect of hypoestrogenism might have no effect on tryptophan hydroxylase and monoamine oxidase, thus not changin the serotonin level. These data supported the previous findings that chronic hypoestrogenism had no effect on serotonin level.

7.Inflammatory Pseudotumor of the Liver.

Inflammatory pseudotumor of the liver is a rare benign tumor with microscopic finding of fibrous stroma and chronic inflammatory cell infiltration. Predominance of plasma cells, histiocytes, lymphocytes, some eosinophils and whorled pattern of fibrosis is often characteristic. Due to similar radiologic appearance of hepatocellular carcinoma, inflammatory pseudotumor of the liver was often misdiagnosed and hepatic resection is often performed before preoperative tissue diagnosis. We report a case of inflammatory pseudotumor, which was a 47-year-old woman with fever, upper aMominal pain and weight loss, diagnosed by sonoguided liver biopsy and treated with antibiotic therapy only without surgery. After two weeks of treatment, the radiologic image of the mass was almost disappeared with improvement of previous symptoms. This opportunity and the possible effectiveness of antibiotic treatment should keep in mind in the differential diagnosis of the hepatic mass with similar radiologic appearance of hepatocellular carcinoma.

8.Clinical Usefulness of Primary Broth Cultures.

BACKGROUND: Although enriched broth cultures have been recommended as an adjuvant to the direct plating of tissue and body fluid specimens, the cost-effectiveness of broth cultures has been questioned in regard with the clinical significance of "broth only isolates(BOI)". The purpose of this study was to investigate the usefulness of thioglycollate broth(THIO) cultures. METHODS: We reviewed retrospectively results in the culture specimens of body fluids, tissue biopsies, and puses received during the month of July 1997. All specimens were inoculated into THIO in addition to agar plates. We reviewed the medical records of culture-positive patients to determine the clinical significance and relevance of their isolates. Clinically significant isolates were defined as those for which an appropriate antimicrobial therapy was done except one judged as contaminants by clinicians and clinically relevant isolates as the clinically significant one isolated first. RESULT: Of 2,008 specimens, 512(25.4%) from 365 patients grow 561 isolates 464 plate isolates and 97 BOI. Two hundred eighty nine(62.3%) of the 464 isolates from plate cultures were clinically significant, compared to only 12(12.4%) of 97 BOI (P<0.05). Only four (4.1%) BOI were clinically relevant, including one Pseudomonas aerugiosa from ascites. one Klebsiella pneumoniae and two Staphylococcus aureus from tissue specimens. CONCLUSION: A routine use of enriched broth culture rarely recover clinically relevant isolates. Considering the laboratory and medical costs of the recovery of contaminants and clinically irrelevant isolates, the enrichment broth cultures should be used more selectively.

10.A Systematic Review of Child Abuse Screening Instruments.

PURPOSE: The aim of this study was to conduct a systematic review and to describe the characteristics of child abuse screening instruments. METHODS: Articles regarding the development of a child abuse screening instrument were investigated using the systematic review method. A literature search using the keywords "child and abuse or maltreatment and instrument or screening tool" in English, and "child," "abuse," and "instrument" in Korean, was conducted of material published in PubMed, PsycINFO, CINAHL, SCOPUS, ERIC, and RISS. Database and bibliographic searches, and quality appraisal using the Quality Assessment of Diagnostic Accuracy Studies tool that included systemic reviews, yielded 17 records. RESULTS: Key elementary child abuse screening instruments were developed for physical, psychiatric, affective and sexual and child neglect assessment. The instruments' target populations were children at home and in institutions. The reviewed instruments had the advantage of diagnosing past, concurrent, and indirectly, potential child abuse. CONCLUSION: The results of this study demonstrate that child abuse screening instruments are available for screening and for assessment of abused children in various circumstances. This review of child abuse screening instruments offers evidence for the acceptable use of optimal psychometric tools for child abuse assessment and provides guidelines for child health nursing practice.
