2.Factors Influencing on Resilience of Elderly Gastric Cancer Patients.

PURPOSE: The purpose of this study was to review the resilience of elderly patients of gastric cancer, their self-efficacy, family support, spiritual well-being and depression. METHODS: The subjects were 100 patients who were 65 years old or older who had been admitted to the surgical department at K University Hospital in B City. Data were collected from August 10 to September 2, 2016. RESULTS: Resilience of patients showed significant correlations with the degree of self-efficacy r=.51 (p<.001), the degree of family support r=.47 (p<.001), the degree of spiritual well-being r=.59 (p<.001), the degree of depression r=.34 (p<.001). The factor that affected resilience most was spiritual well-being (β=.35), followed by self-efficacy (β=.21), monthly average income (β=.20), family support (β=.18), and experienced symptoms (β=.15). The overall explanatory power was 47.4%. CONCLUSION: The degree of resilience in elderly patients of gastric cancer was higher when self-efficacy, family support and spiritual well-being were higher. To improve the resilience of elderly patients of gastric cancer, the development and application of a nursing intervention program that take these variables into account is needed.

3.A Study on Suicidal Ideation of High School Students in Urban and Rural Area-Focused on Developmental Assets and Violence.

PURPOSE: The purpose of this study was to identify suicidal ideation in high school students in urban and rural areas. METHODS: For this research two urban and two rural high schools in four cities were chosen. The participants were 713 students (373 in the urban area, 340 in the rural area). The survey was conducted from June 1 to July 31, 2012. Data were analyzed with chi-square test, t-test, ANOVA, and multiple regression using SPSS 18.0. RESULTS: Results showed that 19.5% of respondents had suicidal ideation within the past 1 year. There were significant differences in suicidal ideation and developmental assets between the two areas. Multiple regression analysis revealed that support, school violence, gender, family abuse, constructive use of time, and positive identity were significant predictors and explained 26% of suicidal ideation for high school students in urban areas. On the other hand, in rural areas, family abuse, gender, positive identity, smoking and empowerment were significant predictors and explained 31% of suicidal ideation. CONCLUSION: The findings indicate that implementation of a suicide prevention program in which consideration is given to these factors, and taking into account differences between urban and rural students would be helpful in reducing suicidal ideation.

4.Combined Multimodality Treatment including Surgery.

An ipsilateral supraclavicular lymph node recurrence of breast cancer after surgery has been considered a predecessor to distant metastases. There still is a debate as to whether breast carcinoma patients with the isolated supraclavicular lymph node recurrence should be considered to have disseminated disease or if aggressive treatment, with curative intent, is justified. We report two cases of an isolated ipsilateral supraclavicular lymph node recurrence following modified radical mastectomy, and multimodality treatments with modified radical neck dissection, systemic chemotherapy and involved field radiotherapy. These patients have lived without locoregional recurrence or distant metastases for 3 and 2 years, respectively. Conclusively, we recommend aggressive combined multimodality treatments, including surgery, such as modified radical neck dissection or complete excision of the involved lymph nodes, systemic chemotherapy, and involved field radiotherapy, in patients with isolated supraclavicular lymph node recurrence, but with no other evidence of distant metastases.

5.Characterization of MACS Isolated Cells from Differentiated Human ES Cells.

OBJECTIVE: Human embryonic stem (ES) cells have a great potential in regenerative medicine and tissue engineering. The human ES cells could be differentiated into specific cell types by treatments of growth factors and alterations of gene expressions. However, the efficacy of guided differentiation and isolation of specific cells are still low. In this study, we characterized isolated cells from differentiated human ES cells by magnetic activated cell sorting (MACS) system using specific antibodies to cell surface markers. METHODS: The undifferentiated hES cells (Miz-hESC4) were sub-cultured by mechanical isolation of colonies and embryoid bodies were spontaneously differentiated with DMEM containing 10% FBS for 2 weeks. The differentiated cells were isolated to positive and negative cells with MACS system using CD34, human epithelial antigen (HEA) and human fibroblast (HFB) antibodies, respectively. Observation of morphological changes and analysis of marker genes expression were performed during further culture of MACS isolated cells for 4 weeks. RESULTS: Morphology of the CD34 positive cells was firstly round, and then it was changed to small polygonal shape after further culture. The HEA positive cells showed large polygonal, and the HFB positive spindle shape. In RT-PCR analysis of marker genes, the CD34 and HFB positive cells expressed endodermal and mesodermal genes, and HEA positive cells expressed ectodermal genes such as NESTIN and NF68KD. The marker genes expression pattern of CD34 positive cells changed during the extension of culture time. CONCLUSION: Our results showed the possibility of successful isolation of specific cells by MACS system from undirected differentiated human ES cells. Thus, MACS system and marker antibodies for specific cell types might be useful for guided differentiation and isolation of specific cells from human ES cells.

RESULTS: Mean daily time spent by the preschoolers on smartphones was about 45 minutes/day. About 40% spent more than 60 minutes/day on smartphones. The majority (69.8%) used smartphones without adult supervision. The level of smartphone addiction tendency among the preschoolers was 1.52+/-0.45 on a 4-point scale. Preschoolers whose mothers use smartphones for more than 60 minutes/day showed a higher level of smartphone addiction tendency than preschoolers whose mothers use smartphones for less than 60 minutes/day. Also smartphone addiction tendency among preschoolers had a positive correlation with hyperactive-distractible behavior (r=.228, p=.038) and a negative correlation with parenting efficacy of their mothers (r=-.299, p=.006). CONCLUSION: Our findings demonstrated that preschooler's smartphone addiction tendency is significantly associated with hyperactive-distractible behaviors of the children and low parenting efficacy among their mothers. Therefore well-designed care plans for these populations should be provided to decrease smartphone addiction tendency.

8.Relations of Postpartum Depression with Socio-Demographic and Clinical Characteristics of Preterm Infants and Mothers.

PURPOSE: This study was conducted to explore relationships of postpartum depression with socio-demographic and clinical characteristics of preterm infants and mothers. METHODS: Participants were the mothers of 80 premature infants admitted to neonatal intensive care units. Postpartum depression was measured using the Edinburg Postnatal Depression Scale (EPDS). Clinical characteristics were examined through the medical records. The physiological state for the infants was evaluated using the revised Neurobiologic Risk Score (NBRS). Data were analyzed using the t-test, ANOVA, and Pearson correlation coefficients. RESULTS: Average for EPDS was 9.75+/-5.06, and 30% of the participants were at high risk for postpartum depression. There were significant differences in postpartum depression according to mother's education level (F=3.493, p=.035), economic state (F=5.828, p=.004), multiple pregnancy (t=2.141, p=.037), chorioamnionitis (t=2.349, p=.021), oligohydramnios (t=-2.226, p=.029), broncho-pulmonary dysplasia (t=2.085, p=.040), germinal matrix hemorrhage (t=2.259, p=.027), and revised NBRS (t=-2.772, p=.007). There was a significant positive correlation between postpartum depression and number of health problem of infants (r=.252, p=.024) and revised NBRS (r=.316, p=.004). CONCLUSION: As 30% of the mothers with preterm infants were at high risk for postpartum depression, they require attention. When providing interventions, socioeconomic status as well as the physiological state of premature infants should be considered.

9.Testicular Transplantation in the Rat Model: The Comparison of Testosterone, Gonadotropin, and Histological Change in Allogenic and Isogenic Transplantation Group, and Implantation Group.

PURPOSE: The fate of avascular implants was compared with that of testicular transplants using vascular anastomosis in castrated inbred rats. MATERIALS AND METHODS: Mature inbred male rats were bilaterally castrated and received one testicle as a transplant to the scrotum. The blood supply of the grafts was reestablished by performing an end-to-side anastomosis between an aorto-caval segment of the donor bearing the testicular vessels and the abdominal aorta plus vena cava of the recipient. The average ischemia was 90 min. Lutenizing hormone(LH), follicle-stimulating hormone(FSH) and testosterone were determined by radioimmunoassay in serum samples. RESULTS: In 12 transplant recipients(6 isografts and 6 allografts), a good blood perfusion of the graft was achieved. Out of these, 6 isogenic transplants maintained normal testosterone,LH and FSH during follow up. Implant recipients,however, all displayed a strking early rise of the serum gonadotropins and concomitant castrsted levels of serum testosterone. CONCLUSIONS: Vascular transplantation rather than simple implantation is the method of choice for further research. And in allogenic testicular transplantation, there remains a problem of rejection after the operation, as in any other organ transplantation.

10.Primary Ectopic Mediastinal Goiter Combined with Papillary Microcarcinoma of the Cervical Thyroid Gland.

True primary mediastinal ectopic goiters are quite rare, and they occur in less than 1% of all goiters. We report here on a case in which a primary mediastinal ectopic goiter was accompanied with papillary microcarcinoma of the cervical thyroid. The mediastinal ectopic goiter was not connected to the cervical thyroid and its blood was supplied from intrathoracic vessels, which necessitated a thoracotomy or sternotomy for removal rather than a standard cervical collar incision. We performed total thyroidectomy for the papillary microcarcinoma of the cervical thyroid gland and transsternal removal for the mediastinal goiter; this resulted in a favorable outcome.
