1.Generalized Chorea-Ballismus Associated with Nonketotic Hyperglycemia in Diabetes Mellitus: A Case Report.

Even though the nonketotic hyperglycemia is a metabolic disorder, it complicates hemic- horea-hemiballism rarely. Moreover, generalized chorea-ballism associated with nonketotic hyperglycemia in diabetes mellitus is very rare, so it has not been reported in Korean literature. Although the precise pathophysiologic mechanisms of these disorders are still poorly understood, deficiency of gamma aminobutyric acid (GABA) in nonketotic hyperglycemia or reduced GABAnergic inhibition by striatal lesion may increase inhibitory output to subthalamic nucleus. These result loss of pallidal inhibition and produce contralateral hemichorea-hemiballism. The striatal lesions, such as transient ischemia with reactive astrocytosis or small amount of petechial hemorrhage, are related with changes of magnetic resonance image (MRI) findings presumably. We report a diabetic old woman who developed generalized chorea-ballismus as a very rare complication of nonketotic hyperglycemia. Her brain MRI showed high signal intensity in left lentiform nucleus and right pallidum on T1 weighted images and low signal intensity in bilateral putamen on T2 weighted images with highly enhanced corresponding lesions on T1 weighted enhancement images.

2.Study on Effect of Conjugated Equine Estrogen and Progestogen on Serum Lipid Profiles and Bone Mineral Density in Postmenopausal Women.

Hormone replacement therapy combined with progestogens induces changes in effect of estrogen on serum lipid levels and it has been known that the changes depend on a type and dosage of progestogen. It is also known that progestational agent induces positive ch-ange in bone mineral density. To study the effects of progestogen on lipoprotein and bone metabolism, we administ- ered conjugated equine estrogen 0.625 mg alone to 50 postmenopausal women, in combinat- ion with medroxy- progesterone acetate 5 mg to 40 postmenopausal women. The data demonstrated a beneficial effect in lipoprotein profiles in both groups. Total cholesterol in two groups decreased from the baseline values, LDL-cholesterol decreased significantly by 4.8 % in group I and 16.2 % in group II(p < 0.05), HDL-cholesterol increa- sed significantly by 11.3 % in group I and 14.7 % in group II(p < 0.05), triglyceride incre- ased slightly in both groups. Bone mineral density of femur was maintained and BMD of vertebrae increased by 1.1 % in group I and 2.0 % in group II, but it is not statistically significant. The differences of changes between two groups were not statistically significa- nt. Our results suggest that medroxyprogesterone acetate have no adverse effect on HDL -cholesterol and have no additive effect on bone mineral density in hormone replacement therapy.

3.Balloon Catheter Dilatation in Esophageal Achalasia: Long Term Follow-Up.

PURPOSE: To evaluate the clinical efficacy of balloon catheter dilatation in the treatment of esophageal achalasia. MATERIALS AND METHODS: Seven patients(three males and four females) with esopha-geal achalasia were treated with balloon catheter dilatation. Balloon catheters of variable sizes were used depending on patient's conditions. The patients were followed up over a period of 12-39months. RESULTS: Balloon catheter dilatation in esophageal achalasia was successful in all patients without esophageal perforation. All patients were relieved from dysphagia. Recurrence was not found in 5 patients on long term follow-up study, but was seen in 2 patients after 18 and 21 months, respectively. CONCLUSION: Balloon catheter dilatation was a safe and effective method in the treatment of esophageal achalasia with low recurrence rate of 29% on follow-up study.

6.A Study on Related Factors to Overweight for Primary School Children.

The purpose of this study was to investigate the related factors to child overweight through investigation of children aged 6-12. Data on physical examinations, family history and dietary habits were obtained. Children were classified into normal(98 persons) and overweight(118 persons) group using the combination of height for age, weight for age and weight for height standard by the World Health Organization. Children's height, weight and BMI exhibited a significant difference between groups except children aged 8 for height. Unbalanced diet was a risk factors for overweight(Odd ratio : 1.765, 95% CI : 1.022~3.048). Number of brothers showed negative significance especially in two brothers compared to that of one(Odd ratio : 0.456, 95% CI : 0.209~0.995). But Birth weight, feeding practice, overeating, taking nutrition pills, sleeping time, mother's education level and employmental status didn't have any difference. We found out there were different risk factors between obese and overweight group. They should be divided into different groups in studying risk factors. And we should pay much attention to overweight children in order to prevent improving to obesity.

9.Comparison of the frictional resistance between orthodontic bracket & archwire.

Practitioners are aware of the presence of friction between bracket system and arch wire during sliding movement of teeth. Clinically a mesiodistally applied force must exceed the frictional force to produce a tooth movement. The objective of this study were to determine, on a dry condition, changes in magnitude of friction with respect to load, 3rd order inclination (Torque), arch wire materials and ligature type. Three wire alloys (Stainless Steel, TMA, NiTi) in two wire sizes (.016, .016x, .022 inch) were examined respect to two bracket system (Straight, Standard), and two ligature type (Metal, Plastic ligature) at three levels of load (100g, 150g, 200g). The results were as follows; 1. Frictional resistance was found to increase with increasing load for S.S., TMA, NiTi. 2. The straight bracket system was exhibited more frictional force than standard bracket system for .016x, .022 S.S. tightly ligated metal ligature. But, torque difference did not increase friction for loose metal ligature & plastic ligature. 3. Regardless of the ligature type, torque and load, stainless steel wire sliding against stainless steel exhibited the lowest friction, and TMA sliding against stainless steel exhibited the highest friction. 4. The loose stainless steel ligature generated lower frictional resistance than plastic ligature in all experimental groups. 5. The following factors affected friction in decreasing order; wire material ligature type, and load.
