1.Musculoskeletal Symptoms and Ganglions Developed in Repetitive Job Workers.

BACKGROUND: Musculoskeletal symptoms, such as pain, numbness, and ganglions on involved joints were common problems in repetitive job workers, who exposed to prolonged, repetitive use of the wrist in factory. This study was performed to compare the degree of musculoskeletal symptoms and prevalence of soft tissue mass (esp. ganglion on wrist) in repetitive job group with those of non-repetitive job group and to evaluate the association of development of ganglion and the duration of wrist use. METHODS: Study subjects were 253 workers who work on a brick manufacturing factory and they were composed of 153 non-repetitive job workers and 100 repetitive job workers. Authors conducted a questionnaire survey among workers in the factory and examined their lesions. RESULTS: There were no significant difference statistically in age, level of education and work duration in both groups. It was statistically significant that the degree of wrist use was more frequent in repetitive job group than in non-repetitive job group(P<0.01). Musculoskeletal symptoms, such as pain and numbness on affected joints were more common in repetitive job group than in non-repetitive job group. Affected side was bilateral in three joints and right only in two joints(P<0.01, P<0.05). The prevalence of ganglions in repetitive job group(6 subjects, 6.0%) was much higher statistically than that in the non-repetitive job group(2 subjects, 1.3%). In a comparison of two groups according to the existence of ganglions, no stasistically significant differences were found in age, level of education and work duration. However, duration of wrist use was statistically significant relation with development of wrist ganglions(P<0.05). The 8 subjects with ganglion on wrist all worked on manufacturing part in the factory. The duration of work ranged from two to fourteen years. The affected sides of ganglions on wrist were right in 3 cases and left in 5 cases. Size of ganglions ranged from 0.5 to 3 cm in diameter. Painful symptom in the ganglion was complained by two cases(25%) among 8 cases. CONCLUSIONS: We suggest that prolonged, repetitive activities such as carrying a brick, have a tendency to develop musculoskeletal symptoms and ganglions in the workers. Because high prevalence of ganglions in this factory seems to be an important occupational problem among repetitive job workers, further studies on the factors contributing to development of ganglions in this factory are needed.

3.Bone Graft or Cement Fill for Tibial Defects in Total Knee Arthroplasty

The purpose of this study is to evaluate the results of bone graft or cement fill for tibial bone defect in total knee arthroplasty. 51 knee in 45 patients which had bone graft or cement fill for tibial bone defect of more than 1cm3 in total knee arthroplasty, between March, 1990 and July, 1994 at the department of Orthopedic Surgery in Hanyang University Hospital were followed for average 30 months. Bone grafts were performed in 33 knees and cement fills in 18 knees. The size of tibial defect was measured after removal of sclerotic and dense bone in the tibial plateau. The average size of tibial defect treated by bone graft was 4.9 cm3 in volume, 11 mm in height and the average size of tibial defects treated by cement fills was 1.5cm3 in volume, 6.8 mm in height. The results were as follows ; 1. In true A-P and lateral x-ray checked by fluoroscopic guide, trabecular union was seen in all 33 knees of bone grafts without collapse, loosening and bone resorption. In one of 18 knees of cement fill, radiolucent line was detected 2 mm in width between cement mantle and the adjacent tibial bone. In other 17 knees, radiolucent was seen 1 mm or less in width. 2. According to the system of Knee Society Score, average knee rating score was improved from 23.2 points preoperatively to 90 points postoperatively in bone graft cases, from 23.4 points preoperatively to 93.2 points postoperatively in cement fill cases. Average functional score was improved from 14.8 points preoperatively to 81.5 points postoperatively in bone graft cases, from 13.3 points preoperatively to 81.4 points postoperatively in cement fill cases. 3. Finally, we obtained the good results by cement fill during cement fixation of tibial component or bone graft without cement fixation of tibial component for small tibial bone defect and by bone graft for large tibial bone defect.

4.Comparison of Direct-labeling Method of Antibody with 99mTc and 188Re.

PURPOSE: We investigated the direct labeling method of antibody with 99mTc and 188Re and examined the stability and function of these labeled compounds in in vitro and in vivo. MATERIALS AND METHODS: Disulfide bond of nonspecific human IgG was reduced to -SH group by 2-mercaptoethanol. Stannous ion was used to reduce 99mTc and 188Re. The stability of 99mTc-IgG and 188Re-IgG was estimated upto 24 hrs. Biodistribution was evaluated in abscess bearing rats at 4 and 24 hr post-injection of 99mTc or 188Re labeled IgG. RESULTS: The number of -SH group per reduced IgG molecule was 2.34. The labeling yield of 99mTc-IgG and 188Re-IgG were 90% and 95%, respectively. The stability of 99mTc-IgG at 1, 4, 6 and 24 hr was 91%, 83%, 78%, 7% and that of 188Re-IgG, high uptake was found on kidney, blood, stomach and abscess (9.42+/-0.68, 1.43+/-0.24, 0.86+/-0.18, 0.72+/-0.10 %ID/g, respectively). The uptakes at 24 hr were kidney, abscess, stomach, and blood in descending order. In case of 188Re-IgG, high uptake at 4 hr post injection appeared on kidney, blood, abscess and stomach (3.92+/-0.62, 1.32+/-0.08, 0.88+/-0.01, 0.26+/-0.06, respectively). The upatkes at 24 hr were kidney, abscess, blood abd stomach in descending order. The abscess to blood uptake ratio of 99mTc-IgG was 0.5 at 4 hr and 2.02 at 24 hr and that of 188Re-IgG was 0.67 and 1.29. CONCLUSION: 99mTc-IgG and 188Re-IgG and 188Re-IgG canbe labeled efficiently with direct labeling method. However, 99mTc-IgG and 188Re-IgG, labeled with direct method, was unstable. Further study in needed to enhance the stability of the antibody labeling.

6.Eosinophilic Otitis Media 3 Cases Discovered with Recurrent and Sticky Otorrhea after Ventilation Tube Insertion.

We studied three patients in whom otorrhea occurred without bacterial infection following ventilation tube (VT) insertion. These patients took oral steroids since the conventional therapy was not effective; however, the oral steroids produced temporal effect since sticky otorrhea occurred when the oral steroids were stopped. We collected ear discharge from each patient and consulted the pathologist about histopathologic findings of it. We identified the predominant eosinophilic infiltration at specimen. We tried to relate this type of otitis media with eosinophilic otitis media (EOM) and treated the patients with intratympanic steroid injection.

7.Prevalence of Osteoporosis, Related Factors in 66-Year-Old Women in Korea.

BACKGROUND: With the rapid increase in the elderly population and the ensuing increase in osteoporosis and subsequent fractures, there has been a rise in socioeconomic costs. This study evaluated the relationship between osteoporosis and physical function in 66-year-old Korean women. METHODS: All of the 193 Korean women aged 66 years were recruited from life-transition health examinations from May to December, 2007. Background information was collected through a self-administered questionnaire; and height, weight, and waist circumference were measured. Bone mineral density of the lumbar spine was measured with the Dual Energy X-ray Absorptiometry and the 'timed up and go (TUG)' and 'one-leg balance (OLB)' tests were administered. Risk factors for osteoporosis were analyzed by correlation coefficients and binary logistic regression analysis. RESULTS: The prevalence of osteoporosis for 66-year-old women was 25.9%. A short stature and lower body mass index (BMI) were associated with osteoporosis of the lumbar spine. A longer time to complete the TUG test (>10 second) was associated with a 4-time higher prevalence of lumbar osteoporosis, which was not observed with the OLB test. CONCLUSIONS: One out of four 66-year-old Korean women had osteoporosis. A longer TUG time, short stature, and lower BMI were associated with osteoporosis.

8.Factors affecting the contamination of bag urine culture in febrile children under two years.

PURPOSE: Since children under two years with suspected urinary tract infections (UTIs) cannot control urination, urine cultures in such children are usually performed via urine bags. This method is noninvasive but has a high contamination rate. We studied the contamination rate of bag urine culture in diagnosing UTIs in infants under two years and the factors responsible for contamination. METHODS: We examined patients under 2 years in whom urine culture through the urine bag method yielded over 105 colonies of a single pathogen. We defined UTIs by referring to the guidelines of The Korean Society of Pediatric Nephrology, 2005. We examined the factors responsible for contamination according to sex, duration of urine collection, and whether diarrhea took place with contamination rate. RESULTS: We examined 717 patients (412 males and 305 females). The contamination rate of one bag urine culture was 37.9%. Gender was not related to the contamination rate (P>0.05). Duration of urine collection showed an association with the contamination rate. The longer the duration of collecting urine, the higher was the contamination rate. Duration of urine collection was divided into three groups: first group, <2 hours; second group, 2-4 hours; and third group, > or =4 hours. The contamination rates were 30.0%, 42.2%, and 43.7% for the first, second, and third groups, respectively, with statistical significance (P=0 .001). Diarrhea at admission had no impact on the contamination rate (P>0.05). CONCLUSION: The contamination rate of urine culture in the examined patients was 37.9%. Gender and diarrhea symptoms were not responsible for contamination. In infants with a suspected UTIs, urine should be collected within 2 hours through the urine bag method. If urine collection takes over 2 hours, the urine bag should be resterilized and reattached to the patient.

9.A Clinical Study on Coenzyme Q10(Neuquinon(R)) in the Treatment of Congestive Heart Failure.

Coenzyme Q is concentrated in Golgi apparatus membranes and mitochondria, but not in other membranes. Although it is difficult to prove the metabolic action of coenzyme Q administered exogenously in clinical cases, the effect of this substance can be evaluated by criteria based on clinical findings. In an attempt to evaluate the effect of coenzyme Q for the treatment of 67 patients(male 26 cases, female 41 cases) of congestive heart failure, we administered Coenzyme Q1030mg daily for 4 to 8 weeks. Most of them were valvular heart disease(74.6%) and hypertension (14.9%). Clinical effects were evaluated at least 4 weeks later by the criteria using a scoring method of severity of congestive heart failure which was devised by Ishiyama, etc. In summary, a definite effect was found in 13 cases(19%) and a mild effect was observed in 46 cases(69%). During treatment there were no significant side effects, and also no significant changes in heart rate and blood pressure.

10.Hypotensive Efficacy and Safety of Manidipine on the Patient with Essential Hypertension.

Thirty patient with essential hypertension were administered Manidipine, a new calcium antagonist, 10~20mg once daily to evaluate the hypotensive efficacy and safety for 8 weeks. And the followings were the result. 1) Patients were consists of 14 male, 16 female, aged 53 in average and classified as mild in 21 and moderate in 9 patients. 2) Optimum intial dose was 10mg and 10 to 20mg were the doses recommended. 3) Blood pressure dropped after 8 weeks 24/13mmHg in average, rewarding 80% effectiveness and normalized in 87%. 4) Most frequent side reaction was facial flushing in 5 patiens followed by palpitation and dizziness, all of which did not disturb the continuation of medication. 5) Most of routine laboratory parameter were normal and unchanged between before and after the trial. 6) Overall rating of usefulness was 77%. In conclusion, Manidipine 10 to 20mg once daily regimen is well tolerated and effective in the treatment of mild to moderate essential hypertension.
