3.Changes in Nail Plgmentation with Cancer Chemotherapy.

A clinical observation of the nail pigmentation change was made on 200 patients receiving cancer chemotheray who were seen at; the Department of Internal Medicine, Seoul National University Hospital, from January through May, 1986. The results were as follows: l. Among the 200 patients, 118 cases(59%,) showed nail pigmentation changes. 2. The patterns of nail pigmentation change were as follows: Parallel transverse band(70 cases 59.3%), longitudinal pigmentated band(32 cases, 27. 1%), brown arc(25 cases, 21. 2%), Proximal black pigmentation(10 cases, 8. 5%), diffuse pigmentation(5 cases, 4.2%), parallel transverse white line(5 cases, 4.2%), half and half nail(3 cases, 2. 5%), pigmentations with transverse white band(1 case, 0. 8%). 3. Various cambinations of nail pigmentation pattern were found in 33 patients (28. 0%). 4. 11 cases of the nail dystrophy with nail pigmentation change were observed: Longitudinal ridge(7 cases), transverse groove(3 cases), wavy transverse fissure (1 case).

5.Epidemiologic Survey of the Serlogic Markers of Hepatitis B Virus and Serologic Tests for Syphilis in Institutionalized Persons.

To survey the seroprevalence of hepatit!s B virus(HBV) in relation to serologic tests for syphilis(STS), using VDRL and TPHA test, the tests were done in the seri of 1252 institutionalized persons, who lived in Taegu Hope Village for the poor or crippled, and 1831 controls, who had the regular routine physical examination from March to May, 1987. The positive rates nf HBsAg and anti-HBs were 11.7%(146) and 27.0%(338'). compared with 6.25%(ll) and 18.4%(337) in controls, respectively. The prevalence of HBV including HBsAg and anti-HBs was higher in institutionalized persons (38,7+i) than in controls(24.6%)(p<0.0001). The positive rate of STS was higher in institutionalized persons(7.5%) than in controls(2.9%)(p<0.0001). There was a tvend for a higher rate of seropositivity in women than in men of institution- alized persons in the both diseases. The prevalence of HBV among STS-positive cases wa.s 42.6%(40) of instituti- onalized persons and 26.4%(14) of controls, but not, statistically significant, the gre,ater part of them distributing in 4th and 5th decades. The positive rates of I-tBi.Ag in institutionalized persons was 45.5%(5) of STS-positive cases and 25. 9%,'35) of STS-negative cases. No correlation was found between the serologic ma kers of HBV and VDRL titration.

6.Cutaneous Complications of Cancer Chemotherapy.

A clinical observation of cutaneous complications was made on 200 patients receiving cancer chemotherapy at the Department of Internal Medicine, Seoul National University from January through May, 1986. The results were as follows: 1. Among the 200 patients, 191 case(95. 5%) showed cutaneous complications 2. The cutaneous complications included the following; hyperpigmentation(14l cases, 70. 5%), alopecia(138 cases, 69.4%), nail change(118 cases, 59.0%), mucositis(47 cases, 23.5%), dryness of the skin(40 cases, 20.0%), seborrheic dermatitis(24 cases, 12. 2%), increase of seborrheic keratosis(11 cases, 5.6%), folliculitis or acneiform eruptions(9 cases, 4,5%), melasma(6 cases, 3.0%), gynecomastia(3 cases, 1.5%), vessel hardening or dimpling(3 cases, 1.5%), radiation recall(2 cases, 1.0%), hyperhydrosis(2 cases), photosensitivity(1 case, 0.5%), tissue necrosis(1 case), facial flushing(1 case), purpura(1 case) and obesity(1 case), 3 Steps were taken to determine the chemotherapeutic agents causing these cutaneous complications, though in some cases it was difficult in determining exaetlr which chemotherapeutic agent was the cause of the observed cutaneous complication.

7.Vascularized Osteocutaneous Fibular Transfer

Only several stages of conventional operation have been able to treat the open fracture of the limb with extensive bone and soft tissue loss. So we, authors, reviewed 25 cases of vascularized osteocutaneous fibulat transfer which were performed to the extensive bone defect of the limb and associated soft tissue injury(from May 13, 1982 to September 30, 1988). In 25 cases, one case was a chronic osteomyelitis combined pseudocarcinomatous epithelial hyperplasia of surrounding soft tissue, the other(24 cases) were the open comminuted fractures with the extensive bone and soft tissue loss of the limbs. The average size of the skin flap was 12.33 × 5.83cm, the average length of vascularized fibula was 16.5cm in length. The complications were fractures of grafted fibula, partial necrosis of both ends of the skin flap and uncontrolled infection of proximal end of grafted fibula. During postoperative period, the patency of the anastomoses were monitored by observing the color of the skin of buoy skin flap. The average amounts hypertrophied fibula at 12 months after operation were 20.6cm. And the following remarkable result were obtained. 1. With the uncontrolled bone infection, the vascularized osteocutaneous fibular transfer was able to be performed. 2. During the follow up period, the grafted fibula had been hypertrophied. 3. In roentgenogram, as compared with conventional bone graft, grafted bone was not resorbed and early united. 4. In adult, free fibula can be obtained and grafted as 23cm as long and the skin flap with fibula can be obtained and grafted as 21 × 7cm as large in or experience. 5. In the vascularized osteocutaneous fibula transfer, the patency of anastomoses was monitored indirectly by confirming skin flap. 6. The fractured fibula was united early by cast immobilization, and it was hypertrophied at the fracture site. 7. The vascularized osteocutaneous fibula transfer is also useful in application of the other long bones.

8.Arthroscopic Excision of the Complete Type of Suprapatellar Plica.

The suprapatellar plica is a remnant of the embryonic septum separating the suprapatellar pouch from the knee joint. Complete type of suprapatellar plica is rarely reported and its clinlical significance is controversial. The purpose of this study is to verify the complete type of suprapatellar plica as an important cause of anterior knee pain and to evaluate the clinical results after arthroscopic excisien of it. From September 1991 to April 1997, we studied prospectively 54 patients, 90 knees which were diagnosed as cornplete type of suprapatellar plica preoperatively. The mean age was 32 years (15-57 years) and sex distribution was 14 males and 40 females. For the preoperative diagnosis, we checked clinical symptom, past history, physical finding and plain X-ray routinely and performed bone scan, double contrast arthrogram and MRI, if necessary. All patients complained vague anterior knee pain at standing, sitting, stair climbing and exercise for long times. All 90 knees were examined arthroscopically. Among them, 80 knees had complete type of suprapatellar plica and remaining 10 knees had other findings. After all, positive predictive value of preoperative diagnosis for complete type of suprapatellar plica was 89%. Among 36 patients diagnosed as bilateral preoperatively, 33 patients(92%) were confirmed as bilateral in arthroscopic examination. After arthroscopic excision of plica, clinical results were total relief of pain in 19 knees(24%), improvement in 55 knees(69%), no change in 6 knees(7%) and none of worsening. In conclusion, complete type of suprapatellar plica should be considered as an important cause of anterior knee pain especially which is vague, intermittent and bilateral.

9.The Efficacy of MRI in Anterior Cruciate Ligamant Injury.

Ligament injuries of the knee joint are very common among competitive athletes. Acrurate assessment ot the nature of these injuries is a prerequisite for appropriate therapy. The purpose of this study is to evaluate the significance of MRI findings in assesing the status of ACL tear and associated injuries. 31 cases of ACL injuries were examined and reconstructed arthroscopically from Jan. 1997 to Sep. 1997 at Kyung Hee University Hospital. 1n all cases, we reviewed preoperative physical and MRI findings and performed postoperative radiologic assessments. Results were as follows; 1. Lachman test and Pivot shift test were positive in all 31 cases but anterior drawer test was positive in 29 case,s. 2. In MRI findings, 24 cases were interpretated as complete ACL tear and / cases as inccomplete ACL tear. But these 7 cases were confirmed as complete tear in mthroscopic exanunation with careful probing of ACL fiher. 3. MRI showed associated meniscal injuries in 19 cases(61%)-14 cases(74%) at medial meniscus, 5 cases(26%) at lateral meniscus. All cases were confirmed arthroscopically. In conc1usion, physical findings are more reliable than MRI findings in diagnosing the complete ACL tear and deciding the ACL reconstruction. But MRI findings are very helpful to assess the associated injuries and plan the treatments.
