Three cases of spondyIolysis were experienced and treated by anterior interbody fusion of the involved spines at the department of orthopaedic surgery in the Taegu Hospital One case was accompanied with severe lumbago with radiating pain and other two cases with lumbago significantly. Review of literature was done with report of three cases of spondylolysis.

2.Cloning and Sequencing Analysis of the Gene Encoding the 66-kDa Protein in Borrelia hermsii.

The gene encoding the 66 kilodalton (kDa) protein of Borrelia hermsii HS1 was cloned and sequenced. Chromosomal DNA was prepared from purified B. hermsii and used in construction of genomic library. The library was screened for positive clones by 314 bp DIG-labeled probe synthesized on the basis of the part of the sequence of B. hermsii. Positive clone was subcloned into p2ErO vector and was designated as pBH11. pBH11 were subcloned into pBluscript vector and were designated as pBH11-1 (500 bp), pBH11-2 (800 bp), pBH11-3 (600 bp) and pBH11-4 (800 bp). The plasmids were sequenced and determined the nucleotide sequence of p66. The open reading frame of the p66 consisted of 1803 base pairs coding for 600 amino acid protein. The basic information on the p66 gene of B. hermsii HS1 obtained from this study will be useful for further analysis and experiment of pathogenesis of the borrelia.

3.New three-dimensional cephalometric analyses among adults with a skeletal Class I pattern and normal occlusion.

OBJECTIVE: The purpose of this study was to assess new three-dimensional (3D) cephalometric variables, and to evaluate the relationships among skeletal and dentoalveolar variables through 3D cephalometric analysis. METHODS: Cone-beam computed tomography (CBCT) scans were acquired from 38 young adults (18 men and 20 women; 22.6 +/- 3.2 years) with normal occlusion. Thirty-five landmarks were digitized on the 3D-rendered views. Several measurements were obtained for selected landmarks. Correlations among different variables were calculated by means of Pearson's correlation coefficient values. RESULTS: The body of the mandible had a longer curve length in men (102.3 +/- 4.4 mm) than in women (94.5 +/- 4.7 mm) (p < 0.001), but there was no significant difference in the maxillary basal curve length. Men had significantly larger facial dimensions, whereas women had a larger gonial angle (117.0 +/- 4.0 vs. 113.8 +/- 3.3; p < 0.001). Strong-to-moderate correlation values were found among the vertical and transverse variables (r = 0.71 to 0.51). CONCLUSIONS: The normative values of new 3D cephalometric parameters, including the maxillary and mandibular curve length, were obtained. Strong-to-moderate correlation values were found among several vertical and transverse variables through 3D cephalometric analysis. This method of cephalometric analyses can be useful in diagnosis and treatment planning for patients with dentofacial deformities.

4.Effects of calcium antagonists on contractions of chorionic arteries in normal and preeclampsia placenta.

This study was undertaken to observe the effects of organic or inorganic calcium antagonists and to investigate the involvement of cyclic nucleotides in regulating the vascular tone in the chorionic artery from normal or preeclamptic placenta. KCI and prostaglandin (PG) F2 alpha produced marked and constant contractions in chorionic arterial preparations of both normal and preeclamptic placentas. Nifedipine (NIF), verapamil (VER) and diltiazem (DIL) reduced the tension that had been produced by KCI and PGF2 alpha in a concentration-dependent fashion in both preparations, and the potency order of the three agents was NIF> VER > DIL. In preeclamptic arteries, however, the magnitudes of vasodilatation induced by NIF and DIL were much smaller than those in normal chorionic arteries. Mg2+ and Cd2+ also relaxed the tension induced by KCI and PGF2 alpha. In preeclamptic chorionic artery, the vasodilatation induced by Mg2+ was significantly potentiated, while that by Cd2+ was not. Removing endothelium did not alter cyclic GMP content in both preparations. In both preparations contracted by PGF2 alpha, nitroprusside markedly increased cyclic GMP content, but neither cyclic GMP nor cyclic AMP content was affected by acetylcholine, NIF, isopro-terenol, or Mg2+. The above results suggest that neither cyclic AMP nor cyclic GMP is involved in regulating the vascular tone of chorionic artery and that sensitivity of the artery in preeclampsia to the inhibitory action of calcium antagonist might be different from that in normal placenta.

5.Immunoblastic Sarcoma Arising in Angiommunoblastic Lymphadenopathy: A case report.

Angioimmunoblastic lymphadenopathy (AILD) is a systemic disease clinically characterized by fever, generalized lymphadenopathy, hepatosplenomegaly, polyclonal gamma-globulinemia and Cooms' positive hemolytic anemia. The lymph node in AILD reveals a polymorphic feature consisting of a proliferation of small vessels, immunoblasts and plasma cells and acidophilic interstitial material. Progression into immunoblastic sarcoma is reported as high 35% of the patient with AILD. Nathwani et al have observed not only malignant transformation of AILD in sequential tissue examination, but also the coexistence of AILD and immunoblastic lymphoma in the same lymph node or at different sites in the same patient. Multiple clusters or islands of compactly arranged large lymphoid cells constitute the initial histologic evidence of immunoblastic sarcoma. Immunoblastic sarcoma is a large cell lymphoma conceptually related to transformed T-and B-lymphocytes of the extrafollicular compartment of the immune system, which proignosis is poor. We have recently experienced a case of immuno blastic sarcoma arising in angioimmunoblastic lymphadenopathy in a 24-year-old woman. She had history of multiple enlarged lymph nodes in the inguinal, axilla and supraclavicular areas. Previous lymph node biopsies revealed reactive change. Six month later, right axillary lymph node biopsy reveled AILD with focal clusters of immunoblasts. Subsequent lymph node biopsy at the same site revealed diffuse immunoblasic sarcoma, B-cell type. A case presentation with histologic findings and a brief review of literature were done.

7.A study on the skeletal and dentoalveolar dimensions in relation to vertical facial patterns.

The purpose of this study was to investigate the variations of the skeletal and dentoalveolar dimensions in relation to vertical facial patterns. Lateral cephalogram of 200 cases (100 cases of male and 100 cases of female, average age of which was 23.2 years) were traced and some measurements of skeletal and dentoalveolar dimensions were measured. The ratio of UAFH/LAFH was employed to classify the samples into groups of excess and short lower anterior facial height. And the comparison between two groups were taken statistacally. The following results were obtained. 1. The dentoalveolar height, lower anterior facial height, lower gonial angle, and FMA in the excess-lower-anterior-facial-height group were significantly larger than those in short-lower-anterior-facial-height group. 2. The dentoalveolar height, ramus height, and Jarabak ratio in the male subjects were significantly larger than those in the female subjects. 3. The UAFH/LAFH ratio showed a significant correlation to upper, lower facial height, AUDH, PUDH, ALDH, PLDH, Lower gonial angle, FMA, and Bjok`s Sum.

8.Doppler Sonography of the Kidney: Differentiation of Obstructive and Nonobstructive Hydronephrosis.

PURPOSE: The purpose of this article is to study whether the Doppler ultrasound differentiate obstructive hydronephrosis from nonobstructive hydronephrosis in sonographically diagnosed hydronephrosis. MATERIALS AND METHODS: We studied 35 kidneys of 27 patients with hydronephrosis which were diagnosed with sonography. Resistive indices were calculated from Doppler waveforms at the level of interlobar or arcuate arteries. We divided the materials into two groups with obstructive and nonobstructive hydronephrosis according to the findings of intravenous pyelography, retrograde pyelography, and anterograde pyelography. These were compared with normal control group(20 kidneys in 10 persons). We also grouped patients regarding to the duration after onset of clinical symptom, and graded the hydronephrosis into three group according to sonographic findings. We analyzed the correlations of the above sonography and clinical findings with resistive index. Hydronephrosis was obstructive in 20 kidneys(in 17 patients) and nonobstructive in 15 kidneys(in 10 patients). RESULTS: There was statistically significant difference between the mean resistive indices in obstructive hydronephrosis(0.74 +/- 0.02) and that in nonobstructive hydronephrosis (0.60 +/- 0.02)(p<0.05). Also there was no difference between the mean resistive indeces in nonobstructive hydronephrosis and that in normal control group(0.59 +/- 0.07)(P > or = 0.05). CONCLUSION: The use of Doppler sonography can be helpful in detecting the presence or absence of true obstruction in patients with hydronephrosis.

9.A Case of Neonatal Graveses Disease.

Neonatal Gaves disease is a relatively rare condition due to transplacental passage of Thyroid-stimulating antibody(TSAb) from a mother with active or inactive Graveses disease or autoimmune thyroiditis. A 11-day-old female newborn was referred to our department of pediatrics from a local clinic because of low level T4(3.55microg/dl) concurrent with high level TSH (501.74uIU/ml) on the 5th day neonatal metabolic screening. But, our repeated laboratory data showed very high serum T4(59.6microg/dl), T3(1,600ng/dl), suppressed TSH(0.43uIU/ml), and the presence of TSH receptor antibody. Her mother was treated with propylthiouracil(PTU) for Graves disease during pregnancy. Therefore, we thought it was a delayed-onset neonatal hyperthyroidism, because the fetal thyroid gland was initially suppressed by antithyroid drug taken during pregnancy. After initiating antithyroid drug therapy for the hyperthyroid nature, TSH levels became elevated again, while thyroid hormone levels decreased. Maternal and infant blood samples at the 23th day after birth were examined for serum autoantibodies directed towards the TSH receptor(Thyrotropin-binding inhibitory immunoglobulin:TBII, Thyroid-stimulating antibody:TSAb, Thyroid-stimulating blocking antibody:TSBAb) and high levels of TBII and TSAb were detected. About 2 months after birth, TBII and TSAb decreased within normal limit, and then we could stop antithyroid medication in safety. We report here a case of neonatal Graveses disease with very high level of T4 and T3, but firstly presented as hypothyroid nature on neonatal screening because of the maternally transferred antithyroid drug, PTU.

10.Relationship between Severity of Dementia and Behavioral Psychopathology in Probable Alzheimer's Disease.

Behavioral problems are thought to be pervasive and devasting to patients with dementia of the Alzheimer's type and their families. Despite this, little empirical data are available concerning the nature of such impairments, their rate of occurrence or their relationship to the disease process. This study was designed to investigate the nature and the rates of behavioral disturbance among DAT patients. The subjects involved in this study were 72 patients who were hospitalized and had primary diagnosis of dementia of the Alzheimer's type. Severity of the dementia was assessed by MMSE, and abnormal behavior by E-BEHAVE-AD. Pearson correalation was used to analysis data. The subjects were divided into mild/moderate dementia group and severe, and then evaluated significant differences. Independent-samples T-test was used to analysis data. The results were as follows: 1) Wandering and delusional ideation were associated with declining MMSE score 2) Wandering was only differenciated between mild to moderate group. In conclusion, problems found associated with level of impairment such as wandering and delusion are thought to be characteristic of the disease and therefore predictable. Problems found not associated with level of impairment, are likely to be idiosyncratic. The former should probably be incorporated into education and intervention programs, the latter addressed as needed on an individual basis.
