Gastric volvulus is a rare surgical disorder in the pediatric population. We experienced a case of gastric volvulus. A 2-year-old boy was admitted to hospital with abdominal distension. An upper gastrointestinal series showed reversal of the greater and lesser curvatures. Surgical exploration revealed an organoaxial volvulus of the stomach, and anterior gastropexy was performed.

3.Urinary Tract Stone in Patients with Spinal Cord Injury: A Retrospective Radiological Study.

PURPOSE: To compare the incidence between author's first and current report on urinary tract stone in patient with spinal cord injury and to evaluate the effectiveness of recent developed in medical technology and care on in treating the patients. MATERIALS AND METHODS: We reviewed urinary tract stone in 257 patients with paraplegia or quadriplegia after spinal cord injury. These patients were diagnosed retrospectively by KUB and intravenous urography at the Korea Veterans Hospital during 10years from January, 1984 to December, 1993. We evaluated and compared the overall incidence, incidence of specific location of urinary tract, recurrent rate, incidence according to the level of spinal cord injury, and the duration of development in urinary tract stone. RESULTS: Total patients were 257 with 186(72.4%) paraplegia and 71(27.6%)quadriplegia. Overall incidence of the stone was 16.0% in this study and 38.1% in the first study. Incidence of the stone in individual organ ;5.5% in kidney, 1.2% in ureter, and 13.6% in urinary bladder. The recurrent rate was 29.3% in this study and 40.6% in the first study. Incidence of the stone according to the level of spinal cord injury was as follows;15.6% in cervix, 17.1% in upper thorax, 17.9% in lower thorax and 13.9% in lumbar. The stone developed during the first 4 years and between 12 to 16 years following spinal cord injury was 28.3% each. CONCLUSION: Overall incidence and recurrent rate of urinary tract stone was obviously decreased since the first study. Highest incidence of the stone occurred in urinary bladder and in patient with lower thoracic spinal cord injury, which is similar to first report. Peak incidence of the stone was in the first 4years, and another peak was in 12-16years after spinal cord injury. The decreased overall incidence of urinary tract stone maybe attributable to the development in medical technology and care, and active rehabilitation.

5.Thumb Reconstruction with a Free Neurovascular Wrap

On the length, strength, free lateral motion, and perfect mobility of the thumb, depends the power of the human hand. The thumb is called pollex because of its strength and that strength is necessary to the power of the hand being equal to that of all the fingers."So stated Sir Chales Bell in the fourth Bridgewater Treatise, first published in 1833. Morrison and O'Brien advocated reconstruction of the thumb with a free wrap around flap from the big toe to recreate a stable, sensate and functional digit including the nail in 1980. From March, 1982 to July, 1988, Twelve thumb reconstructions were performed using the wrap around procedure at Hanyang University Hospital;12 successful thumb reconstructions were reviewed at an average of 16 months after surgery. There were several complications;these included skin necrosis(4), malunion(1), resorption of the bone graft(7). We considered that cosmesis and function were good;pinch grip averaged 68% of normal, sensibility returned in all patients as assessed by two-point discrimination(average 9.5mm). Over all the results are as follows. 1. The wrap around technic provides a good method of reconstruction. 2. The wrap around technic is one stage operation and can be used in the level of metacarpal amputation. 3. The surgeon should be familiar with microsurgical technic.

8.A comparison of the efficiency of the three egg count techniques.

Usually egg count technique has been employed to know the intensity of infestation of human intestinal helminths and to elucidate the effects of anthelmintics. Comparing the three egg count methods - Stoll's egg dilution method, Beaver's direct smear method and FHK counting board method - the authors reached the following results: As the result of application of the three methods to 75 Ascaris egg positives and 95 Trichocephalus egg positives, in the case of Ascaris eggs, the egg detection rate was 99 per cent by Stoll's method, 97 per cent by Beaver's method and was lowest by FHK method at 79 percent. Meanwhile, in Trichocephalus eggs, Stoll's method revealed 92 percent, Beaver's method 95 percent, and FHK method 95 percent, there being no noticeable differences. With regard to EPG, in counting Ascaris eggs, Stoll's method showed a value closest to the actual number of eggs, and the value revealed by FHK method was smaller than the actual number. As the result of comparing the coefficiencies of variation of the three egg counting methods, both in Ascaris eggs and Trichocephalus eggs, the FHK method showed the smallest, followed by Stoll's method and Beaver's method.

10.An Analysis of the Admitted Geriatric Orthopedic Patients Over the Age of 70

The geriatric population tends to increase, worldwidely, and the same in orthopedic fields. In this age group, senile changes, both physical and mental, appear. In addition, psychiatric problems due to retirement from the job and changes of the role in his society and in the family, are included. The geriatric orthopedics is peculiar and its importance is increasing, which makes great demands on both medical and surgical skills as well as an enthusiastic teamwork. Fifty six patients over the age of 70, who were admitted and treated at the Department of Orthopedic Surgery, Seoul National University Hospital, between 1986 to 1987, were analysed. The results were as follows. 1. Thirth four patiens were female and twenty two were male. 2. The fracture was the most common cause, and degenerative arthritis, tumor are the next causes. 3. Operation was performed in 38 cases. 4. The associated diseases were hypertension, diabetes, U.T.I., renal failure, poor sight, pulmonary and cardiovascular diseases, which were more common than other age group. 5. Intra or postoperative complications were hypertension, liver function abnormalities, renal and respiratory failures and et al., which were also more common. 6. Old age is a major risk factor for surgery, but is not a contraindication, so careful evaluation and managements are needed, which can be done by specialized geriatric orthopedic team.
