1.Chromosomal Vulnerability to gamma-irradiation of Peripheral Blood Lymphocytes in Patients with Various Cancers.

BACKGROUND: Human cancel can be induced by various environmental factors such as virus, chemicals, and radiations. However, susceptibility of host to these environmental factors is not well studied. The purpose of this study was to investigate the chromosomal vulnerability to gamma-1rradiation of peripheral blood lymphocytes in patients with luring, gastric, and liver cancer. METHODS: Micronuclei (MN) test was done in 15 patients with gastric cancer, 18 patients with lung cancer, and 20 normal controls. Sister chromatid exchange (SCE) test was done in 13 patients with hepatocellular cancer and 14 normal controls. RESULTS: The baseline frequency of MN before irradiation in patients with lung cancer and gastric cancel was significantly higher than in controls (47+/-8, 73+/-9, and 8+/-l, respectively; p<0.01). After gamma-irradiation, the frequency of MN was also significantly higher than in controls (223+/-19, 269+/-43, and 285+/-56, respectively; p<0.05). The frequency of SCE in Patients with hepatocellular cancer was significantly higher her than in controls (9.0+/-0.6, 4.3+/-0.7, respectively; p<0.001). CONCLUSIONS: From this preliminay data, we concluded that chromosomes of peripheyal blood lymphocytes in patients with various cancers were more vulnerable to gamma-irradiation as compared to normal controls.

5.Relationships between Prostate-Specific Antigen, Prostate Volume, and Components of Metabolic Syndrome in Healthy Korean Men.

PURPOSE: Metabolic syndrome (MS) plays a potential role in the etiology of benign prostatic hyperplasia (BPH). Recent studies have reported on an association between MS and BPH. However, there has been no consensus on recent results. This study was conducted to evaluate the associations among prostate-specific antigen (PSA), prostate volume (PV), and metabolic components in men who visited our health promotion center. MATERIALS AND METHODS: During the period from January 2005 to December 2010, 521 consecutive men (age range, 40 to 70 years) who underwent transrectal ultrasonography were enrolled in this retrospective study. The health screening program includes blood pressure, body measurements (height, weight, waist circumference, body mass index), biochemical analysis (serum glucose, total cholesterol, triglycerides, high-density and low-density lipoprotein cholesterol, fasting plasma glucose, tumor markers), stool and urine analysis, and a detailed clinical examination. RESULTS: The serum PSA level and PV were significantly higher in patients with MS than in patients without MS, retrospectively (p<0.001, p<0.001). Patients with more than one metabolic component were significantly more likely to have a larger PV and higher serum PSA level. The serum PSA level and PV were increased in a similar manner with the increasing sum of MS components (p<0.0001, p<0.0001). CONCLUSIONS: The MS components were associated with larger PV and higher serum PSA level. Therefore, each MS component could be an important factor in BPH development and management.

7.Separate Vertical Wiring for the Fixation of Comminuted Fractures of the Inferior Pole of the Patella.

PURPOSE: Among patients over 50 years of age, separate vertical wiring alone may be insufficient for fixation of fractures of the inferior pole of the patella. Therefore, mechanical and clinical studies were performed in patients over the age of 50 to test the strength of augmentation of separate vertical wiring with cerclage wire (i.e., combined technique). MATERIALS AND METHODS: Multiple osteotomies were performed to create four-part fractures in the inferior poles of eight pairs of cadaveric patellae. One patella from each pair was fixed with the separate wiring technique, while the other patella was fixed with a combined technique. The ultimate load to failure and stiffness of the fixation were subsequently measured. In a clinical study of 21 patients (average age of 64 years), comminuted fractures of the inferior pole of the patellae were treated using the combined technique. Operative parameters were recorded from which post-operative outcomes were evaluated. RESULTS: For cadaveric patellae, whose mean age was 69 years, the mean ultimate loads to failure for the separate vertical wiring technique and the combined technique were 216.4+/-72.4 N and 324.9+/-50.6 N, respectively (p=0.012). The mean stiffness for the separate vertical wiring technique and the combined technique was 241.1+/-68.5 N/mm and 340.8+/-45.3 N/mm, respectively (p=0.012). In the clinical study, the mean clinical score at final follow-up was 28.1 points. CONCLUSION: Augmentation of separate vertical wiring with cerclage wire provides enough strength for protected early exercise of the knee joint and uneventful healing.

8.Th1 Cytokine ( IFN-gamma ) Secretion Pattern of Peripheral Blood Mononuclear Cells Response to Trophoblast Antigen in Women with Unexplained Recurrent Spontaneous Abortion and Normal Fertile Controls.

OBJECTIVE: A dichotomous Thl and Th2 cytokine profile has been associated with reproductive failure and success, respectively. The purpose of our study was to determine the levels of Thl cytokine (IFN- y ) secreted by peripheral blood mononuclear cells (PBMCs) form women with unexplained recurrentabortion (URA) and fertile controls in response to trophoblast antigen. METHODS: PBMCs were isolated from 30 nonpregnant women with URA and from 10 nonpregnant fertile controls. Following 4 days of culture (1 * 10(6) cells/mL) with and without a protein extract derived from a trophoblast cell line (30 ug/mL, protein). None of the women had allergies, atopy or recent infection. Cytokines were measured in supernatants with enzyme-linked immunosorbent assay (ELISA) kits. IFN- r kit was obtained from BOISOURCE (lower limit of sensitivity, 15.6 pg/mL for IFN- r ). All values below the lowest limit of sensitivity as determined by test kit standards were considered negative. The cytokine stimulation test is considered positive if the IFN- r concentration increases by 200% or more with the trophoblast antigen stimulation. Datas are presented as mean+ SEM. Nonparametric testing (Mann-Whitney U) was used for analysis with P<0.05 considered statistically significant. RESULTS: The Thl-type cytokine (IFN- r ) was detected in 20(67%) of 30 supernatants from women with URA. In contrast, 2 (20%) of trophoblast-activated PBMC culture supernatants from the 10 parus women with normal reproductive histories was detected IFN- r and but were significantly lower than levels in women with URA who had secreted IFN- r upon trophoblast stimulation (99.80+ 18.17 pg/mL versus 166.47 + 36.96 pg/mL, p<0.05). Spontaneous secretion of IFN- r was significantly higher in culture supernatants from women with URA than in supernatants from women with successful reproductive histories (41.36.09+6.99 pg/mL versus 25.89+9.34 pg/mL, p<0.05). CONCLUSION: These data indicate that there are significant differences between women with URA and women with normal reproductive histories in their regulation of the Thl-cytokine (IFN- r) in response to trophoblast. Thl-type immunity to trophoblast is associated with URA and may play a role in reproductive failure.

9.Intracranial "De Novo" Aneurysms: Case Report.

There are numerous theories of pathophysiology in intracranial aneurysm, but currently accepted hypothesis was congenital defect of medial layer of vascular wall and the major developing factor was hemodynamics. Almost physicians were belived that treatment of intracranial aneurysm was completely through clipping of aneurysmal neck. But many reports were emphasized newly deloped intracranial(De Novo) aneurysm after clipping of initially identified intracranial aneurysm. The authors have same experienced of 2 cases intracranial "Do Novo" aneurysms. We are stress there were never completed treatment of aneurysm that clipping of initially identified intracranial aneurysm and should be attention to "De Novo" aneurysm.

10.Ruptured Intracranial Aneurysms in Patients with Polycystic Kidney Disease: Report of Two Cases.

The authors present two cases of ruptured intercraial aneurysm s associated with nearly total destruction of kidney function due to polycystic kindney in one aged 35 years and the other aged 53 years. One patient had a aneurysm on anterior communicating artery and the other had three aneurysms located in cavenous and bifurcated portions of internal carotid artery, and anterior communicating artery. They were successfully treated by a direct surgical approach. We stress surgical management in a state of good consciousness regardless of poor kidney function, because the risk of early death by rebleeding is seemed to be higher than that by renal failure or others.
