1.Tuberculosis Cutis Orificialis of the Tongue.

A tender, non-healing ulcer of 3 months' duration developed on the tongue tip of a 51-year-old male with active pulmonary tuberculosis. Acid-fast bacilli were grown in the tissue and sputum cultures. The patient was anergic to PPD skin test before the treatment and IL-2 productivity was significantly decreased. Sections from the tongue tip showed ulceration, infiltration of neutrophils and tuberculoid granulornas accompanying caseation necroses in the dermis. The ulcer healed after administration of anti-tuberculosis medication for 2 months.

2.The evaluation of the cervical lymph node metastases using computed tomography in the head and neck cancer

Fity-four patients with head and neck cancers were prospectively examined with the CT to determine its valuein staging cervical lymph nodes metastases and to compare with the clinical staging. The CT staging agreed withthe clinical staging in 61%(33/54), upgraded the clinical staging in 22%(12/54) and downgraded in 17%(9/54). Thepost-surgical confirmation was made in only 11 patients, The possible causes of disagreement between the CT andthe clinical staging, and the diagnostic value of the CT in the evaluation of the cervical nodes metastases werediscussed.

3.Hemorrhagic Chicken Pox.

Primary varicella infection usuelly runs a benign clinical course in the helthy population. However, hemorrhagic chicken pox presents with a very extensive eruption of hemorrhagic vesicles in patients with decreaed platelets or impaired immunity and is accompanied by severe constitutional symptoms. A 7-year-old male was admi due to abdompal pain for 1 day and peneralized vesicular eruption for 5 days. The eruption first appeared on the trunk and then spread to involved face, scalp and extrsmities. The skin rash was compatable with varicella but despite the administration of acclovir intravenously, the vesicular eruption became hemorrhagic. Ten days after admission, having experienced cardisc arrest twice and with his mental state in coms, he was diseharged as his situation was hopelesa. We have experienced a giatient diagnosed as having hemorrhagie, chicken pox complicated by encephalopathy.

6.Mucoceles in Post-operative Maxillary Sinuses: CT and MR Findings.

PURPOSE: This study describes the CT and MR findings of mucoceles occurred in the post-operative maxillary sinuses. MATERIALS AND METHODS: CT and/or MR of 19 maxillary mucoceles in 16 patients who had been operated by CaldwelI-Luc procedure were reviewed. CT scans were performed after contrast enhancement in axial and coronal planes with 5mm thickness. Three cases were studied with a 2.0 T or 0.5 T MRI, which demonstrated multi-compartment lesions. RESULTS: The lesions were bilateral in 3 cases. Compartmentalization of the antral cavity was seen in 7 out of 19 involved sinuses. Surgical bone defect of the anterior wall was the most frequent route of extension (11 cases) and extension into the infratemporal fossa with erosion of posterolateral wall was seen in 7 lesions. In 6 cases, the lesions involved orbit. The expansile and erosive bone changes were localized in every case. In one case with multi-compartment lesion, both T1- and T2-weighted MR images showed different signal intensities in each compartment which represented different protein concentration. CONCLUSION: Post-operative maxillary mucocele showed CT findings of localized erosion and bulging most frequently at the anterior wall in which the bone windows were made during the previous surgery. Post-operative compartmentalization of maxillary antrum may cause eccentric expansion of mucocete and each compartment may show different signal intensities on MR.

7.Analysis of Proximal Tibial Resection Surface Dimention with Korean Total Knee Arthroplasty Specimens.

In shape and dimensions, the tibial plateaus are asymmetric with the larger medial tibial plateau and both have a posterior inclination with respect to the shaft of the tibia. Maximizing tibial coverage is an impotant consideration in total knee arthroplasty to provide stahility and load transfer and to improve long-term survival rate of the implants. Most tihial tray designs are symmetric, but several asymmetric designs are available. We evaluated the proximal tibial resection surface during total knee aithroplasty to delineate the tibial plateaus in korean. After tihial bone cut during 100 TKA procedures, the outline of tibial resection suiface was traced and rotational axis of true tibial component was marked intraoperatively. A line was drawn at the maximal anteroposterior (AP) diameter of lateral tibial condyle with parallel to rotational axis of component, and then a transverse axis was drawn at the midpoint of maximal AP diameter of 1ateral tibial condyle. Anteroposterior 10, 20, 30., 40% and midpoint from the media1 and lateral peripheries were calculated manually. The average AP medial 10, 20, 30 and 40% dimensions were 34.7, 43.8, 48.0 and 43.2mm, respectively. The average AP lateral 10, 20, 30 and 40% dimensions were 30.2, 38.4, 42.0 and 40.9mm, respectively. The ratio of medial/lateral AP dimensions 10, 20, 30 and 40% from periphery were 116.9, 114.7, 114.5 and 106.4%, respectively. From these data, we know the asymmetry of the proxirnal tibia1 plateaus in korean. We hope that rnore data will be ohtained in multicenter studies and it will help us to select tibia1 tray and to design the tibial component in korean. But, more accurate standard measures will he need to minimize an error of measurements.
