Cystic nephroma (CN) is a benign cystic neoplasm composed of mixed epithelial and stromal elements. Less than 200 cases have been reported. We had a patient, a 41-year-old woman, who had a huge typical CN. The patient was admitted for a right renal mass that was found incidentally. On laparaoscopic right nephrectomy, there was an encapsulated 7 cm multilocular cystic mass at the upper pole. Microscopically, the cystic wall was lined by a single layer of low cuboidal or hobnail epithelium without a solid area. The thin septa were composed of bland, ovarian type spindle cells. The main differential diagnoses were mixed epithelial and stromal tumor (MEST), low grade multilocular renal cell carcinoma, and tubulocystic carcinoma. The results of immunohistochemical staining were cytokeratin 7/19(+/+) and CD10(-) in lining epithelium, estrogen receptor/progesterone receptor(+/+) in stromal cells. After surgery, she was free of recurrence for 10 months. We report this rare case and compare it with other cystic renal tumors, especially MEST.

Purpose: The purpose of this study was to explore patients’ awareness and treatment experiences with Chronic Obstructive Pulmonary Disease (COPD) through in-depth interviews. The objective was to develop data to support patients in the successful management of their illness. Methods: Purposeful sampling was used to collect information- rich cases and participants were recruited at a university hospital in Daegu, South Korea. Data collection was done through face to face in-depth individual interviews with 10 patients with COPD from November 2016 to December 2018. Interview began with open-ended questions and each interview lasted on average one hour. All interviews were audio-taped and transcribed verbatim and transcripts were analyzed using qualitative thematic analysis. The findings were verified by the participants. Results: Four themes emerged from data analysis: ‘Feeling like the disease is not my disease,’ ‘Terminology spills like a flood,’ ‘Negligent daily disease-management with relief,’ and ‘Relying on health professionals and family members hoping for healing’. Conclusion Participants do not have a clear understanding of their illness nor feel that they are principal agents of their health and illness for treatment and disease management; instead, they depend mainly on their family members and doctors. The findings elucidated the need for increasing patients’ awareness of the illness. Nurses should develop educational support programs to help COPD patients improve their awareness of their illness and successfully manage their treatment.

BACKGROUND: Repeated 24 hour recall has been considered as a preferred method for obtaining accurate dietary information while time and cost for coding and data processing have been a major barrier for their use in large studies. This burden can be resolved by automating the interview and data processing. However, there has been no report about a computerized interview system for dietary survey in a free-living population in Korea. METHODS: This study attempts to test the feasibility of a newly-developed web-based dietary assessment program, Diet Evaluation System (DES) for subjects in a mixed region of urban and rural areas via wireless internet. We conducted total of 134 interviews, twice for each of 67 subjects of various age. As another aspect of feasibility, the group discussion among interviewers was done. RESULTS: Success rate of total attempted interviews was about 70%. Major reasons for problem with DES were instability of wireless internet and consequent inefficient booting of laptops in some areas. It took 14 minute 56 seconds on average to complete an interview and data processing conducted automatically. Subjects' age and internet environment influenced the DES interview time. The group discussion revealed that one-stop system with DES is fast and convenient assuming good wireless internet environment. CONCLUSIONS: Web-based dietary assessment was feasible in this community nutrition survey. To confirm the feasibility in large scale, studies with more comprehensive area and subjects are needed with various wireless condition.

Secondary amyloidosis is a severe complication of refractory rheumatoid arthritis for which no effective treatment exists. Although the benefits of tumor necrosis factor alpha inhibitors in rheumatoid arthritis treatment are well known, their role in renal amyloidosis secondary to rheumatoid arthritis is unclear and their safety in patients with chronic kidney disease is not well reported. We present an unusual case of a 65-year-old female with moderate renal failure and severe proteinuria, who was diagnosed with secondary amyloidosis associated with refractory rheumatoid arthritis subsequent to treatment with corticosteroids, methotrexate, hydroxychlorquine, and leflunomide. She was treated with etanercept 25 mg, administered as a subcutaneous injection twice weekly for 8 months. The patient had no complications following the treatment. Treatment with etanercept led to a decrease in proteinuria and stabilization of renal function over time.
