1.Up-regulation of NMDA Receptor Subunit 2B Induces Degradation of Cytoskeletons in Hypoxic Rat Cerebral Cortex.

In the rat brain, global hypoxia cause a delayed neuronal degeneration that occurs hours to days after reoxygenation. It is generally thought that the ischemic damage is initiated by neurotoxicity mediated through glutamate receptors, particulary NMDA subtypes. Calcium entry through the NMDA receptor is responsible for the synaptic plasiticity and neuronal pathology. Degradation of MAP-2 and NF200, a major components of neuronal cytoskeleton, by Ca2+-dependent protease after NMDA receptor activation has been postulated in delayed neuronal damage. Changes of NR subunit 2B, MAP2 and NF200 in rat brain postsynaptic density[PSD] after hypoxic injury were investigated through immunoblot analyses. To understand the effect of Ca2+ influx through NMDA receptors on neuronal damage which is manifested by cytoskeletal disruption, morphological change was examined through immunohistochemistry and H & E staining. We found that immunoreactivity to NR2B antibody in the cerebral cortex PSD was up-regulated while MAP2 and NF200 was down-regulated at 30 hours after initial hypoxic insult. At this time, morphological changes of neuronal cells in hypoxic conditions were manifested as down-regulation of MAP2 and NF200 immunoreactivities, hyperchromatic condensation of cytoplasm and nucleus, homogenizing cell change, expansion of perineuronal space and dispersion of chromatin. From 3 days, NR2B, MAP2, NF200 were up-regulated simultaneously. On the other hand, morphological alterations in hypoxic neurons were progress further. Our present results suggests that Calcium influx through NR1/NR2B receptor channel is effective whithin 30 hours but ineffective from 30 hours. Delayed neuronal cell death triggered by Ca2+ influx through NR1/NR2B receptor channel within 30 hours, which may activate intracellular profeases. Proteolysis of cytoskeleton by activated protease leads to its abnormal reorganization and eventually damages normal function of cell membrane which causes neuronal cell death.

2.Effects of Transient Cerebral Ischemia on the Expression of N-methyl-D-aspartate receptor subunit 2A, 2B and Neurofilament 200 in the Rat Cerebral Cortex.

Transient cerebral ischemia was induced by bilateral common carotid artery ligation with reperfusion to understand its effect on the expression of NMDA receptor subunits 2A (NR2A), 2B (NR2B), and NF200 The changes of the expressions of NR2A, NR2B, and NF200 in cerebral postsynaptic density (PSD) were evaluated through immunoblot analyses. The expressions of NR2A and NF200 were markedly decreased until 18 hours after reperfusion, while that of NR2B was increased. The immunohistochemistry with NFIGO antibody showed that NF200 protein, which is a marker for neuronal damage, was also significantly decreased at this time point indicating neuronal damages, and the morphological damages of neuronal cells were evident by hyperchromatic condensation of nucleus, irregular cell membrane, displacement of nucleus, and chromatolysis of Nissl substances in toluidine blue stain. However, from 18 hours to 3 day after reperfusion, immunoblot analyses showed that NF200 was increased significantly, while the expression of NR2A were recovered to the control level and that of NR2B was returned to somewhat higher level than control. The NR1/NR2B-type receptor is known to have a longer offset decay time than NR1/NR2A-type ones, and to be more potent in Ca2 influxing. Therefore, our results suggest that, until 18 hours, neurons are damaged by overinflux of Ca2 through NR1/NR2B receptors which helps to degrade NF200 by Ca2 sensitive professes resulting in damages to intracellular transport. The fact that the expression of NF200 was increased even though the NR2A and NR2B are control level during 18 hours to 3 days after damage suggests that NMDA receptor subunits expressed at this time may not form functional receptors. The worsening of some neuronal damages after 3 days may indicate that an abnormal reorganization of elevated NF200 between 18 hours to 3 days further disturb intracellular transport and functions of cell membrane which cause cell death.

3.NR1/NR2B receptor Activation induces Cytoskeletal Proteolysis in Partial Ischemic Rat Hippocampal Formation.

It is generally thought that the ischemic damage is initiated by neurotoxicity mediated through glutamate receptors, particulaly NMDA subtypes. Calcium entry through the NMDA receptor is responsible for the synaptic plasiticity and neuronal pathology. Degradation of MAP-2 and NF200, a major components of neuronal cytoskeleton, by Ca2+-dependent protease after NMDA receptor activation has been postulated in delayed neuronal damage. Calcium-activated protease calpain, excessive degradation of MAP-2, together with the calpain-sensitive microtubule and neurofilaments, would be expected to disrupt intracellular transport- and membrane-related functions that is vital to neurons. Changed of NR subunit 2A, 2B, MAP2 and NF200 in rat hippncampal postsynaptic density[PSD] after partial ischemic injury were investigated though immunoblot analyses. To understand the effect of Ca2+, influx through NMDA receptors on neuronal damage which is manifested by cytoskeletal disruption, morphological change was examined through immunohistochemistry and routine staining method. We found that immunoreactivity to NR2B receptor subuit in the hippocampal formation PSD was upregulated while MAP2 and NF200 was down-regulted at 18 hours after initial partial ischemic insult. On the other hand, morphological changes of neuronal cell in partial ischemic conditions were manifested as eosinophilic inclusion bodies in the cytoplasm which is progression of neuronal damage after 6 days. Calcium influx through NR1/NR2B receptor channel may activate intracellular proteases which would degrade cytoskeleton. Proteolysis of cytoskeleton leads to its reorganization and eventually damages normal function of cell membrane which cause neuronal cell death.

4.A Breast Cancer in Premenopausal Women -Pathologic Findings and an Analysis of Prognostic Factor-.

PURPOSE: The relationship between menopausal status at diagnosis and the prognosis in breast carcinoma remains uncertain. However, it is widely considered that breast cancer in young women is more lethal than in older patients. We therefore attempted to determine whether menopausal status could be a useful prognostic factor for breast cancer. METHODS: A retrospective study was conducted of premenopausal women who had undergone a definite operation between Jan. 1997 and Dec. 1998 in the Department of Surgery, Samsung Medical Center. Clinical features, histopathologic findings, and prognostic factors were evaluated and compared with those for the equivalent surgical group of postmenopausal women. RESULTS: There were 207 cases (86.3%) of infiltrating ductal carcinomas, 10 (4.2%) of infiltrating lobular carcinomas, 6 (2.5%) of ductal carcinomas in situ, and 16 (6.7%) of special type cancers which showed good prognosis. There were some differences in these incidences from those of the postmenopausal women, but they were not statistically significant (P>0.05). Tumor size and lymph nodal status showed no difference between the two groups (P=0.288), nor were there any significant differences in terms of TNM stage, ER/PR status, nuclear or histologic grade (P>0.05). CONCLUSION: There were little differences in pathologic and prognostic factors between premenopausal and postme no-pausal breast cancer patients. Premenopausal status and young age did not have poorer prognostic factors and were predicted to have not worse prognosis.

6.Factors Influencing Performance of End-of-life Care by ICU Nurses.

PURPOSE: This study was a descriptive research to assess the level of and the relationship of related factors in the performance of end-of-life care by intensive care unit (ICU) nurses. METHODS: Participants were 238 ICU nurses from university and general hospitals. Data were collected, using structured questionnaires, and collected data were analyzed using t-test, ANOVA, Scheffé test, Pearson correlation coefficients, and multiple regression analysis with SPSS/WIN 22.0. RESULTS: There were significantly positive effects between performance of end-of-life care and end-of-life care attitudes. Performance end-of-life care was negatively associated with end-of-life care stress and obstacles. Factors that significantly influenced ICU nurses' performance of end-of-life were end-of-life care stress, medical team in obstacles related to end-of-life care, and end-of-life care attitudes, which explained about 53% of the variance in the performance of end-of-life care. CONCLUSION: Findings indicate that hospital organizations should carry out stress management and counseling programs in order to lower ICU nurses' end-of-life care stress, and to enhance end-of-life care attitudes. In addition, concern needs to be given to the medical team which was one of the obstacles to end-of-life care. It is also necessary to decrease the heavy workload and increase the communication with medical workers.

7.The Changes of the Left Ventricular Contractility and the Cardiac Vector of the Pilot by the Flight Time.

To study on the left ventricular contractility and the cardiac vector of fighter pilots, 56 pilots and 15 normal cadets were selected and they were divided into six groups by flight time (hours). To evaluate the left ventricu-lar contractility of subjects, R amplitude of chest V1 , V5 , V6 lead were recorded by ECG, and R-Z interval were recorded by ICG (impedance cardiography). And, to evaluate the cardiac vector of subjects, mean QRS vec-tor of I, II, V2 , V6 lead were recored by ECG. Results of this study are following, 1) Systolic and diastolic blood pressure were decreased against flight time. 2) Sv1 +Rv5 was decreased against flight time; Sv1 +Rv5 was 27.8 mm in 50 hr Group that is the longest among the group, and 23.7 mm in 2,000 hr Group that is the shortest among the groups. V6 /V5 was no significant difference among the groups. 3) R-Z interval was decreased against flight time; R-Z interval was 166.8 X10(-3) sec in 50 hr Group that is the longest among the group, and 154.4 X10(-3) sec in 2,000 hr Group that is the shortest among the groups. 4) In frontal plan, the range of QRS vector axis was +57.3 degrees ~ +78.2 degrees that was deviated to left of body center according to the flight time. In horizon-tal plan, the range of QRS vector axis was -29.6 degrees ~ +47.2 degrees that was deviated to back of body center according to the flight time. In both plan, the amplitude of QRS vector was tend to decreasing against the flight time. Results of this study indicate that the cardiac function of fighter pilots was doubted to degenerate or weaken. Conclusionally, fighter pilots have to increase their cardioventricular fitness by the aerobic training and other methods.

9.Foreign Body Granulomas of the Breast Presenting as Bilateral Spiculated Masses.

In Asia, mammography following the injection of foreign materials into the breasts for cosmetic augmentation is frequently seen and diagnosis based on the typical radiologic findings is straightforward. We report the unusual radiologic findings in two patients with foreign body granulomas caused by injected foreign materials and discovered incidentally during screening work up. The mammographic findings were bilateral, hyperdense, spiculated masses, with occasional microcalcification, and at sonography, markedly hypoechoic, spiculated solid masses, located near the pectoralis muscle and partly extending into it, were observed. These radiologic findings mimicked malignancy.

10.The Effects of Combined Use of Hemodilution and Induced Hypotension on Hemodynamics and Oxygen Carrying Capacity in the Patients Under Posterior Spinal Fusion.

To decrease homologous transfusion and bleeding, hemodilution may be combined with in- duced hypotension. The effects of the above method on hemodynamics and oxygen carrying capacity in human is not studied. Ten patients, age 16-62 yr, having posterior spinal fusion, were studied. Acute preoperative hemodilution to hematocrit 27% with infusion of 10% pentastarch was performed in the patients. After induction of anesthesia and positioning we deereased mean arterial pressure to around 55mmHg by isoflurane and sodium nitroprusside. We observed the changes of hemodynamics and oxygen carrying capacity before hemodilution, after hemodilution, after induced hypotension and before the end of operation. After hemodilution, heart rate and arterial pressure were not changed significantly. Cardiac output and central venous pressure increased significantly. Systemic vascular resistance decreased significantly. There were no significant changes in arterial and mixed venous blood gas analysis. Oxygen flux was not changed significantly. Oxygen consumption and oxygen extraction ratio increased significantly but remained within normal range. After induced hypotension, heart rate was not changed. Cardiac output became lower than that after hemodilution, but was not different from the preoperative value. Systemic vascular resistance became lower than the preoperative value. There were no significant changes in arterial and mixed venous blood gas analysis except for mixed venous blood oxygen saturation. The mixed blood oxygen saturation increased to the preoperative level. Oxygen flux decreased, but oxygen consumption and oxygen extraction ratio were not significantly different from the preoperative values. There were no clinically significant changes in hemodynamics and oxygen carrying capacity before the end of operation. By the above results, we concluded that the combined use of hemodilution and induced hypotension is safe in the aspect of hemodynamics and oxygen carrying capacity.
