To observe the influence of kidney transplantation (KT) on erectile and endocrine function, we evaluated the PBI, penile pulse volume plethysmography, RigiScan and endocrine study before and after KT in 21 patients. We also interviewed with these patients or their sexual partners before KT. at 3 months and 6 months after KT. 7 cases (33%) were improved, 8 cases (38%) were same and 4 cases(19%) were aggravated in sexual desire and erectile function and 2 cases (10% ) had been suffered from erectile dysfunction since 19 months and 6 years prior to KT. The PBI was changed from 0.97 +/- 0.09 to 0.95 +/- 0.16 and it was not a significant difference. The penile plethysmography was not a good screening test in the evaluation of erectile function in KF. But the Rigiscan could detect all of the tested aggravated cases and it was thought to be a good screening test. In the endocrine studies, testosterone and FSH were decreased and LH and prolactin were increased before KT but testosterone and LH were significantly decreased after KT. It was thought to be the influence of KT and immunosuppressive agent (cyclosporin A). In conclusion, sexual desire and erectile function may be improved by the KT and most reliable screening test was Rigiscan. Endocrine function may be affected by the KT and cyclosporin A.

Purpose: As the daily life experiences of community-dwelling people with mental disabilities are closely correlated to mental health recovery, this study aimed to analyze their daily life experiences in depth to determine their significance. Method: Participants included eight community-dwelling people with mental disabilities. A total of 12 in-depth interviews were conducted once or twice per participant. The collected data were analyzed using the phenomenological analysis method described by Giorgi. Results The daily life experiences of community-dwelling people with mental disabilities were categorized into the following four components: “encountering shocking reality in the outside world”, “life tied down by the mental illness”, “happiness and gratefulness encountered in daily life”, and “wishing for a change and self-sustaining life”. Conclusion: This study found that the recovery and welfare of community-dwelling people with mental disabilities were promoted and that these patients made necessary efforts to become a member of the local community. These findings could be used as a reference for establishing social welfare policies which enable people with mental disabilities to participate as functioning members of the community that facilitate the prevention of relapse, to promote their integration into the community. Furthermore, the present results will contribute to the development of nursing intervention programs to promote recovery and prevent relapse, to ultimately establish a mental health management system.

Glomus tumors are benign cutaneous neoplasm, occurring in the vascular hamartomatous tubercles of the glomus body, which is a myoarterial apparatus typically found in the reticular dermis of the skin. Histopathologically, glomus tumors are composed of neoplastic cells that resemble normal glomus cells, arranged around a varying number of thin-walled, sometimes branching capillaries. Immunohistochemically, neoplastic cells in glomus tumors clearly exhibit smooth muscle characteristics with expression of muscle-specific actin and alpha-smooth muscle actin; whereas, the expression of CD34 is rare. In this report, we present two cases of benign glomus tumors on the subungual area and palm. Tumor cells of glomus tumor in two patients were both positive for smooth muscle actin (SMA) and CD34 immunohistochemical staining. We thought that coexpression of SMA and CD34 adds support to the interpretation of these lesions as a family of perivascular neoplasms.

This study was conducted to determine whether dietary factors could be related with serum adiponectin concentrations in 243, year-three female university students living in Seoul. The mean of body mass index (BMI) and adiponectin levels of all subjects were 20.17 kg/m2 and 11.07 microg/ml, respectively. When the subjects were divided into 3 groups based on BMI (underweight: < 18.5, normal: 18.5 < or = < 23, overweight and obesity: > or = 23), serum adiponectin levels in underweight group was significantly higher than that in 'normal' or 'overweight and 'obesity' groups. Also when the subjects were divided into two groups by mean adequacy ratio (MAR), serum adiponectin concentration of the high MAR group (MAR > 75) was slightly higher than that the low MAR group (MAR < or = 75). Serum adiponectin levels showed a negative correlation with body weight (p < 0.01) or BMI (p < 0.001) and a positive correlation with intakes of of animal or total protein (please clarify) or vegetable protein.Among the dietary behaviors, serum adiponectin levels of females who answered 'yes' to the question about 'eating breakfast' was significantly higher than that of those who answered 'sometimes' or 'No' (p < 0.05) and serum adiponectin levels were higher among those who reported higher fruit intakes. Overall, our results suggested that healthy lifestyle including acceptable BMI, eating breakfast and higher fruit consumption might play an important role in the prevention of obesity and enhancement of blood adiponectin levels.

PURPOSE: The objective of this study was to examine whether Hamcho freeze-dried powder and Hamcho freeze-dried nano powder have an anti-obese effect on high fat diet-induced obese rats. METHODS: Sprague Dawley rats were divided into 6 different groups, each of which was bred for 8 weeks with a different experimental diet: normal diet group (NC), high fat diet group (HFC), high fat diet + 2% Salicornia herbacea L. freeze-dried powder group (2% HS), high fat diet + 5% Salicornia herbacea L. freeze-dried powder group (5%HS), high fat diet + 1% Salicornia herbacea L. freeze-dried nano powder group (1% HSN), high fat diet + 2% Salicornia herbacea L. freeze-dried nano powder group (2% HSN). RESULTS: The 5% HS group showed a weight loss effect in body weight, liver and adipose tissue by reducing the amount of dietary intake and food efficiency ratio. In addition, the 5% HS group showed a significant reduction of serum leptin concentration, while having a beneficial effect on the improvement of lipid metabolism such as increase the serum concentration of adiponectin. However, Hamcho freeze-dried nano powder did not show a weight loss effect. CONCLUSION: Overall data indicated that Hamcho seemed to have anti-obesity effects. In particular, consumption of 5% Hamcho freeze-dried powder might have beneficial effects on body weight, serum adiponectin level, and lipid profiles since it led to reduced body weight and growth of adipose tissues by suppressing dietary intakes. However conduct of further research studies is needed for analysis of active components and biochemical action mechanism of Hamcho.

Background/Aims: Sequential monotherapy is recommended for anthracycline-and taxane-resistant metastatic breast cancer (MBC), but combination chemotherapy is considered in patients with visceral crisis. Cisplatin-doublet chemotherapy is a combination regimen for MBC, but prolonged treatment is challenging because of toxicity. We analyzed the role of single-agent maintenance chemotherapy after cisplatin-doublet chemotherapy for MBC. Methods: From January 2011 to December 2017, 96 anthracycline- and taxane-resistant MBC patients were retrospectively reviewed, and 49 patients with a sustained clinical benefit during the initial 6 cycles of cisplatin-doublet chemotherapy were enrolled for study. Patients were treated with gemcitabine-cisplatin (gemcitabine, 1,250 mg/m2, intravenously [IV], days 1 to 8; cisplatin 60 mg/m2, IV, day 1) or capecitabine-cisplatin (capecitabine 2,500 mg/m2, orally, days 1 to 14; cisplatin 60 mg/m2, IV, day 1) during the induction period. After 6 cycles, 16 patients were switched to single-maintenance treatment (gemcitabine or capecitabine) and the doublet regimen was continued in 24 patients. Survival outcomes (progression-free survival [PFS] and overall survival [OS]) were analyzed. Results: Among the 49 patients who showed a clinical benefit during cisplatin-doublet therapy, 24 were maintained on the doublet regimen, 16 were switched to single-maintenance treatment, and chemotherapy was suspended until disease progression in nine patients. The single-maintenance chemotherapy group showed superior survival than the chemotherapy holiday and doublet regimen groups (median PFS 15.43 months vs. 8.37 and 10.67 months, respectively, p = 0.008; median OS 43.67 months vs. 22.17 and 22.33 months, respectively, p = 0.014). Conclusions Patients showing a clinical benefit during 6 cisplatin-doublet chemotherapy cycles may have a sustained survival benefit from single-maintenance chemotherapy.

Non-typhoidal salmonella is rarely the cause of pacemaker infection. A 68-year-old man was referred to our hospital with tenderness and swelling at his cardiac resynchronization therapy defibrillator (CRT-D) implantation site. He had undergone CRT-D implantation because of sustained ventricular tachycardia and heart failure 7 years earlier, and the generator had been changed 2 months earlier. Twenty-four years earlier, he had undergone aortic valve replacement and mitral valve repair. We removed the generator and all of the CRT-D leads. After lead extraction, non-typhoidal salmonella serogroup B was cultured at the pocket and lead tip. The patient was managed successfully with lead extraction and antibiotic therapy.

BACKGROUND/AIMS: To identify the predicting factors of present hepatitis C virus (HCV) infection among patients with positivity for antibodies to HCV (anti-HCV). METHODS: We analyzed patients who showed positive enzyme immunoassay (EIA) results and performed an HCV RNA test as a confirmatory test at Kyung Hee University Hospital at Gangdong from June 2006 to July 2012. The features distinguishing the groups with positive and negative HCV RNA results were reviewed. RESULTS: In total, 490 patients were included. The results of the HCV RNA test were positive and negative in 228 and 262 patients, respectively. The index value of anti-HCV, mean age, platelet counts, total bilirubin, prothrombin time international normalized ratio, albumin and alanine transaminase (ALT) levels differed significantly between the two groups. On multivariable analysis, an index value of anti-HCV >10 [odds ratio (OR)=397.27, P<0.001), ALT >40 IU/L (OR=3.64, P=0.001), and albumin <3.8 g/dL (OR=2.66, P=0.014) were related to present HCV infection. CONCLUSIONS: Although EIA is not a quantitative test, considering the anti-HCV titer with ALT and albumin levels may be helpful in predicting present of HCV infection.
