Chronic eosinophilic pneumonia is a rare diasease characterized by chronic infiltration of the lung with ~eosinophils, usually associated with peripheral eosinophilia. In 65% of cases, the chest rad Ogroaph shows typiical nonsegmental air-space consolidation confined to the outer third of the lung and in 25)/0 of case "photographic negative of pulmonary edema". Typical lung manifestations with 'peripheral eosinophiliSa' tahrcharacteristic of chronic eosinophilic pneumonia. In the remaining cases, radiographic findings are nonspecific and require lung biopsy for confirmation. We repot a case of chronic eosinophilic pneumonia in which chest radiograph and CT scans revealed bilateral patchy or diffuse opacity with nodules scattered throughout the lungs.

The congenital long-QT syndrome (LQTS) is characterized by recurrent syncope, prolonged QT intervals, QT interval lability, polymorphic ventricular tachycardia, and sudden death. We report a case of congenital long QT syndrome in a 28-day-old male infant who presented with syncope, bradycardia with 2: 1 pseudo-atrioventricular block and a markedly prolonged QT inteval. One episode occured after crying and degenerated into ventricular fibrillation and terminated after cardioversion. A VVI type cardiac pacemaker was implanted. Subsequently, the infant's heart rate was over 110/min and 2: 1 AV block and any other arrhythmia were absent. The infant recovered from the accompanied pneumonia and sepsis and was discharged 47 days after adrnission. However, 13 days after discharge, the infant returned to our hospital ER with syncope. Ventricular fibrillation ceased after cardioversion. Despite medication with propranolol, ventricular tachycardia persisted. The infant expired the day after he was discharged against medical advice.

The purpose of this study was to investigate the characteristics of craniofacial morphology in adult with crowded dentition. The craniofacial morphology associated with normal and crowded of mandibular dentition was studied in lateral cephalometric radiograph obtained from 145 subjects grouped 44 males and 40 females in normal group, 33 males and 28 females in crowded group. The conclusions were as follows: Means, standard deviations and facial polygons from measurements were obtained from normal and crowded groups. Palatal plane to SN, occlusal plans to SN, mandibular plane to SN, mandibular plane to palatal plane and gonial angle measurements of crowded group were significantly different from normal group and were larger than normal group. SNA, SNB measurements of crowded group were significantly different from normal group and were smaller than normal group. Significant differences were observed between normal group and crowded groups on the basis of SN/1, PP/1, MP/1, and compared with normal group, upper incisors of crowded group were inclined more labially, whereas lower incisors of crowded group were inclined more lingually. Posterior facial height and ramal height were shorter in males of crowded group than those of normal group. Posterior facial height, mandibular length and posterior cranial base length were shorter in females of crowded group than those of normal group. Anterior facial height was longer in females of crowded group than those of normal group.

Heterotopic pregnancy is defined as coexistence of intrauterine and ectopic pregnancy. The reported incidence of heterotopic pregnancy, which is normally rare, is particularly high (1%) after IVF treatment. It is important to have a high index of suspicion for the occurrence of heterotopic pregnancies because only 40% to 84% of cases can be diagnosed with transvaginal ultrasound at the initial presentation. Edwards syndrome occurs in 8,000 newborns and the incidence is much higher in elderly gravidas. We report a case of heterotopic pregnancy following IVF-ET resulting in 16 weeks of intrauterine pregnancy with fetal Edwards syndrome, and tubal pregnancy with a brief review of literature.

OBJECTIVES: The purpose of this study was to explore insomniac demographic characteristics and the type of consultation provided to hospitalized patients asked to the Department of Psychiatry for insomnia and to compare patient insomnia characteristics by consultation type. METHODS: We performed a retrospective chart review of 4,966 patients who were hospitalized from August 1, 2005 to December 31, 2011 that received consultation in the Department of Psychiatry. Among them, 236 patients were referred for insomnia. We compared the differences in demographic characteristics and types of consultation between the insomnia patient group and other patient group. We also compared the difference between demographic characteristics and type of consultation by dividing total subjects into ‘with reconsultation’ and ‘without reconsultation’ groups. RESULTS: Our results came from the analysis of 9,689 consecutive consultation requests. There were 4,966 patients that participated in the study over 6 years and 6 months. The overall consultation rate was 3.3% of all admissions and insomnia patients comprised 4.8% of those. The ratio of re-consultation for insomnia was 27.5%. There was no significant difference in mean age between the insomnia ‘with reconsultation group’ and the insomnia ‘without reconsultation group’, but the ‘with reconsultation’ group had significantly more male patients and medical patients than the ‘without re-consultation’ group. For insomnia patients, consultation types were in the order of Mending request (51.3%), Paralle request (36.6%), Complementary request (9.0%) and this composition differed from that of total admission patients. CONCLUSION: Hospitalized patients referred for insomnia showed a higher proportion of male patients, lower rates of re-consultation compared with other patients, and most of these were for secondary insomnia. Each doctor should be aware of the possibility of inpatient insomnia, conduct positive assessments and referrals as necessary, and psychiatrists who might be asked for consultation need to prepare an active intervention with initial diagnosis and treatment, as well as recommendations for the timing of reconsultation.

BACKGROUND/AIMS: Stomach cancer is the most common malignancy and one of the leading causes of cancer-related death in Korea. The early diagnosis and treatment of gastric cancer are very important because the prognosis of early gastric cancer is excellent. Gastric screening may provide an opportunity to detect asymptomatic early gastric cancer. We analyzed the characteristics of gastric cancer diagnosed with a health screening test to evaluate the usefulness of a screening program for gastric cancer. METHODS: We retrospectively reviewed the clinicopathological characteristics of 111 gastric cancer patients diagnosed using gastric endoscopy as a gastric screening test at the Health Promotion Center of Kyungpook National University Hospital from July 1997 through December 2005. RESULTS: The incidence of gastric cancer was 0.38% and the mean patient age was 58.5 years old. The proportion of early gastric cancer was 73% of all gastric cancer. In 55 cases, constituting 49.6% of all gastric cancer and 68% of early gastric cancer, the lesions were confined to the mucosa. The most common macroscopic types were type IIc in early gastric cancer and Borrmann type 3 in advanced gastric cancer. There were significant positive correlations between lymph node metastasis and both the depth of tumor invasion and size of the lesion. The 5-year survival rates are 82.7% in total gastric cancer and 97.2% in early gastric cancer. CONCLUSIONS: Gastric cancer detected during health screening has a favorable prognosis because gastric cancer confined to the mucosa predominates and early gastric cancer detected at screening has a lower incidence of metastasis to regional lymph nodes.

BACKGROUND: Hyperlipidemia has been known as an independent risk factor in the develop-ment of coronary artery disease. This study was carried out to compare nutrient intakes, smo-king status, antioxidant vitamins, and plasma lipids in patients with coronary heart disease (CHD) and in normal healthy subjects among Korean population in Taegu. Possible causes of this dis-ease in patients are discussed. METHODS: Anthropometric assessments included mean intakes of nutrients, and the levels of plasma lipids (apolipoprotein [Apo] A - I, Lipoprotein [Lp] [a]), and antioxidant vitamins (such as vitamins A and E) were measured in female and male subjects with CHD against healthy con-trols. RESULTS: Dietary cholesterol and fat intakes were significantly higher in CHD groups in men and women. Total plasma cholesterol, LDL-C, triglyceride, thiobarbituric acid reactive subst-ance (TBARS), atherogenic index and Lp (a) levels were significantly higher in CHD patients than in the normal group in both men and women. Apo A-I, HDL-C and vitamin E levels were lower in CHD patients than in the normal group. The number of smokers was higher in CHD patients than in the normal group in both sexes of subjects. CONCLUSION: High fat and high cholesterol intakes seemed to be a major factor for the hyperlipidemia in the CHD patients. Their abnormal lipoprotein profile, which appeared in pati-ent plasma, corresponded well to dietary intake patterns. However, long term studies are need-ed to investigate the effects of smoking on lipid metabolism in CHD patients among the Korean population.

PURPOSE: The aim of this study was to evaluate whether cardiovascular disease risk factors and depression are associated with the frequency of self-report in patients with erectile dysfunction (ED). MATERIALS AND METHODS: Randomly selected men more then 40 years of age who visited the department of urology between January 2005 and July 2008 were evaluated by clinical examination and questionnaires. Trained doctors assessed the participants by using structured questionnaires that included medical history as well as report of ED. All participants were evaluated for cardiovascular disease risk factors. The group with unreported ED was defined as men with an International Index of Erectile Function-5 (IIEF-5) score of < or =21 who did not self-report ED. The group with self-reported ED was defined as men as above who self-reported ED. Symptoms of depression were assessed by the Center for Epidemiological Studies-Depression Scale (CES-D). RESULTS: The mean age of a total of 459 enrolled patients was 53.4+/-7.8 years. The overall frequency of ED was 39.8%. Of the total study group, 25.9% were categorized in the unreported ED group and 13.9% in the self-reported ED group. The men with self-reported ED had significantly severe forms of ED and a higher frequency of cardiovascular disease risk factors than did the men with unreported ED. Moreover, the self-reported ED group had a significantly higher frequency of symptoms of depression. CONCLUSIONS: Our results suggest that men with self-reported ED had a higher frequency of cardiovascular disease risk factors and symptoms associated with depression than did men with unreported ED. Furthermore, men with self-reported ED had more severe ED than did men with unreported ED.
