1.A Case of allergic Contact Dermatitis from Glutaraldehyde.

Glutaraldehyde is commonly used as a biocide to disinfect delicate instruments such as dental instruments, to treat warts, hyperhidrosis and bullous diseases and as a preservative in various materials. Contact sensitivity to glutaraldehyde may not be detected, because it is not included in the standard screening chemicals recommended in the screening tests. We report a case of allergic contact dermatitis caused by Wydex(2% glutaraldehyde). A 54-year-old female made a visit to our departrnent with eczematous dermatitis of both hands, neck and face. She worked as a cleaner in delivery room and newborn nursery, and had developed the dermatitis 1 month after Wydex' containing 2% glutaraldehyde had been to clean the rooms. She had no family or personal history of atopy or contact dermatitis. Patch tests with Korean Standard(KS-1000) and Wydex were performed, and patch tests with 2% glutaraldehyde, 0.2% gluteraldehyde and rubber mix followed. The positive results were, Wydex 2% ++, 0.2% +, 0.02% + and. glutaraldehyde 2% aq. +, 0.2% aq. + on day 2 and day 4. We confirmed that the skin lesions had been caused by Wydex . The symptoms were eradicated after she changed her work environment and there has been no evidence of recurrence.

3.An Anthropolgical Study on the Human Skeleton Excavated from Dugmoe Tomb of the Technopols of Kwang Ju City.

Authors have anthropologically measured the human skeleton from a Dugmoe Tomb of the technopolis of Kwang-ju city. The results obtained were as follows : 1. The human skeleton was thought to be constructed at the beginning of the period of the Chosun, judging from the Dugmoe Tomb. 2. It is female and its stature is estimated as about 163-165cm. The age of the skeleton is estimated to be late 50. 3. The cranial index is 77.09mm and the type is mesocephaly. 4. The cranial length-height index and the cranial breadth-height index were hypsicrane and acrocephaly. 5. The orbital index 95.0mm and the type is hypsiconch. 6. The humerus is comparatively long, slender and has more rounded diaphysis. 7. The femur is similar that of present day, and the type is platyer. 8. The abrasion of the occlusal surface of the teeth was 2.5 point.

4.Significance of Microsurgical Varicocelectomy for Non- obstructive Azoospermic Patients.

PURPOSE: This study attempted to assess the treatment outcomes after microsurgical varicocelectomy in non-obstructive azoospermic patients and correlate the surgical outcomes with the histopathological patterns of their testes. MATERIALS AND METHODS: Fourteen non-obstructive azoospermic men who underwent testicular biopsy and varicocelectomy, between September 2001 and December 2002, were reviewed. Varicocelectomy was unilaterally and bilaterally performed in 12 and 2 patients, respectively, using an inguinal approach with a microsurgical technique. Postoperative semen analyses were performed in each patient four months after the varicocelectomy. RESULTS: In the testicular histology, hypospermatogenesis was identified in three, maturation arrest in five and Sertoli-cell-only syndrome in six of the subjects. After a mean follow-up of 7.4 months, motile sperm in the ejaculate was identified in 6(43%) of the non-obstructive azoospermic patients. These included 2 in hypospermatogenesis, 3 in maturation arrest and 1 in Sertoli-cell-only syndrome, in terms of their histopathology. These six had improvements in their sperm concentration and motility, which were 0.45x10(6)/ml and 51.3%, respectively. However, of these six, with motile sperm after varicocelectomy, 1 with maturation arrest and 1 with Sertoli-cell-only syndrome returned to their previous azoospermic state on their second postoperative semen analyses. A pregnancy was achieved by natural intercourse for 1 of the men(7.1%) with hypospermatogenesis. CONCLUSIONS: Microsurgical varicocelectomy offers non-obstructive azoospermic patients the chance to have sperm in their ejaculate, and therefore, the possibility of natural pregnancy. Microsurgical varicocelectomy may be considered with the initial treatment modality in selective non- obstructive azoospermic patients with varicocele prior to intervention with assisted reproductive technology.

5.Treatment effect of protraction head gear on skeletal Class III malocclusion.

Before 1970, mandibular overgrowth was known as main cause of skeletal Class III malocclusion in growing children ; however, recent study reports that many skeletal Class III malocclusion patients also show maxillary deficiency. Since 1972, when Delaire re-accommodated Protraction Head Gear (P.H.G.), many researchers have reported that skeletal Class III discrepancies could be corrected through use of P.H.G., which induces anterior movement of maxilla and change in mandibular growth pattern into infero-posterior direction ; nevertheless, it is very difficult to predict resultant changes of orofacial region. The purpose of this study was to find out what treatment effect P.H.G. has on different study samples. Author divided 51 skeletal Class III malocclusion patients with maxillary deficiency who were treated with P.H.G. into different study groups depending on sex, treatment beginning age, intraoral appliance, and facial growth pattern. By doing so, following results were obtained. 1. Treatment beginning age and Sex Four age groups (5.8 to 8 year-old, 8 to 10 year-old, 10 to 12 year-old, 12 to 14 year-old) were compared, and no significant difference was observed. (p<0.05) There was no significant difference between the sex groups, either. (p<0.05) 2. Intraoral appliance Treatment effects of study groups that used R.P.E.(mean age of 10.2) and Labio-Lingual appliance(mean age of 8.9) were compared. There was no significant difference depending on the type of intraoral appliance that was used. (p<0.05) 3. Facial growth pattern 1) Amounts of SNB and ANB corrections were smaller in clockwise growth pattern group than those in normal or counterclockwise growth pattern group. (p<0.05) 2) Amounts of increase in Wits appraisal and mandibular plane angle were greater in counterclockwise growth pattern group than those in normal or clockwise growth pattern group.(p<0.05) 3)Amounts of increase in articular angle were greater in counter lockwise growth pattern group than those in clockwise growth pattern group. 7.The Short Term Efficacy of Entecavir Therapy in Chronic Hepatitis B.

BACKGROUND/AIMS: Entecavir is a synthetic nucleoside analogue, cyclopentyl guanine nucleoside, which has a potent antiviral effect and the least viral breakthrough in hepatitis B virus (HBV) replication. Entecavir has been available in Korea since 2007 but there are few reports on its effects. The aim of this study was to evaluate the virological response (VR) and biochemical response (BR) to entecavir in HBV patients at 3, 6 and 9 months after treatment with entecavir. MATERIALS AND METHODS: Thirty-three chronic hepatitis B patients who took entecavir for at least 9 months were enrolled. We investigated VR and BR by retrospectively reviewing medical records. Patients who satisfied the following criteria were chosen: 1) initial alanine aminotransferase (ALT) levels = 1.5upper limit of normal (ULN) and 2) initial HBV DNA levels = 5 log10 copies/ml. We measured ALT levels every 3 months until month 9. HBV DNA was measured every 2 or 3 months by polymerase chain reaction (PCR) method. RESULTS: Most patients taking entecavir showed good BR (ALT < 40 IU/L). The BR rates were 61%, 73% and 67% at months 3, 6 and 9, respectively. VR (HBV DNA < 5 log(10) copies/ml or 2 log lower than initial HBV DNA) rates were 82%, 91% and 91% at months 3, 6 and 9, respectively. Undetectable HBV DNA (HBV DNA < 4 log(10) copies/ml) rates were 49%, 73% and 85% at months 3, 6 and 9, respectively. Two patients presented with virological breakthrough without adverse effects until month 9. CONCLUSIONS: Entecavir showed good BR and VR from month 3 and these effects continued through the 9-month observation period. This suggests that entecavir is also a good choice for the first line treatment of chronic hepatitis B (CHB). Further studies are needed to determine the long-term efficacy and drug resistance of entecavir in Korean CHB patients.

8.The K121Q Polymorphism in ENPP1 (PC-1) Is Not Associated with Type 2 Diabetes or Obesity in Korean Male Workers.

Type 2 diabetes is characterized by insulin resistance, and ENPP1 plays an important role in insulin resistance. We investigated the association of the ENPP1 K121Q polymorphism with both diabetes and obesity (body mass index [BMI]) in Korean male workers. The study design was case-control. Subjects were 1,945 male workers (type 2 diabetes, 195; non-diabetes, 1,750) of nuclear power plants who received examinations from March to October in 2004. We collected venous blood samples under fasting (> or =8 hr) conditions, calculated BMI by height and weight, and assessed relevant biochemical factors. The results of this study demonstrated that the ENPP1 121Q genotype (KQ+QQ types) was not associated with type 2 diabetes (odds ratios [OR], 0.854; 95% confidence interval [CI], 0.571-1.278) or obesity (OR, 0.933; 95% CI, 0.731-1.190). In addition, the frequency of the Q allele was not related to type 2 diabetes (OR, 0.911; 95% CI, 0.630-1.319) or obesity (OR, 0.962; 95% CI, 0.767-1.205). We concluded that the ENPP1 121Q allele is not a critical determinant for either diabetes or obesity in Korean males. The discordance between the results of this study and those derived from studies of Dominican, South Asian, Caucasian, Finnish, and French populations might be due to differences in genetic backgrounds between these populations.

9.A Case of Mitochondrial Myopathy Showing Severe Hypoxemia during REM Sleep.

Mitochondrial myopathy is characterized by variable clinical manifestations from mild limb weakness to fatal respiratory failure and central nervous system sequela. But it is a rare event that sleep disordered breathing become a clue of diagnosis for mitochondrial myopathy. We report a case of a 21 year-old man who was diagnosed as mitochondrial myopathy during the investigation for the possible cause of chronic hypoventilation syndrome. Before being admitted to our hospital, he was suspected as having sleep apnea syndrome in another hospital. We re-evaluated the history, physical examination, laboratoy findings and polysomnography in detail. Severe hypoxemia was noted during REM sleep on nocturnal polysomnography and the diagnosis of mitochondrial myopathy was made by muscle biopsy in rectus abdominis muscle. We treated him with bilevel positive airway pressure therapy during sleep and it could reverse the hypoxemia during REM sleep. He could be discharged with improved condition and is being well with the use of this ventilatory assistance.

10.Vertebral compression fractures after spine irradiation using conventional fractionation in patients with metastatic colorectal cancer.

PURPOSE: To evaluate the risk of vertebral compression fracture (VCF) after conventional radiotherapy (RT) for colorectal cancer (CRC) with spine metastasis and to identify risk factors for VCF in metastatic and non-metastatic irradiated spines. MATERIALS AND METHODS: We retrospectively reviewed 68 spinal segments in 16 patients who received conventional RT between 2009 and 2012. Fracture was defined as a newly developed VCF or progression of an existing fracture. The target volume included all metastatic spinal segments and one additional non-metastatic vertebra adjacent to the tumor-involved spines. RESULTS: The median follow-up was 7.8 months. Among all 68 spinal segments, there were six fracture events (8.8%) including three new VCFs and three fracture progressions. Observed VCF rates in vertebral segments with prior irradiation or pre-existing compression fracture were 30.0% and 75.0% respectively, compared with 5.2% and 4.7% for segments without prior irradiation or pre-existing compression fracture, respectively (both p < 0.05). The 1-year fracture-free probability was 87.8% (95% CI, 78.2-97.4). On multivariate analysis, prior irradiation (HR, 7.30; 95% CI, 1.31-40.86) and pre-existing compression fracture (HR, 18.45; 95% CI, 3.42-99.52) were independent risk factors for VCF. CONCLUSION: The incidence of VCF following conventional RT to the spine is not particularly high, regardless of metastatic tumor involvement. Spines that received irradiation and/or have pre-existing compression fracture before RT have an increased risk of VCF and require close observation.
