Pigmented neurofibroma, a variant of neurofibroma, has rarely been observed in patients with or without neurofibromatosis. Pigmented neurofibroma is characterized histologically by the coexistence of scattered melanin-laden cells and benign spindle cells with neural differentiation. We report a case of pigmented neurofibroma in a 22-year-old female patient.

3.A Case of Child with Obstructive Sleep Apnea Syndrome Recurred after Adenotonsillectomy.

The most common cause of obstructive sleep apnea syndrome (OSAS) in childhood is adenotonsillar hypertrophy. Adenotonsillectomy improves the symptoms quite well in most cases. However, some patients could experience the OSAS again after adenotonsillectomy, who might have several risk factors such as incomplete operation, misdiagnosis, combined anatomical malformation, sinusitis or chronic allergic rhinitis, obesity, initial severe OSAS, and early onset OSAS. We report a case of 11-year-old obese boy who presented with snoring for several years. He was obese with body mass index (BMI) of 26.3 kg/m2 and also found to have fatty liver by ultrasonogram. Initial polysomnography (PSG) showed that he met the criteria of severe OSAS with the apnea-hypopnea index (AHI) of 70.5. He underwent adenotonsillectomy and symptoms improved immediately. Four months later symptoms were relieved with AHI of 0, but 1 year after the adenotonsillectomy he started to complain snoring again and the subsequent PSG results showed that OSAS has relapsed with AHI of 43. Paranasal sinus X-ray and physical examination showed sinusitis and re-growth of adenoid. Obesity was proved not to be a contributing factor because his BMI decreased to normal range (23.1 kg/m2) after diet control and regular exercise. Also, liver transaminase was normalized and fatty liver was disappeared on follow-up abdominal ultrasonogram. After treatment of sinusitis, symptoms were relieved with decreased AHI (8.5). This case suggests that simple adenotonsillectomy might not be the end of OSAS treatment in childhood. Patients who had adenotonsillectomy should be followed by subsequent PSG if symptoms recur. It is also important to be aware of risk factors in the recurrent OSAS for the proper intervention according to the cause.

4.Cellular study of replicative senescence in human periodontal ligament fibroblast using molecular biology.

Human periodontal ligament fibroblast(hPDLF) is very important to cure periodontal tissue because it can be diverged into various cells. This study examined the expression of MMP-1, TIMP-1, periodontal ligament specific PDLs22, Type I collagen, Fibronectin, TIMP-2, telomerase mRNA in a replicative senescence of hPDLF. The periodontal ligament tissue was obtained from periodontally healthy and non-carious human teeth extracted for orthodontic reasons at the Chosun University Hospital of Dentistry with the donors' informed consent. The hPDLF cells were cultured in a medium containing Dulbecco's modified Eagle medium(DMEM, Gibco BRL, USA) supplemented with 10% fetal bovine serum(FBS, Gibco BRL, USA) at 37C in humidified air with 5% CO2. For the reverse transcription-polymerase chain reaction(RT-PCR) analysis, the total RNA of the 2, 4, 8, 16, 18, and 21 passage cells was extracted using a Trizol Reagent(Invitrogen, USA) in replicative hPDL cells. Two passage cells, i.e. young cells, served as the control, and beta-actin served as the internal control for RT-PCR The results of this study about cell morphology and gene expression according to aging of hPDLF using RT-PCR method are as follows: 1. The size of hPDLF was increased with aging and it was showed that the hPDLF was dying in the final passage. 2. PDLs22 mRNA was expressed in young hPDLF of the two, four, and six passage. 3. TIMP-1 mRNA was expressed in young hPDLF of the two and four passage. 4. There was a tendency that MMP-1 mRNA was weakly expressed over eighteen. 5. Type 1 collagen mRNA was expressed in almost all passages, but it was not expressed in the final passage. 6. Fibronectin mRNA was observed in all passages and it was weakly expressed in the final passage. 7. TIMP-2 and telomerase mRNA were not expressed in this study. Based on above results, it was observed that PDLs22, Type 1 collagen, Fibronectin, MMP-1. and TIMP-1 mRNA in hPDLF were expressed differently with aging. The study using the hPDLF that is collected from healthy patients and periodontitis patients needs in further study.

6.A Case of Ulcerative Trichilemmal Carcinoma.

We report a case of trichilemmal carcinoma in a 82-year-old man who presented an ulcerative tumor. Initially, a crusted mass was observed on the scalp and with subsequent removal of the crust, purulent and ulcerative lesion was seen. After treatment with sensitive antibiotics for 8 days, purulent discharge was markedly decreased in the lesion. Histopathologic findings showed clear cells tumor with trichilemmal keratinization. The tumor cells stained positive to PAS, sensitive to diastase and showed negative findings to CEA, EMA and S-100.

7.The Role of the Peripheral Chemokine, CCL3, in Hyperalgesia following Peripheral Nerve Injury in the Rat.

BACKGROUND: Upregulation of one type of the pro-inflammatory chemokine (CCL2) and its receptor (CCR2) following peripheral nerve injury contributes to the induction of neuropathic pain. Here, we examined whether another type of chemokine (CCL3) is involved in neuropathic pain. METHODS: We measured changes in mechanical and thermal sensitivity in the hind paws of naive rats or rats with an L5 spinal nerve ligation (SNL) after intra-plantar injection of CCL3 or met-RANTES, an antagonist of the CCL3 receptor, CCR1. We also measured CCL3 levels in the sciatic nerve and the hind paw skin as well as CCR1 expression in dorsal root ganglion (DRG) cells from the lumbar spinal segments. RESULTS: Intra-plantar injection of CCL3 into the hind paw of naive rats mimicked L5 SNL-produced hyperalgesia. Intra-plantar injection of met-RANTES into the hind paw of rats with L5 SNL attenuated hyperalgesia. L5 SNL increased CCL3 levels in the sciatic nerve and the hind paw skin on the affected side. The number of CCR1-positive DRG cells in the lumbar segments was not changed following L5 SNL. CONCLUSIONS: Partial peripheral nerve injury increases local CCL3 levels along the degenerating axons during Wallerian degeneration. This CCL3 binds to its receptor, CCR1, located on adjacent uninjured afferents, presumably nociceptors, to induce hyperalgesia in the neuropathic pain state.

8.Clinical Observation of Ruptured Right Aortic Sinus of Valsalva.

A Clinical observation was made on five patients with ruptured right aortic sinus of valsalva who admitted Soonchunhyang College Hospital during the period of may, 1983-Jan., 1985. 1) Age distribution was from 18 to 46 years and four patients were male and the rest one was female. 2) Chief complaints were dyspnea, chest pain and palpitation. Continuous murmur was heard at third and fourth intercostal space along left sternal border with palpable thrill in all cases. 3) The ECG showed left ventricular hypertrophy in 4 cases and the M mode echocardiogram revealed the increased internal dimesion and the augmented motion of the left ventricle in all cases. The 2 dimensional echocardiogram revealed the aneurysmal sac in the right ventricle in 4 cases. 4) The aortogram by DSA method showed regurgitant flow from aorta to right ventricle in 4 cases. The cardiac catheterization showed a significant oxygen step up in the right ventricle in all cases. 5) Operation was done successfully in all cases, of which ventricular septal defect were in 3 cases and aortic regurgitation was in one case.

9.A Clinical Study of the Congenital Hypertrophic Pyloric Stenosis.

A Clinical study was carried out on 39 cases with congenital hypertrophic pyloric stenosis who were admitted to the department of Pediatrics, Catholic Medical College, during the 7 years 8 months from January 1971 to August 1978. The following results were obrained. 1. The age incidence was highest in the 2-4 weeks of age and 82.0% were under the 8 weeks. Af age Male to female ratio was 12:1. 2. The age at onset was within 3 weeks of age in the majority cases(69.2%). 3. In distribution of birth weight, the majority cases(75%) were 3.0~4.0kg. and 18 cases(46%) were first born bady. 4. In the body weight percentile on admission, the jahority cases(75.4%) were under the 25 percentile. 5. The kinds of feeding in order of frequency were breast(41.0%), bottle(30.8%) and mixed feeding(28.2%). 6. The average duration of symptoms prior to operation was 17 days. 7. On the physical examination, gastric peristaltic wave was observed in 30 cases(77%) and the pyloric tumor mass was palpated in 26 cases(67%). And Projectile vomitiong and dehydration were observed in all cases. 8. Barium meal X-ray study showed the string sign in 33 cases(88.9%).

10.The Frequency of Serum Autoantibodies to the Epidermal Antigens among Normal Elderly People.

BACKGROUND: One of the age-associated changes in immunity is the increase of autoantibodies due to dysregulation of the immune function. Anti-basement membrane zone antibodies were also reported to be found in the sera taken from normal elderly people. OBJECTIVE: The purpose of this study is to examine the frequency of autoantibodies to the epidermal antigens in the sera among normal elderly people. METHODS:Indirect immunofluorescence using salt-split human skin and immunoblotting with extracts from epidermoid carcinoma cell line(A431) were performed using sera from forty elderly persons without any systemic disease. RESULTS: Among 40 elderly individuals, 3 cases(7.5%) were positive for anti-basement membrane zone IgG antibodies in indirect immunofluorescence, and the titers were 1:10 to 1:40 dilution factor. In immunoblottings, 4 cases(10%) were positive with epidermal antigens; 2 were positive against the 180kD protein with IgG and IgA autoantibodies, 1 showed positive bands at 230kD and 180kD antigens with IgG, and remaining one showed positive at 130kD antigen who had negative result in indirect immunofluorescence. CONCLUSION: Autoantibodies against epidermal antigens(bullous pemphigoid angigen[230/180kD] or pemphigus vulgaris antigen[130kD]) can be detected without any bullous cutaneous diseases among elderly people; possibly as an epiphenomenon of immunosenescence.
