3.Evaluation of bone response by resonance frequency analysis of anodized implants.

STATEMENT OF PROBLEM: Resonance frequency analysis has been increasingly served as a noninvasive and objective method for clinical monitoring of implant stability. Many clinical studies must be required for standardized data using RFA. PURPOSE: This study was performed to evaluate RFA value changes in two anodized implant groups. MATERIAL AND METHOD: Among a total of 24 implants, twelve screw shaped implants as a test group (H2-R8.5) were manufactured, which had a pitch-height of 0.4 mm, an outer diameter of 4.3 mm, a length of 8.5 mm, and external hexa-headed, were turned from 5 mm rods of commercially pure titanium (ASTM Grade IV, Warantec Co., Seoul, Korea), and another twelve implants as a control group were Branemark Ti-Unite MK4 (diameter 4.0 mm, length 8.5 mm). Each group was installed in tibia of rabbit. Two implants were placed in each tibia (four implants per rabbit). Test two implants were inserted in right side and control two in left side. ISQ values were measured using Osstell(TM) (Integration Diagnostics Ltd. Sweden) during fixture installation, and 12 weeks later and evaluated the RFA changes. RESULTS: Mean and SD of baseline ISQ values of test group were 75.0 +/- 3.4 and 68.7 +/- 8.1 for control group. Mean and SD of ISQ values 12 weeks after implant insertion were 73.2 +/- 4.7 for test group and 72.6 +/- 3.9 for control group. There were no statistically significant differences between groups in ISQ values after 3months (P>0.05). From the data, RFA gains after 3 months were calculated, and there was statistically significant difference between groups (P<0.05). CONCLUSION: Although there were RFA changes between groups, implant stability after experimental period shows alike tendency and good bone responses.

4.The Relationship between Health Value Cognition, Health Promotion Behavior and Health Examination Results Among Transit Corporation's Workers.

OBJECTIVES: This study was performed to compare health value cognition and health promotion behavior compliance between healthy group and not being healthy group, and to evaluate the difference of health promotion behaviors compliance between high group and low group in health value cognition. METHODS: The subjects of this study were 565 workers, 233 healthy workers, 172 observation cases and 160 disease cases, selected from 4,919 transit coporations workers. RESULTS: In health promoting behavior compliance, not being healthy group showed lower level than healthy group in exercise and showed higher level than healthy group in interpersonal support significantly. A worker who replied on health as the most value in life was consisted in 37.3% of healthy group and 36. 1% of not being healthy group, Health value cognition according to general characteristics showed no significant difference except only variable of age in healthy group. Health promoting behavior compliance according to cognition of health value showed in healthy group that a high level group of health value cognition was more significant difference than a low level group in self actualization and health responsibility of health promoting behavior and didn't show difference significantly in not being healthy group. CONCLUSIONS: In conclusion, the relationship between health value cognition, health promotion behavior compliance and periodic health examination results showed weakly. Therefore, in order to develop and apply spontaneous health promotion program, it was considered that should emphasize compliance than cognition.

5.The Relationship among Job Overload, Self-efficacy, Emotional Exhaust and Turnover Intention in Clinical Nurses.

PURPOSE: This study was to identify the relationship among clinical nurse's job overload, self-efficacy, emotional exhaust and turnover intention. METHODS: The study was conducted through a survey on 537 clinical nurses in five general hospitals. The data were collected by self-reporting questionnaires and analysed by the SPSS/WIN 17.0 program using descriptive statistics, t-test, ANOVA, Pearson correlation coefficient and stepwise multiple regression. RESULTS: The mean score of turnover intention was 3.04. Turnover intention according to age, total career, job objective, and work off showed significant differences. The significant positive correlations between emotional exhaust and turnover intention (r=.531, p<.001), job overload and turnover intention (r=.514, p<.001) were found. The significant factors influencing turnover intention were emotional exhaust (beta=.488, p<.001), career (beta=.175, p<.001), job overload (beta=.119, p=.003), and number of night (beta=.117, p=.002), which accounted for 40.3% of the variance. CONCLUSION: Thought this result, nursing managers should analyze the reasons for high turnover intention, focusing on the main factors affecting turnover intention and consider the solutions to decrease the turnover intention.

6.A clinical study of testicular relapse in childhood acute lymphocytic leukemia.

From January 1984 to June 1991, we studied testicular relapsed patients among 105 cases of acute lymphocytic leukemia in children who were admitted to the Department of Pediatrics and Yonsei Cancer Center, Yonsei University Severance Hospital. The results were as follows: 1) 15 out of 105 (14.4%) cases of acute lymphocytic leukemia were diagnosed as testicular relapse. According to the prevalence by the age groups, there were no patient under 2 years old, 14 out of 73 (19.4%) in 2~9 years old and 1 out of 26 (3.8%) over 10 years old groups. 2) At initial diagnosis of acute lymphocytic leukemia there were significant correlation between testicular relapse and initial lymphadenopathy (p=0.09), but not with WBC count, hemoglobin level, platelet count, hepatomegaly and splenomegaly. 3) Mean duration to testicular relapse from initial diagnosis of acute lymphocytic leukemia was 3.1+/-1.7 years. 4) Among 15 patients, 7 cases combined with bone marrow or central nervous system relapse. Their survival rate was lower than isolated testicular relapse groups. 5) The 5 years survival rate of testicular relapsed patients was 67% after the combinations of chemotherapy, radiotherapy and orchiectomy.

8.A Case of Amitraz Insecticide Intoxication after Ingestion of Large Amount.

Amitraz is used as farm-animal insecticide. Its side effects in humans are related to its pharmacological activity on alpha 2-adrenergic receptors. The case describes a previously healthy 46-year-old woman who intentionally ingested approximately 250mL of liquid amitraz. She presented with vomiting, altered mental status, miosis, dry mouth, hypopnea, metabolic and respiratory acidosis, hypotension, hypothermia, polyuria, metabolic acidosis, elevated serum aminotransferase and abdominal distension. Supportive treatments including mechanical ventilation, hydration, dopamine infusion, bicarbonate infusion and gastric decompression resulted in improvement. By hospital day 3, she recovered with resolution of abdominal distension. It is paramount to recognize amitraz poisoning when apesticide-intoxicated patient presets with signs and symptoms consistent with organophosphate intoxicated patients but with greater alpha 2-adrenergic related symptoms such as decreased bowel motility and xerostomia.

9.Clinical Observation of Ruptured Right Aortic Sinus of Valsalva.

A Clinical observation was made on five patients with ruptured right aortic sinus of valsalva who admitted Soonchunhyang College Hospital during the period of may, 1983-Jan., 1985. 1) Age distribution was from 18 to 46 years and four patients were male and the rest one was female. 2) Chief complaints were dyspnea, chest pain and palpitation. Continuous murmur was heard at third and fourth intercostal space along left sternal border with palpable thrill in all cases. 3) The ECG showed left ventricular hypertrophy in 4 cases and the M mode echocardiogram revealed the increased internal dimesion and the augmented motion of the left ventricle in all cases. The 2 dimensional echocardiogram revealed the aneurysmal sac in the right ventricle in 4 cases. 4) The aortogram by DSA method showed regurgitant flow from aorta to right ventricle in 4 cases. The cardiac catheterization showed a significant oxygen step up in the right ventricle in all cases. 5) Operation was done successfully in all cases, of which ventricular septal defect were in 3 cases and aortic regurgitation was in one case.

10.Pityriasis Rotunda Treated with Combination of Topical and Oral Retinoid.

Pityriasis rotunda (PR) is a rare disease characterized by persistent, sharply defined, oval, scaly patches of dry skin, localized mainly on the trunk and extremities. Its etiology remains unknown. However, several reports suggest that it is a form of acquired ichthyosis vulgaris or a skin manifestation of systemic disease, such as malnutrition, chronic illness, hepatic disease, and malignancies. Although a variety of treatment modalities, including topical lactic acid, urea, tars, emollients, and corticosteroid, have been applied to it, their efficacies are not satisfactory. Herein, we report a case of PR in a healthy man who was successfully treated with oral and topical retinoids.
