1.Intracranial meningeal Masson's hemangioma: CT and angiographic features

Masson's hemangioma is a rare benign vascualr condition with a papillary intravascular endothelial proliferation which may appear either as aprimary form or as a secondary form in a pre-existing vascular process.CT and angiographic features of 2 cases with Masson's hemangioma were presented. Both of them were locatedextra-axially in the posterior fossa. CT findings were not specific in both cases: One showed homogeneously enhancing mass, simulating meningioma. And the other demonstrated a multiolocular rim enhancing mass. However, the angiographic features were rather characteristic; Both cases showed persistent vascular poolings of contrast mediawhich were supplied form the meningeal vessels. Angiographic differential diagnosis of similar lesions in theposterior fossa is discussed.

2.The Use of Nd-YAG Laser in Neurosurgery.

The authors report our experiences with the use of Nd-YAG laser in extripation of selected intracranial tumors:5 Meningiomas, 1 Acoustic neurinoma, 1 Hemangioendothelioma and 1 Oligodendroglioma. The Nd-YAG laser is very helpful to coagulate the pathologic vessels. And because of it's excellent coagulation the Nd-YAG laser is particularly useful when applied in highly vascular tumor as well as in high contents of mesenchymal structures within the tumors.

5.Clinical Analysis of Functiononing Pituitary Adenoma.

Forty-four patients were operated on functioning pituitary adenomas during 5 years since Jun. 1977 at the Department of Neurosurgery, Seoul National University Hospital. Clinical and laboratory findings were analyzed and the following results were obtained. 1) The peak incidence was in the 3rd and 4th decade 21 cases were male and 23 cases were female. 2) Endocrinologic study revealed prolactinoma in 28 cases(37%), somatotropic adenoma in 13 cases(17%), corticotropic adenoma in 3 cases(4%), endocrine-inactive adenoma in 32 cases(42%). 3) MAin symptoms were endocrine symptoms(86%), headache(75%) and visual field defect(59%). 4) Seven(16%) were in the stage I(micro-adenoma), 27(61%) in the stage II, 6(14%) in the stage III, 4(9%) in the stage IV. 5) The rate of endocrinologic cure or improvement was 65% in prolactinoma and 75% in somatotropic adenoma. This result was less favorable less favorable than the result of other recent studies, but it seemed to be due to large proportion of large or invasive adenoma instead of micro-adenoma in this series. 6) In prolactinoma, 7 out of 12 patients with amenorrhea resumed menstrual cycle and 2 patients had pregnancies postoperatively. 7) The visual field defect was improved in 16 of 21 cases(76%) postoperatively.

6.Two Cases of Posttraumatic Middle Cerebral Artery Occlusion.

While posttraumatic occlusion of the extracranial carotid and vertebral arteries is well known, reports of occlusion of intracranial vessels following closed head injury are rare. Since Deveer and Browder reported the first case in 1942, only 22 cases of posttraumatic middle cerebral artery occlusion have been reported until now. We experienced 2 cases of posttraumatic middle cerebral artery occlusion recently. One patient was 27 years old male who developed right hemiplegia after relatively minor head injury. Reported angiograms taken on the 13th day after trauma revealed sparse middle cerebral artery branches with retrograde filling through left anterior cerebral artery in middle cerebral artery territory. He showed relatively satisfactory recovery by conservative treatment with steroid and cerebral vasodilator. The other patient became semicomatous with left hemiplegia after fall down accident from running train. Left carotid angiograms showed complete obstruction of left middle cerebral artery in its initial portion with corresponding avascular area. This patient died one week after the angiography in spite of all possible conservative care.

7.Surgical Management of Chronic Subdural Hematoma:The Outcome and Factors Relating to The Brain Expansion.

Ninety-seven patients with chronic subdural hematoma were treated surgically between 1983 and 1992. The case records and radiological findings on computerized tomography(CT) scan were studied with regard to surgical outcome and postoperative brain expansion after burr hole craniostomy and closed system drainage. Male:Female ratio was 3.6:1 and the peak incidence was in the sixth decade of life. The patients were graded according to the scale used by Markwalder et al. and 56% were in Grade 0 and 1. The CT scan demonstrated the brain expansion and resolution of subdural space within 2 weeks in 53% of patients. Risk factors causing poor postoperative brain expansion were poor intraoperative brain expansion, too young(below 20 years) or too old(more than 71 years) age, and underlying disease. Following surgery, most of the patients improved. Whereas at the time of admission only 7% were neurologically normal(Grade 0), 67% of the patients fell into this category during the postoperative phase and 90% of the patients were postoperatively in Grade 0 and 1. Four patients died in the hospital, for a mortality rate of 4.1%. The most important factor influencing outcome was preoperative neurologic state.

8.Surgical Approach to the Juxtasellar Meningioma.

Meningioma of the sellar and parasellar origins grows usually to enormous size having complicated anatomical relations with olfactory and optic nerves, internal carotid artery and its major branches, cavernous sinus, and ventral diencephalons, so as daren't attack them. Visual failure, symptom and signs of increased intrcranial pressure are the most important problems to be solved in the treatment. Three cases of juxtasellar meningioma, one tuberculum sellae, two medial sphenoid ridge were removed under special designs including craniotomy permitting bi-directional approaches, frontal polar wedge resection, and pursuit of major vessels etc., and it was felt that complete removal with functional recovery used not to be always impossible in some cases. Technical problems and results are reported.

9.Pulmonary Embolism in Neurosurgical Patients : A Mortality Cases Analysis.

Pulmonary emboli originate from deep vein thrombi which occur frequently in pregnancies, prolonged bed-ridden state, cardiac diseases, cerebrovascular diseases, etc.. The authors analyzed 10 cases of sudden unexplained deaths and found that 4 cases of them probably had pulmonary embolism as a cause of deterioration and death. The estimated pulmonary embolism-related operative mortality was 0.1-0.3% according to the degree of suspicion. Most of the natural course of the disease are rapidly progressive ones and it is sometimes very difficult and impractical to obtain a definite diagnosis due to rapid deterioration and /or sudden death before studies. So the recognition of predisposing factors and prevention in risky patients are stressed.
