1.Squamous Metaplasia of the Pleura.

We report a case of squamous metaplasia of the pleura observed in a 51 year old man. Squamous metaplasia of serosal surface, and in particular of the pleura, is uncommon. The pathogenesis of squamous metaplasia is obscure, but it may be associated with chronic irritation.
2.Prognostic significance of angiogenesis in non-small cell lung cancer.

BACKGROUND: Angiogenesis plays a critical role in human tumor growth and metastasis. Microvessel count, as a measure of tumor angiogenesis, has been significantly correlated with invasive and metastatic patterns in breast, prostate and cutaneous carcinomas. Materials and METHODS: Fifty patients with curatively resected non-small cell lung cancer were evaluated. Tumor tissues embedded in paraffin block were stained by anti CD 31 (PECAM, platelet endothelial cellular adhesion molecule) using immunohistochemical method to assess microvessel count. Microvessels were counted in the most active areas of neovascularization(microscopy, 200×). RESULTS: 1) Mean microvessel count was 47.1 ± 17.7(per 200×field) in total 50 cases. 2) Mean microvessel count of adenocarcinoma (54.4±19.9) was significantly higher than that of squamous cancer(43.9±16.2)(p<0.05), but there were no relationship between microvessel count and TNM stages. 3) Median survival time, 2-year and 5-year survival rates of the low microvascular group(microvessel count<45, 22cases) were 61 months, 80% and 40%, respectively, and those of the high microvascular group(microvessel count ≥ 45, 28 cases) were 46 months, 75% and 12%, respectively. As results, prognosis of low microvascular group is statistically significantly superior to that of the high microvascular group(p=0.0162, Kaplan-Meier, log-rank). CONCLUSION: Angiogenesis assessed by microvessel count can be used as one of the significant prognostic factors in non-small cell lung cancer.

3.Prognostic Value of Vascular Endothelial Growth Factor(VEGF) in Resected Non-Small Cell Lung Cancer.

BACKGROUND: Angiogenesis is an essential component of tumor growth and metastasis, and the vascular endothelial growth factor (VEGF) is one of the most important angiogenic factors. Several solid tumors produce substantial amounts of VEGF, which stimulates proliferation and the migration of ednothelial cells, therby inducing neovasculization by a paracrine mechanism. To evaluate the prognostic roles of angiogenesis and VEGF expression in patients with non-small cell lung cancer, the relationship between VEGF expression in tumor tissues, the clinicopathologic features and the overall survival rate were analysed. METHODS: Sixty-nine resected primary non-small cell lung cancer specimens were evaluated. The pareffinembedded tumor tissues were stained by anti-VEGF polyclonal antibodies using an immunohistochemical method to assess VEGF expression. RESULTS: In Forty-one patients (59%), the VEGF antigen was expressed weakly in their tumor tissue, whereas in twenty-eight patients (41%) the VEGF antigen was expressed strongly. The median survival time of the weak VEGF expression group was 24 months, and that of the strong VEGF expression group was 19 months. The three year-survival rates were 35%, 33%, respectively. The survival difference between both groups was not statistically significnat. CONCLUSION: Although results were not statistically significant, the strong expression group tended to poorer prognosis than weak expression group.

4.Myeloid-Derived Suppressor Cells in Inflammatory Bowel Disease.

Immature myeloid cells, also known as myeloid-derived suppressor cells (MDSCs), include neutrophilic and monocytic myeloid cells, and are found in inflammatory loci and secondary lymphoid organs in mice with intestinal inflammation, inflammatory bowel disease (IBD) patients, and tumor tissues. However, the roles of MDSCs in IBD are not yet well understood, and there are controversies regarding their immunosuppressive functions in IBD. In addition, recent studies have suggested that endoplasmic reticulum (ER) stress in intestinal epithelial cells, especially in Paneth cells, is closely associated with the induction of IBD. However, the ER stress in MDSCs accumulated in the inflamed tissues of IBD patients is not yet fully understood. In the current review, we discuss the presence of accumulated MDSCs in the intestines of IBD patients, and further speculate on their physiological roles in the inflammatory condition with interleukin 17-producing cells, including Th17 cells. In particular, we will discuss the divergent functions of MDSCs in ER stressed intestinal environments, including their pro-inflammatory or immunosuppressive roles, based on the consideration of unfolded protein responses initiated in intestinal epithelial cells by ER stress.

6.Two Cases of Spleen Tuberculosis.

Tuberculosis is a common chronic infectious disease, although the spleen is an uncommon organ to harbor tubercle bacilli. Immunocompromised subjects are primarily prone to miliary tuberculosis and in them the spleen is invaded by Mycobacterium tuberculosis. Spleen tuberculosis is manifested commonly as a miliary form. The basic pathology is granulomatous inflammation. The CT finding of splenic tuberculosis are multiple, well-defined, roung or ovoid, low-density masses. Lymphadenopathy in the abdomen and mediastinum and pleural effusion can be found. We report two cases with tuberculosis of the spleen proved by computed tomography and histologic identification. One paitient did not improve following antituberculous medication, so splenectomy was performed. The other patient has been treated with antituberculous medication.

9.Clinical Rewiew of Stomach Cancer in Young Patients.

Several studies have demonstrated the poor prognosis for young patients with gastric cancer. However few reports exist about the dependence of the survival rate on curative resection and cancer stage. Hospital records of 107 young gastric cancer patients who underwent operations from 1980 to 1996 were reviewed, and the results were compared with those from 3176 cases involving whole age gastric cancer patients. There were no significant differences in operation method, tumor invasion, and tumor stage between both age groups. In whole age patients, males were predominant; however in young patients, females were predominant (p=0.005). In young patients, the tumors more frequently occurred in the upper 1/3 than in whole age patients(p<0.0001). A comparison of histologic types showed that poorly differentiated histologic types, such as signet ring cells and diffuse types, according to Lauren's classification, were more frequently observed in young patients (p<0.0001). The 5- year overall survival rate revealed a difference, but with borderline significance, between the young and the whole age patients (39.4% vs 48.6%, p=0.0505). However, patients who had undergone a curative resection showed no statistically significant difference(56.9% vs 63.9%; p=0.264). And patients with noncurative resection showed a statistically significant difference(0% vs 11.7%, p=0.034). The survival rate according to TNM stage revealed a statistically significant difference at stages I b and III b (Stage I b - 100% vs 84.5%, p=0.047; Stage III b - 8.8%, vs 28.0%, p=0.049). In conclusion, young patients with stomach cancer who underwent a curative resection showed a similar survival rate to that of whole age patients, and the largest survival rate was for cancer in a relatively early stage. Hence curative resection and early diagnosis are mandatory in young patients with stomach cancer.

10.A Double-Blind, Sham-Controlled, Pilot Study to Assess the Effects of the Concomitant Use of Transcranial Direct Current Stimulation with the Computer Assisted Cognitive Rehabilitation to the Prefrontal Cortex on Cognitive Functions in Patients with Strok.

OBJECTIVE: To examine the synergistic effects of both computer-assisted cognitive rehabilitation (CACR) and transcranial direct current stimulation (tDCS) on cognitive function in patients with stroke. METHODS: The current double-blind, sham-controlled study enrolled a total of 11 patients who were newly diagnosed with stroke. The patients of the tDCS group (n=6) completed sessions of the Korean computer-assisted cognitive rehabilitation program five times a week for 30 minutes a session during a mean period of 18.5 days concomitantly with the anodal tDCS over the bilateral prefrontal cortex combined with the CACR. The patients of the control group (n=5) also completed sessions of the sham stimulation during a mean period of 17.8 days. Anodal tDCS over bilateral prefrontal cortex (F3 and F4 in 10-20 EEG system) was delivered for 30 minutes at an intensity of 2 mA. Cathode electrodes were applied to the non-dominant arm. All the patients were evaluated using the Seoul Computerized Neuropsychological Test (SCNT) and the Korean Mini-Mental State Examination. RESULTS: Mann-Whitney U test revealed a significant difference between the two groups. The patients of the tDCS group achieved a significant improvement in the post/pre ratio of auditory continuous performance test and visual continuous performance test on the SCNT items. CONCLUSION: Our results indicate that the concomitant use of the tDCS with CACR to the prefrontal cortex may provide additional beneficial effects in improving the cognitive dysfunction for patients with stroke.
