2.Relationships between Health, Depression, Memory Self-Efficacy and Metamemory in Adults.

Defining prediction variables related to meta memory for the adults in aging process has worthwhile meaning from the perspective that the produced results can be helpful to reducing the difficulty of memorizing efforts and it can also enhance quality of life of aged. This study attempted to analysis relationship between perceived health status, depression, memory self-efficacy and metamemory for the subjects of middle age and old age adults. This study was designed by adopting descriptive correlational analysis method for the 468 middle and old age adults who are living in Seoul. Samples were selected by convenience sampling. Data collection was done over I month period in june 1998. The instuments used in this study were health status measuring scale including depression measuring scale, memory self efficacy measuring scale and metamemory measuring scale which were verified for reliability. Data collected were analized by using SPSS for frequency, Peason correlation, t-test and ANOVA according to the variables character and the study purposes. Results of the study were as follows. 1. Relationship between perceived health status, depression, memory self-efficacy and metamemory. Relational analyses between perceived health status, depression, memory self-efficacy and metamemory supported the hypotheses of 1st, 2nd and 3rd(p .001) These results suggested that the aged perceived great health status then their memory self-efficacy, and metamemory showed the high scores. In the case of depression when its level became decreased metamemory was inclined to increased. Thus, it is identified that strong relationship exists between these variables. 2. Perceived health status, depression, memory self-efficacy by subject's general characteristics. Scores of perceived health status were high in the group of man compared to the group of women, and also highly educated group showed great perceived health status. Group of persons having occupation showed high score of perceived health status and low depression score. The score of memory self-efficacy and metamemory showed higher in the middle aged than the old aged. The high scores of memory self-efficacy and metamemory were found in the group of highly educated people and who have continuing education. The high scores of memory self-efficacy were found in the group of persons having their job and high metamemory scores found in the group of persons having religion. In summary, the greater perceived health status and memory self-efficacy, the more metamemory scores were likely increased and the more depression level was decreased, the more metamemory was likely increased. Also it was found that general characteristics like educational level, continuing education and religion influenced the metamemory of the aged. Therfore, prevention the aged from getting depression and activation of health promotion are needed to delay time of memory loss.

3.Stabilization of Fractures and Fracture-Dislocations of the Thoracolumbar Spine

There are many hypotheses of spinal instability after trauma and the concept had been recently changed from 2-column concept by Holdsworth to 3-column concept by Francis Denis & McAfee and also there are controversies about the operative management of the unstable thoracolumbar spine fractures and fracture-dislocations. So we tried to find some relationship between the spinal instability and the effect of the stabilization operation for the fractures and fracture-dislocations in the thoracolumbar spine in this study. Totally 53 cases that stabilization operation had been performed for the unstable fractures and fracture-dislocations of the thoracolumbar spine at Yonsei University Hospital, including Yongdong hospital, from March, 1973 to October, 1984 and followed up over 6 months, were reviewed. 1. Majority of the patients (84.9%) was in an active age group (20–49 years of his age) and more common in males. 2. The first lumbar vertebra was most commonly involved segment (43.3%) and falling from a height was the most common cause of injuries (50.9%). 3. Injuries to the anterior, middle and posterior column of the spine (58.5%) was the most common mechanism of injury according to the classification by Francis Denis and McAfee. 4. The wedged deformity of the involved segment over 50% was usual (71.7%) and mean wedged deformity was 57% of the height of the vertebral body and most commonly associated fractures besides the body itself was laminae fractures (46.5%). 5. Seventy eight percent of the angular deformity was corrected after Luque rod instrumentation and 76%, after Harrington rod instrumentation, 68%, after posterior wiring, 58%, after anterior decompression and anterior interbody fusion. 6. Forty twa point seven percent of the displacement in anteroposterior plain X-ray film and 58.8% in the lateral film were corrected by Luque rod instrumentation and 39.5% in anteroposterior film and 44.3% in the lateral film were corrected by Harrington rod instrumentation. 7. No significant differences in the recovery of the neurologic signs were found according to the method of surgical treatments. 8. So Luque rod or Harrington rod instrumentation is now recommended for obtaining the spinal stability as a method of surgical treatment in the unstable thoracolumbar spine fractures and fracture-dislocations.

4.Two Cases of Recovery of Ovarian Function and Spontaneous Pregnancy in Women Who Were Diagnosed as Premature Ovarian Failure.

Hepatoid carcinoma is a rare type of malignant tumor resembling hepatocellular carcinoma that arises in extrahepatic sites.(stomach, lung, ovary, pancreas, bladder and renal pelvis). Hepatoid carcinoma of the ovary is an extremely rare ovarian tumor, first described by Ishikura and Scully in 1987. Histologically it is important to differentiate this entity from other oxyphil tumors of the ovary as it requires aggressive treatment. We have experienced a case of hepatoid carcinoma of the ovary in 69-year-old postmenopausal woman, who has been treated with operation and adjuvant Taxol - cisplatin chemotherapy. We present this case with brief review of literatures.

5.Risk Factors for Work Related Low Back Pain in Civil Airline Flight Attendants.

BACKGROUND: The purpose of this study was to identify risk factors for low back pain in order to provide basic information for intervention programs to prevent low back pain in civil airline flight attendants. METHODS: Cross sectional observation was used by using systematic randomized sampling method. Among 1,764 study population, 398 people were selected. For statistical analysis, 210 completed questionnaires were used. Data was analyzed using SPSS 11.0 for t-test, chi square and regression. RESULTS: 1. Positive low back pain rate was 58.7% in the whole study group. In the highest positive group age ranged between 25 to 29 years, and career duration between 37 to 60 months. 2. According to job position, the risk of low back pain was the highest in economy class senior, cabin sales charge, and economy class junior group. On the contrary, cabin manager was the lowest risk group. 3. According to the average score of job stress, low job control and low supervisory support were each associated with higher positive rate of low back pain. 4. Wide degree of bending, more frequent waist movement and heavy lifting showed high positive rate of low back pain. CONCLUSION: Low back pain prevention program should be developed on the basis of the confirmed risk factors. These data will be helpful to provide prevention strategy and to promote intervention program for low back pain in airline flight attendants.

7.The Hawthorne Effect between Covert and Overt Observations in the Monitoring of Hand Hygiene Adherence among Healthcare Personnel at Coronary Care Unit and Cardiac Surgery Intensive Care Unit.

BACKGROUND: Direct observation of healthcare workers is commonly used in hospitals to investigate hand hygiene compliance. However, the hand hygiene compliance rate may increase due to the Hawthorne effect, which is the modification of behavior simply because subjects become aware that they are being observed. The objective of this study was to investigate the occurrence of the Hawthorne effect when directly observing hand hygiene compliance in intensive care unit (ICU) healthcare personnel. METHODS: A total of 87 staff members from the coronary care unit and cardiac surgery ICU of a general hospital in Seoul were included in this study: 24 residents and interns, 55 nurses, and 8 nursing assistants. Both covert and overt observations, where subjects were either unaware or aware of any direct observation, were performed on separate occasions. RESULTS: A total of 1,052 covert and 1,336 overt observations were documented over 30 and 34 occasions, respectively. Overall hand hygiene compliance was significantly higher with overt observation than with covert observation (1,041/1,336, 77.9% vs. 659/1,052, 62.6%, P<0.001). The Hawthorne effect was present in all professions and behaviors, with the exception of nursing assistants, and prior to touching a patient. CONCLUSION: Direct observation of hand hygiene compliance was associated with the Hawthorne effect when observations were made overtly and this was likely to contribute to an overestimation of compliance rate.

8.The perforator-based myocutaneous island flap in the reconstruction of sore and perineal wound.

The management of skin and soft tissue defects on the buttock, perineum and lower abdomen has been improved through the development of myocutaneous flap. However, sacrifice of the deep muscles causes some problems such as intraoperative bleeding and functional disabilities of donor sites. And we can not also control the volume of the flap for the skin reliability of myocutaneous flaps. To overcome these disadvantages, we tried perforator-based myocutaneous island flaps for the reconstruction of the sacral, ischial sores or deep wounds of the perineum and lower abdomen. During the dissection of the flap, we could control the volume and shape of the flap exactly depending on the perforators. Clinically, total of 14 cases were treated with 10 parasacral perforator-based myocutaneous island flaps and 4 perforator-based extended myocutaneous island flaps. Parasacral flap is based on the parasacral perforators along the lateral sacral border and, the degree of containing muscles can be easily decided according to the depth of wound. In the cases of the extended myocutaneous flaps, we could design the perforator-based skin flap on the tip of myocutaneous island flap and appropriate volume of skin falp was obtained with the wide arc of rotation. Finally, the donor defect could be closed primarily and there was no significant sequela. There perforator-based myocutaneous island flaps require no significant sacrifice of the muscles and can be easily dissected and applied with the appropriate volumes for the reconstruction of the defect on the buttocks, perineum and lower abdomen.

9.The parasacral perforator-based island skin flaps for sacral pressure sores.

The reconstruction of pressure sore is often complicated problem due to underlying disease and general condition of the patients. Sometimes, it is too radical to use the muscle or musculocutaneous flap for the patients who has severe systemic disease and can not withstand the lengthy operation or the patients who already recovered from paraplegic condition and has no risk recurrence. To get over these problems, we recently have used the parasacral perforator-based island skin flaps for the sacral pressure sores. This flap is based on the axial pattern perforators originating from the internal pudendal artery and lateral sacarl artery. We performed this operation for 11 sacral sores with reasonable results. There was no significant complication, and duration of operation was relatively short and blood loss was a little. We concluded that this flap was useful for the patient with sacral pressure sore who could be categorized nontraumatic nonparaplegic group(NTNP group) or nontraumatic paraplegic group(NTP group). But this island flap has drawback, which is elimination of sensory nerve within the flap.

10.The value of elevated second trimester human chorionic gonadotropin levels in predicting development of pregnancy - induced hypertension.

OBJECTIVE: This study was designed to evaluate whether maternal serum human chorionic gonadotropin(hCG) level measured during second trimester could be used as a predictor of pregnancy-induced hypertension. METHODS: Between March 1996 and November l997, 505 pregnant women undergoing second trimester(14-22 weeks of amenorrhea) triple marker screening for neural tube defect and Down syndrome who delivered at our institution were reviewed. All samples were assayed in the same laboratory and medical records were reviewed about medical and obstetrical history. RESULTS: Maternal serum human chorionic gonadotropin level(multiples of the median, MoM) was significantly higher in the pregnancy-induced hypertension group than in the control group. With the use of 2. l multiples of the median as a cutoff value, the sensitivity of human chorionic gonadotropin as a screening marker for development of pregnancy-induced hypertension was 38.7%, the specificity was 95.9%, positive predictive value was 38.7% and negative predictive value was 95.9%. Conlusion: We suggest that second trimester maternal serum human chorionic gonadotropin levels can be used clinically as an early predictive marker for pregnancy-induced hypertension.
