2.The Students' Evaluation of Practice Sites and Preceptors in Pharmacy Experiential Education.

OBJECTIVE: This study was conducted to assess the current status of experiential education, by analyzing pharmacy students' satisfaction and evaluation for practice sites and preceptors for 3 years from 2014 through 2016. METHODS: Students evaluated the practice sites and preceptors using a 5-point and a 4-point Likert scale, respectively. Analysis was performed based on the types of curriculums and practice sites. In addition, sub-analysis was conducted based on the types of health-system pharmacy and the community pharmacy. RESULTS: Total 203 students responded to the survey questionnaires. The 3-year average score of students' satisfaction with practice sites was 4.25. In the sub-analysis conducted based on the types of health-system pharmacy, the score was higher in the tertiary hospital pharmacies than that of the general hospital pharmacies (4.49 vs. 4.06, P<0.001). For community pharmacy, the difference in the score of the 3-year analysis was not significant, depending on the types (4.51 vs. 4.33, P=0.054). The average score of students' evaluation on the preceptors was 3.37 in the 3-year analysis. The score was significantly higher for the preceptors of tertiary hospital pharmacies than those of general hospital pharmacies (3.61 vs. 3.25, P<0.001). For community pharmacy, no significant differences were found, depending on the types of pharmacy (3.53 vs. 3.43, P=0.309). CONCLUSION: The students' satisfaction with practice sites and preceptors in pharmacy experiential education was high. However, it varied greatly depending on the types of educational institutions. Mutual effort between university and educational institutions is required, to narrow the gap in the degree of students' satisfaction.

3.Unilateral Ptosis Due to Isolated Levator Myositis.

PURPOSE: To present a rare case of idiopathic orbital myositis involving levator palpebrae superioris. CASE SUMMARY: A 27-year-old male presented with a 1-week history of redness, discomfort, swelling, and drooping of his left upper eyelid. A computed tomography scan showed isolated enlargement of the right superior rectus/levator muscle complex. On examination, there was a left blepharoptosis, although eye movements were normal. The authors treated the patient with 3rd-generation cephalosporin; however, after 3 days, the symptoms did not improve. Subsequently, the patient was diagnosed with idiopathic orbital myositis and treated with oral corticosteroids for 1 month; the symptoms gradually resolved. CONCLUSIONS: Idiopathic orbital myositis is a subtype of nonspecific orbital inflammation primarily involving the extraocular muscles. Although the exact cause of orbital myositis is unknown, an immune-mediated pathophysiologic mechanism appears to be one of the causes. Medial rectus myositis is the most common, and isolated levator muscle myositis is very rare. The authors of the present study reported a case of orbital myositis involving the levator palpebrae superioris which should be considered a differential diagnosis of blepharoptosis with eyelid swelling.

4.Clinical Study of Facial Psoriasis.

BACKGROUND: Many clinicians tend to assume that psoriasis rarely involves the face, and therefore have little interest in facial psoriasis. However, the face is the most important area of cosmetic concern and social activity, and facial involvement of psoriasis is more common than we think. A few reports have even suggested that facial psoriasis could be a marker of severe psoriasis. OBJECTIVE: To investigate the epidemiological and clinical characteristics of facial psoriasis. METHODS: A total of 235 patients with facial psoriasis were enrolled in this retrospective study at the psoriasis clinic, Department of Dermatology, Seoul National University. Epidemiological and clinical data from patient records was collected and analyzed. RESULTS: The most frequent age of onset for facial psoriasis was in the 3rd decade of life. The peak age of onset of facial psoriasis in male patients was in the 3rd and 4th decade of life, whilst for female patients it was in the 3rd decade. Most patients had presented with facial psoriasis before they were 40. The sites of the face most often affected were the upper (73.9%) and lower (46.2%) aspects of the forehead and periauricular area (45.7%). The eyelid, perioral area and nasolabial fold were relatively unaffected. Patients who had scalp-unrelated facial psoriasis presented with a more severe condition than patients who only had scalp-related facial psoriasis. With reference to the extent of psoriasis, 43.8% of patients were moderate cases, whereas 29.6% were mild and 26.6% were severe. With reference to the activity, 40.3% of patients were mild, while 38.5% were moderate and 21.2% were severe. Family history was observed in 39.9% of patients and nail involvement was observed in 27.7% of patients. Only 38.2% of patients had no pruritus, while 16.3% of patients experienced moderate or severe pruritus. CONCLUSION: The present retrospective analysis contributes to our understanding of the clinical characteristics of facial psoriasis.

5.Molecular Genetic Study on Human Meningiomas:Loss of Heterozygosity on Chromosome 22.

With the methods of restriction fragment length polymorphisms(RFLPs) and Southern blot, we performed a molecular genetic study in 20 cases of intracranial meningiomas. There were four cases of invasive meningiomas, two cases of recurrent meningiomas and one case of central neurofibromatsis. Comparison of the alleles detected by DNA probes in normal versus tumor DNA revealed chromosomal alteration. The following probes were used : ESR(chromosome 6q), pYN22, pYNH37.3(chromosome 17p), cEFD52(chromosome 17q), D22S10, IGLC2(chromosome 22q). In seventeen of 20 cases(85%), constitutional heterozygosity was observed for IGLC2 probe on chromosome 22q and loss of heterozygosity was detected in nine of 17 informative cases(53%). There was no loss of heterozygosity for other probes. Three of 4 cases of invasive meningiomas and one case of central neruofibromatosis displayed loss of heterozygosity.

7.Screening Tests for Early Diagnosis of Multiple Myeloma and Related Plasma Cell Disorders

Monoclonal gammopathy (MG) encompasses a diverse group of disorders characterized by the secretion of monoclonal immunoglobulins or their light-chain components. The incidence of multiple myeloma (MM) in South Korea is rapidly increasing, and it is important to be aware of its initial clinical presentations and the most efficient laboratory algorithms for early detection. Serum protein electrophoresis (SPE) and urine protein electrophoresis (UPE) are the primary screening tests for patients with clinically suspected MM or amyloid light-chain amyloidosis; these tests are reimbursed in South Korea. We reviewed clinical studies that applied national and international guidelines to evaluate test panels for early detection of MGs, including MM. The serum free light chain (sFLC) with SPE panel is recommended for the initial work up for diagnosis of MGs. In the case of a normal SPE, sFLC should be measured subsequently, so as not to miss the presence of M-protein. Use of this screening panel avoids medical expenses related to delayed diagnosis. Guidelines and recommendations suggest that no single method (SPE, serum immunofixation electrophoresis, sFLC, or UPE) should be used to exclude a diagnosis of MM. We believe that a screening test panel comprising SPE plus sFLC will increase the rate of early and accurate diagnosis of MM and related disorders.
8.Prevalence and Its Correlates of Night Eating Syndrome in Schizophrenic Outpatients.

OBJECTIVES: The aim of study was to examine the prevalence of night eating syndrome(NES) and its correlates in schizophrenic outpatients. METHODS: The 14 items of self-reported night eating questionnaire(NEQ) was administered to 201 schizophrenic patients in psychiatric outpatient clinic. We examined demographic and clinical characteristics, body mass index(BMI), subjective measures of mood, sleep, binge eating, and weight-related quality of life using Beck's Depression Inventory(BDI), Pittsburgh Sleep Quality Index(PSQI), Binge Eating Scale(BES) and Korean version of Obesity-Related Quality of Life Scale(KOQoL), respectively. RESULTS: The prevalence of night eaters in schizophrenic outpatients was 10.4%(21 of 201). Comparisons between NES group and non-NES group revealed no significant differences in sociodemographic characteristics, clinical status and BMI. Compared to non-NES, patients with NES reported significantly greater depressed mood and sleep disturbance, more binge eating pattern, and decreased weight-related quality of life. While 'morning anorexia' and 'delayed morning meal'(2 of 5 NES core components in NEQ) were not differed between groups, 'nocturnal ingestions', 'evening hyperphagia', and 'mood/sleep' were more impaired in NES group. CONCLUSION: These findings are the first to describe the prevalence and its correlates of night eaters in schizophrenic outpatients. These results suggest that NES has negative mental health implications, although it was not associated with obesity. Further study to generalize these results is required.

10.Association of Temporal Lobe Atrophy and Psychosis in Patients with Alzheimer's Disease.

OBJECTIVE: The purpose of this study is to investigate the difference on regional volume in temporal lobe between Alzheimer's disease patients with psychosis (AD+P) and Alzheimer's disease patients without psychosis (AD-P). METHODS: Altogether, 24 AD+P and 25 AD-P matched age, gender, and clinical dementia rating sum of box (CDR-SOB) were include from a Memory impairment clinics of Pusan National University Hospital in Korea. AD+P were diagnosed according to Jeste and Finkel's proposed diagnostic criteria for psychosis of Alzheimer's disease. Grey matter volume of temporal lobe was measured with 3-tesla magnetic resonance imaging and freesufer analysis. Analysis of variance was used to investigate the association between temporal lobe and AD+P after controlling age, gender, education years, CDR-SOB and total intracranial volume. RESULTS: We found an association between AD+P and reduced grey matter volume in total temporal lobe as well as in specific temporal regions such as left middle temporal lobe, left inferior temporal lobe, both hippocampus and both fusiform. CONCLUSION: Our findings suggest that AD+P are associated with reduced grey matter volume of temporal lobe.
