2.CS-3000 Plus Cell Separator; A-35 Collection Chamber vs PLT-30TM Collection Chamber.

BACKGROUND: Recently, it has been established that plateletpheresis needs more efficiency and shorter processing time. Fenwall laboratories developed a new collection chamber for CS-3000 Plus, PLT-30TM collection chamber, which can reduce the processing time with efficient collection. We evaluated the PLT-30TM collection chamber by comparing it with A-35 collection chamber that has been used as a standard collection chamber of CS-3000 Plus us. METHODS: Thirty platelet collection procedures were performed using the CS 3000 Plus with A-35 collection chamber and PLT-30TM collection chamber. The changes of the hematologic parameters between pre- and post-donation in donors and the total platelets yields and the contaminated WBCs in the plateletpheresis products were evaluated. In processing, the yield predictor calibration was adjusted to 1.00 and 1.13 in A-35 and PLT-30TM respectively. Yield predictors of pheresis were the same as 3.5x1011 in both and end point volumes were calculated from the CS-3000 Plus. Processing volume and processing times were compared between A-35 and PLT-30TM groups. RESULTS: With PLT-30TM collection chamber, 3.38+/-0.72x1011/L platelets were harvested, whereas 3.20+/- 0.73x1011/L were collected with A-35 collection chamber, which was not significantly different. But processing time with the PLT-30TM collection chamber was more reduced than that with the A-35 collection chamber by about 20 minutes (PLT-30TM : 88.6+/-8.4 min, A-35 : 106.7+/-11.7min). Collection efficiency of PLT-30TM chamber was 50.7+/-12.5% and that of A-35 chamber was 44.4 + 8.8%. The leukocyte contamination of the platelet concentrates were not statistically different(PLT-30TM: 0.0-3.6x106, A-35 : 0.1-4.1x106). CONCLUSIONS: PLT-30TM collection chamber has the advantages of shortening the donation time and decreasing the processing volume with better collection efficiency and flexibility of platelet concentrate volume.

Clothing dermatitis is a rare disease. The distribution of the eruption coincides with the places on the skin where the garment fits most snugly. It is developed by the chemicals and the dyes rather than textile itself. We experienced 2 cases of clothing dermatitis, one of which was caused by formaldehyde in clothing.

6.Orthrognathic Surgery in Severe Chronic Renal Failure Patient.

The kidneys play a vital role in the maintenance of normal body fluid volumes and in the composition of the extracellular fluid compartments. There are normally more than 2 million functioning glomeruli that regulate total body water and solute concentrations. As renal failure progresses, there is a decrease in the number of functioning nephrons. Chronic renal failure(CRF) is the consequence of a multitude of diseases that cause permanent destruction of the nephron. Azotemia is an elevation in blood urea nitrogen(BUN) and serum creatinine levels subsequent to a decreased glomerular filtration rate(GFR), which results in uremia. This loss of renal function can cause functional and metabolic abnomalities of body. For this problem, oral & maxillofacial surgeons have demanded to routinely treat patients with CRF. However, there has not been a reported case of orthognathic surgery by bilateral sagittal split ramus osteotomy(BSSRO) in patients with CRF, which can cause multiple complications in healthy patients. We report developmental mechanism of complication associated with CRF and preop. and postop. care of orthognathic surgery by BSSRO in ClIII patient with severe chronic renal failure.

7.Size-reductive Neoadjuvant Immunotherapy using Imiquimod in Squamous Cell Carcinoma of the Lower Lip.

rcinoma (SCC) is the second most common skin cancer. In particular, invasive SCC has a high risk of metastasis and sometimes, this can be fatal. At present, the treatment of choice for invasive SCC is complete excision with a proper surgical margin, but most cases of SCC develop on cosmetically-sensitive areas, so sometimes significant scarring or deformities can lead to an unhappy result. An 80 year-old female presented with a 4x1.5cm sized SCC on her lower lip. Although surgery was the first line therapy for this invasive SCC, a cosmetically poor outcome was expected. Thus the patient did not want to undergo a surgical operation, so we had to explore other treatment options. Recently there have been many reports that show imiquimod 5% cream is effective in the treatment of invasive SCC. Therefore we started topical therapy with imiquimod 5% cream five times a week. After 6 months, the lesion size was reduced to 2x1.5cm and we could remove the remnant tumor by wedge resection with cosmetically acceptable result. We suggest this neo-adjuvant immunotherapy can be an alternative for the treatment of cosmetically-critical SCC and present this case as a good example that has shown successful size-reductive neoadjuvant immunotherapy using imiquimod in invasive SCC.

8.Treatment of the Early Mycosis Fungoides.

BACKGROUND: Mycosis fungoides (MF) is the most frequent form of cutaneous lymphoma. Early stage MF is known to be responsible to the various topical and systemic therapies. However, there have been few clinical reviews of the treatment modalities of the early stoge MF in Korea. OBJECTIVE: The aim of this study is to investigate the therapeutic response of early MF to different treatment modalities. METHODS: The medical records, clinical photographs, and biopsied tissues, of fifty three cases of early MF were reviewed. RESULTS: The mean age at diagnosis of early MF was 43.0 years old (varied between 9 and 81 years old). Twenty three (43.4%) were male and thirty (56.6%) were female. Topical carmustine (bis-chloroethyl-nitrosourea, BCNU) was the most commonly used treatment modality in the early MF (34.0%). Patients treated by BCNU in stage IA (<10% skin involved) showed a response rate of 85.7%. Early MF recorred in only one patient after 93 months. However BCNU was not effective for the patients in stage IB (> or =10% skin involved). Even Psoralen plus UVA (PUVA) therapy, which was most efficient in stage IB, revealed a high relapse rate after one year. CONCLUSION: BCNU has been most commonly used for the treatment of early MF during the past 10 years in the authors' hospital. It showed a comparatively high response rate, and the relapse rate of BCNU was lower than for other therapies in stage IA. However many treatments became ineffective when MF progressed to stage IB. Therefore the authors confirm that early diagnosis and proper therapy are most important for the treatment of early MF.

10.Clinical Characteristics of Chronic Rhinosinusitis With Nasal Polyp According to Histopathological Endotypes and Staining Method for Neutrophilic Polyp Classification and Its Clinical Implication

Background and Objectives:
There are various methods used to histologically assess the cell count of neutrophils in nasal polyps. With various classification of neutrophilic nasal polyps, determining the appropriate staining techniques and criteria is deemed important for the diagnosis of neutrophilic nasal polyps by assessing neutrophil cell counts. In this study, we tried to optimize different methods of neutrophil counting and staining by comparing clinical outcomes.Subjects and Method We studied 207 patients who underwent endoscopic sinus surgery for chronic rhinosinusitis with nasal polyp from August 2014 to September 2018. Patients’ nasal polyp were taken to make following three slide sections: hematoxylin and eosin, human neutrophil elastase (HNE), myeloperoxidase. After staining, microscope picture (×400) was taken and neutrophils were counted. Neutrophil polyp (NP) was classified either by the tissue neutrophil count (20/HPF) or by the neutrophil percentage (1%/total cell) for clinical outcome comparison.
Results:
Two groups were made after HNE staining: the NP and the non-NP groups. The L-K scores and delta showed significant differences between each group while neutrophil was defined based on 20 counts/HPF (p<0.001). By Kaplan-Meier estimation, there was no statistical meaning between the two groups. But the NP group had a tendency to recur more than the non-NP group while 20 counts/HPF definition was used (p>0.05).
Conclusion
HNE staining and 20 counts/HPF were the most specific for neutrophil counting. Analyzing the nasal polyp into two groups due to histologic types showed that the NP group had worse prognosis than the non-NP group. In conclusion, neutrophils in the nasal polyp might be considered seriously for clinical outcomes.
