4.CHANGES OF CYCLINS, CYCLIN DEPENDENT KINASES, CYCLIN DEPENDENT KINASE INHIBITORS DURING GLOSSAL DEVELOPMENT IN THE RATS.

The molecular mechanisms that regulate glossal muscle cell cycle and terminal differentiation remain largely unknown. To determine which cyclins, cyclin dependent kinases (CDKs), cyclin dependent kinase inhibitors (CKIs) are important for glossal cell proliferation, we have examined expression of cyclins CDKs, CKIs during normal glossal muscle development in the rat. All cyclins, CDKs, and KIP/CIP family of CKIs were highly expressed during fetal glossal muscle development, then they decreased at different rates after birth. While the mRNAs of cyclin Dl, D3, E , A, and B decreased gradually in glossal muscle during all stages of development, the protein levels of these cyclins decreased differently in tongue during pre- and postnatal development. While the functionally active formed of cyclin Dl, cyclin D3 and E proteins were observed until 7 days after birth, cyclin A and B proteins were decreased more slowly. While the CDK4, CDK6, CDK2, cdc2, and proliferating cell nuclear antigen (PCNA) proteins were higllly present during fetal glossal muscle development and gradually decreased during postnatal development. Particularly, cdc2 levels decreased markedly after birth. Immunohistochemical data for PCNA was consistent with Western blotting data for PCNA temporally and spatially. The mRNA and protein levels of p21, p27, and p57 were high, then their levels changed differently during glossal development. While the mRNA levels of p21 and p57 decreased gradually, the mRNA level of p27 did not change during glossal development. While the protein levels of p21 and p57 in tongue decreased markedly after birth, the protein levels of p27 increased slightly after birth, then decreased at adulthood. These findings suggest that the all cyclins and CDKs observed are involved in glossal muscle cell cycle, and reduction of cyclins and CDKs and induction of p21 are associated with the withdrawal of glossal muscle cell cycle after birth.

7.An Experimental Study and Clinical Investigation on Rotational Deformity in Supracondylar Fracture of the Humerus in Children

In children, the supracondylar fractures of the humerus are common in elbow region, and often associated with various complications and problems in treatment (ie,Volkmann's ischemic contracture, cubitus valgus or varus deformity, rotational deformity). The results of reduction can be measured by Baumann's angle, carrying angle, trochlea-capitellum angle, and displacement of distal radial styloid process on radiological study. So we have made experiments on the process which various three dimensional displacements reflected into two dimensional Baumann's angle and humerocapitellar angle. And we investigated the process of rotation, through various radiological studies by means of molded models of humerus. The results were as follows:l. In experiments, AP & lateral X-ray studies showed no change in rotation of proximal segment of fracture with fixed distal segment of fracture. 2. On the other hand, AP & lateral X-ray showed significant change, in rotation of proximal fracture segment with distal segment of fracture. 3. Supracondylar rotation angle(S.R.A.)depicted as the ratio of the length of proximal fracture width minus that of the distal fracture width in lateral view, to the length of distal fracture width in AP view minus that of the distal fracture width in lateral view. 4. According to the modified Mitchell result, S.R.A. to the result of treatment was average 7.6 degree in excellent group, 15.2 degree in good group and 29.3 degree in unsatisfactory group.

9.A Clinical Study of Essential Hypertension in Koreans.

A clinical study was performed on 940 cases of essential hypertension observed at Dept. of Internal Medicine of Seoul National University Hospital during a period of last seven years from 1971 to 1977. The followings were the results: 1. The incidence of hypertensive patients was 13.8% among all medical in-patients and 68.2% among all cardiac in-patients. 2. The highest occurence was observed in sixth decade and accounted for 34.2% of all cases and least occurence was in second decade of life which accounted for 0.6% of all cases. No difference was found in age distribution between both sexes. The male to female ratio was 1.2:1. 3. The mean value of blood pressure was 178.1+/-21.8/107.4+/-12.3 mmHg and tended to increase with age at least over the decades from second to seventh and there was tendency to decrease after the eighth decade. No paticular difference in blood pressure levels was found in both sexes. 4. The most outstanding symptom was headache which was found in 51.5% of all cases. Dizziness, dyspnea and palpitation were the next orders in frequency. There was a general trend of the the subjective symptoms which were more pronounced in younger than older patients. 5. Hypertensive retinopathy was found in 83.3% of all cases. Hypertensive retinopathy of K-W grade II was found in 36.1% of all cases and was the most frequent finding among the Keith-Wagener grades. Malignant hypertension of K-W grade IV was found in 4.2% of all cases and the largest numbers were between the fourth and fifth decade of life. 6. The abnormal retinal finding was found to be rather proportional to the blood pressure levels. 7. Albuminuria was detected in 56.2% of all cases and it was more frequent in male than in female patients. 8. The appearance of albuminuria seemed to have a close correlation with hypertensive retinal change and blood pressure levels. 9. Electrocardiographic abnormalities were associated in 71.9% of all cases. The most frequent association among electrocardiographic abnormalities was left ventricular hypertrophy which accounted for 45.2% of all cases. There was the reverse correlation between the normal electrocardiogram and degree of blood pressures. The finding of left ventricular hypertrophy and degree of blood pressures were closely correlated. 10. Cerebrovascular accident, renal failure, congestive heart failure and atrial fibrillation were four major complications. It was recorded in 26.5%, 10.4%, 5.4% and 2.0% of all cases respectively. The complications were common in men than in women. 11. Renal failure, congestive heart failure and myocardial infarction were closely corelated with hypertensive retinopathy.

10.Resurfacing of the Open Wound of the Hand with Free Arterialized Venous Falp.

Since introduction of venous flap in 1980, many experimental studies and clinical applications of various kinds of venous flaps were reported. Venous flap has the following advantages : (1) nonbulky and goo-quality of flap (2) long & large vascular pedicle (3) easy & rapid elevation of flap (4) no sacrifice of major arteries (5) a single operative field. But, we also have some disadvantages of difficult handling of the pliable veins and the tmcertainty of flap survival. For the better result we had to design the size of the flap larger than that of defect and increase the number of draining vein to reduce the postoperative edema of the flap. We have treated the defects of soft tissue of the hand using free arterialized venous flap from the flexor aspect of the forearm & had an excellent results.
