4.Legal Approach in Internet Medical Counseling.

Internet medical counseling is increasing through the recent diffusion of super-high speed network. It provides not only medical information but also substantial Internet counseling(home medical care or telemedicine) that may rander definite diagnosis and treatment on line. But Internet medical counseling is still prohibited since Korean medical law stipulates the principle of facing treatment. Soon if will be possible to deliver Internet medical counseling under the revised bill of Korean medical law on telemedicine and electronic medical record. Therefore, I would like to give some legal information and advice that merit attention when medical personnel deliver Internet medical counseling. First, in case of substantial Internet medical counseling, it would be desirable to allow Internet counseling as the second treatment with the trust between doctor and patient being the first. Internet counseling should replace facing treatment only when the latter is practically not possible. It should be initiated not by physicians??soliutation but by requests from patients themselves. Of course, full explanation about the practice, including any potential disadvantages to the patients, should be provided. Second, counseling contents should be recorded and the hard copies of the Q-and-A should be kept with signature on. Patients privacy should not be infringed. Third, in case of Internet medical counseling, medical personnel has the same responsibility as that in the facing treatment. And it is stipulated in the revised bill of Korean medical law that if there is no definite evidence that acknowledges remote doctor's fault, resident doctor has the responsibility. But this could make a resident doctor hesitate to practice telemedicine, and enough discussion should follow on this. Internet medical counseiling gives a big opportunity to provide highly developed medical techniques overcoming the geographic barrier of distance, but it also might be dangerous enough to threaten patient's life by providing wrong information and cause effluence of private inforamtion. Therefore, it is necessary to facilitate public opinion regarding the Internet medical counseling on its limit and regulation.

7.A Clinicopathologic Analysis of Pseudomyxoma Peritonei Originated from Mucinous Ovarian Tumors.

OBJECTIVE: The aims of this study were to analyze the natural history of patients with pseudomyxoma peritonei originated from mucinous ovarian tumor, evaluate clinical and pathologic variables, and review our experience with available therapeutic modalities. METHODS: Six patients were treated for pseudomyxoma peritonei originating from mucinous ovarian tumor at Severance Hospital between 1990 and 1998. The data were collected retrospectively and all charts were reviewed RESULTS: The mean age at diagnosis was 51 years, and most common symptoms were abdominal distension. Pseudomyxoma peritonei was found in association with five mucinous ovarian tumors of borderline malignancy, and one ovarian mucinous adenoma. All patients underwent surgical staging and cytoreduction, three patients received postoperative adjuvant chemotherapy and, of these, two developed recurrence. With respect to survival, all patients were alive, four were alive and free of disease, two were alive with disease at the end point of study. CONCLUSION: Pseudomyxoma peritonei is commonly associated with borderline mucinous ovarian tumors, and is a frequently relapsing and protracted disease. But aggressive and repeated debulking surgery is recommended for long survival.

9.Rhiltral Reconstruction in Facial Burn Scars using Fenestrated Auricular Composite Graft.

The surgical correction of postburn nose and lips deformities still remains a difficult task domain to be done by plastic surgery. Consequently, the performance of autogenous cartilage graft and ear cartilage graft to maintain a better philtral form were found to raise the problem of a defect in the unnatural form due to consecutive tension in the region. Recently, focus had been placed on composite graft, including auricular cartilage, to obtain a unique dimple in the philtrum, and concurrently with this, part of its natural form has been acknowledged. However, composite graft on scar beds with poor circulation on the recipient site dose not represent safe survival, has increased risk allotment, which laeds to difficult application. From Jan. 1992 to Dec. 1998, the authors have experienced auricular composite graft in 15 patients who had a defect on the philtrum due to postburn scar contractures. As types of this operation, in method I, the subcutaneous pocket is made at the midline of the upper lip. The next step is cartilage insertion into subcutaneous tissue. The cartilage graft is fixed to be overlying skin by the bolus sutures. In Method II, the recipient site is excavated by some excision of the soft tissue in the central upper lip after scar tissue excision. The obtained auricular skin composite graft is placed and fixed to the philtral area. In Method III the auricular skin composite graft is harvested, and 2 or more 2mm-sized multiple holes are made along the midline of cartilage. Then composite graft is fixed to the defects of philtrum. The elongated portion of the distal tip of cartilage is embedded into the vermilion tubercle for sprouting and fullness. The most effective method was Method III, which enhanced the survival of auricular cartilage graft and its overlying skin, by trimming the margin of grafts, and enhancement of the connecting vessels through fenestrated holes of cartilage between the recipient site and composite graft. The authors hereby report the results of the present study along with study findings based on literature surveys.

10.Clinical Observation of Simple Localized, Esophageal Ulcer.

Incidence of simple localized esophageal ulcer has been infrequent, but recently, clinical report of the disease has increased with remarkable improvement of diagnostic instrument and handling of endoscopy. Nine patients of simple localized esophageal ulcer with abrupt onset of symptoms and rapidhealing process were clinically studied and observed with the other literatures on the basis af age, sex, site, symptoms, causes, endoscopic findings, microscopic findings, and healing pracess. The results were followed. 1) The eiophageal ulcer revealed variable size and shape and generally localized middle third of esophagus. 2) History of therapeutic drug medication auch as capaule or/and tablet was noted mostly. 3) The symptoms were disappeared with general measures and ulcer lesions were comyletely healed without scar or stricture rapidly.
