1.Assessing Muscle Tensions During VDT Works with Surface Electromyography.

OBJECTS: Thls study was designed to assess muscle tensions while working at a YDT workstation with surface electromyography (SEMG). METHODS: Variables texted were desk height, monitor positon on the desk, distance from edge of the desk to the keyboard. Eleven female volunteers working in the tele-com munication company were seated at an ergonomically adjusted workstation. They were assigned to a task of typing for 5 minutes and were given a 10 minutes resting with hands in their lap. For the initial experiment, the desk height was fixed to standard desk height(65.0 cm) recommended by ANSI and latter adjusted to a higher desk height (72.8 cm) for this study such that the subjects experienced slight elevation of shoulders. The monitor position initially at the center on the desk and latter left site of the desk (45o from monitorneck axles). The keyboard was positioned to an optimal distance (15 cm from the desk edge) and an abnormal distance (30 cm). Five muscle groups (paraspinal muscles, upper trapezium muscle, supraspinatus muscle, infraspinatus muscle, and rhomboid muscle) were selected for the study. The data of SEMG were analyzed by 2x2x2xs mixed factorial design. RESULTS: The results were as follows: 1. SEMG activity recorded from the paraspinal muscles were significantly higher when the angle of the monitor adjusted to 45 degree from the center of the desk.. 2. SEMG activity recorded from the upper trapezius/supraspinatus/infraspinatus muscles were significantly higher when the hight of the desk was elevated from the recommended height.3. SEMG activity from the rhomboid muscle showed no significantiy difference in all test conditions. CONCLUSIONS: The results of thls study confirm that the helght of a VDT workstation and the angle of the monitor are risk factors of upper extremity cumulative trauma disorders. Also SEMG would be useful for measuring muscle activities during workstation assessment.

2.Female Urethral Adenocarcinoma Possibly Arising in Paraurethral Gland: A case report.

Female urethral adenocarcinoma is one of the rare tumors. It has been thought to arise in the paraurethral Skene's gland, the transitional epithelium of proximal urethra or the urethral diverticulum. This is to report a urethral adenocarcinoma developed in a 51 year-old patient who had a past history of suburethral abscess 7 years ago, and to discuss its possible histogenetic origin. The tumor was located in the urethral wall and revealed a centrifugal growth pattern toward the anterior wall of uterus and vagina and an upward extension to the bladder neck. The tumor was composed mostly of well differentiated adenocarcinoma and partly of signet ring cell carcinoma. The urethral and bladder epithelia were well preserved without cancerous or precancerous changes, and there was no urethritis glandularis nor cystitis glandularis. The secretary material of the neoplastic glands was weakly positive for prostate specific antigen (PSA) and prostate specific acid phosphatase (PSAP). Although the tumor cells themselves were not reactive to PSA and PSAP, the histologic findings suggest that the urethral adenocarcinoma arises in the paraurethral Skene's gland which had probably been the site of abscess in this patient.

4.Affinity for 57Co-Vitamin B12 by a Wide Histologic Variety of Tumor Types in Mice.

The search for tumor-avid agents for use in nuclear medicine imaging is an ongoing field of importance. The purpose of this study was to determine the affinity for radiolabeled vitamin B12 by a wide histologic variety of tumor types in mice. Seventeen different types of tumor were grown subcutaneously in female Balb/C or Balb nu/nu(nude) mice. When the tumors reached about 1 cm in diameter, mice were injected intraperitoneally with 57Co-vitamin B12. Twenty-foul hours later, the mice were sacrificed. Organs and tissues were removed, weighed, and activity per mg determined by gamma counter. Values represented cpm/mg tissue that was normalized to 20 grams body weight for each mouse. A wide variety of tumor types showed significant uptake and concentration of 57Co-vitamin B12, as evidenced by tumor:tissue activity ratios. For many tissues of great importance in terms of background(bone, muscle, blood), the tumor:tissue activity ratios of uptake were high. These data strongly suggest that further efforts to evaluate the utility of radiolabeled adducts of vitamin B12 for clinical use in oncologic imaging are warranted.

Leukocytes, when subjected to phagocytic, immunologic or chemical stimuli, are known to exhibit a sequence of morphological and biochemical events which lead to the production of H₂, O₂, O₂⁻, and even OH as well as the secretion of various lysosomal lytic enzymes into the extracellular environments. It was been proposed that some antiinflammatory drugs may exert their therapeutic effects by inhibiting the production of reactive oxygen species or by abrogating the effects of reactive oxygen species ontissue components. In the present study, effects of various quenchers and antiinflammatory drugs were observed on the changes of viscosity of hyaluronate and collagen gelation by oxygen radicals generated by xanthine and xanthine oxidase. Quenching effects of antiinflammatory drugs on reactive oxygen species were also observed with gas chromatography. 1. Decrease of viscosity of hyaluronate and inhibition of collagen gelation by xanthine and xanthine oxidase were inhibited by various quenchers. 2. Several oxygen radical quenchers and antiinflammatory drugs did not affect viscosity of hyaluronate and collagen gelation. 3. Reactive ouygen species generated by xanthine and xanthine oxidase affected both viscosity of hyaluronate and collagen gelation with similar pattem. Therefore, in this study quenching effects of antiinflammatory drugs on reactive oxygen species have been examined by observing viscosity changes of hyaluronate. Sodium salicylate, acetylsalicylic acid, indomethacin and hydrocortisone affected viscosity changes of hyalumnate by xanthine and xanthine oxidase. The pattem of inhibition of hyaluronate degradation by these drugs were comparable to the inhibition produced by OH scavengers and singlet oxygen quencher. 4. To danonstrate the generation of OH ethylene was determined from methional incubated with xanthine and xanthine oxidase according to gas chromatography method. Xanthine and xanthine oxidase produced ethylene fmm methional and the production was inhibited by antiinflammatory drugs. The result obtained in this study suggest that action of antiinflammatory drugs may, to some event, be attributed to their ability to intercept reactive oxygen species in addition to inhibition of synthesis of prostaglandin.

6.Mediating Role of Psychological Capital in Relationship between Occupational Stress and Turnover Intention among Nurses at Veterans Administration Hospitals in Korea.

PURPOSE: The aim of this study was to evaluate the mediating role of psychological capital (PCP) in the relationship between occupational stress and turnover intention in nurses. METHODS: Data were collected from a sample of 447 nurses working at four Veterans Administration Hospitals throughout South Korea from July 1 to July 31, 2014. We collected data from the nurses using the following surveys: the Short Form Korean-Occupational Stress Scale, the Korean version of the Turnover Intention Scale, and the Korean version of the Psychological Capital Questionnaire. Multiple linear regression analysis was performed to examine the mediating role of PCP. RESULTS: The level of occupational stress was 1.81 ± 0.23, the level of turnover intention was 3.29 ± 0.86, and the PCP level was 3.95 ± 0.52. There were significant correlations among the three variables (occupational stress, turnover intention, and PCP). PCP played a partial mediating role (β=−0.22, p = .008) in the relationship between occupational stress and turnover intention (p < .001) among nurses working at the Veterans Administration Hospitals. CONCLUSION: Based on the findings of this study, we recommend that South Korean hospitals offer occupational stress management programs that incorporate relevant programs in efforts to strengthen the overall components of PCP among nurses to reduce turnover intentions. Further studies are required to determine the most effective intervention programs for hospital settings.

7.Introduction of Relative Survival Analysis Program: Using Sample of Cancer Registry Data with Stata Software.

Patient survival is one of the most important measures for the evaluation of progress in cancer patient care across the wide spectrum from diagnosis to treatment. The optimal monitoring method for cancer patient survival is to estimate survival based on representative data from cancer patients in the population, which is only achievable through using population-based cancer registration data. Relative survival is used to compare the survival experience in a study cohort that expected to result from background population mortality rates. This technique is useful when the cause of death is not accurate or not available, since it provides a measure of excess mortality in a group of patients with a certain disease. The purpose of this article is to demonstrate the procedures for estimating relative survival using the statistical software Stata. For this survival analysis to show the procedure, the example data set was randomly selected from the National Cancer Incidence Database, which was used in a recent article reporting the overall relative survival of cancer patients diagnosed during 1993-2002 in Korea.

Patients with breast lesion suggestive of malignancy underwent Tc-99m MIBI scan in order to assess the value of this technique in the detection of breast carcinoma and axillary lymph node metastasis. One hundred five patients with breast lesion underwent 99m-MIBI scan before biopsy. 20mCi of Tc-99m MIBI was given intravenously in contralateral arm of the breast lesion and planar prone lateral and supine anterior views for 10 minutes each were taken. Pathological diagnoses were carcinoma in 77 breasts of 76 patients and benign in 29 patients. Pathologic types were DCIS in 4, IDC in 62, mucinous carcinoma in 3, medullary carcinoma in 2 and others in 6. The pathologic tumor status was DCIS in 4, T1 in 27 ,T2 in 30, T3 in 6, T4 in 4 and Tx is 6 cases. All patients with multiple lesions were positive. The sensitivity of Tc-99m-MIBI scan was 95%(73/77) and specificity was 69%(20/29). Sensitivity of mammography and ultrasonography were 85% and 82%. Axillary metastasis study showed 68% sensitivity and 92% specificity. The Tc-99m MIBI scan is a highly effective method in the diagnosis of breast cancer.

9.The Association of Hypertension and Dyslipidemia in Postmenopausal Women.

BACKGROUND AND OBJECTIVES: The purpose of this study was to figure out the correlation of hypertension and atherogenic dyslipidemia in postmenopausal women. MATERIALS AND METHODS: The study population consisted of consecutive 380 postmenopausal women (age> or =50, FSH>40IU/L and triglyceride[TG]<400 mg/dl) visiting health promotion center at Samsung Cheil Hospital from March 1997 to July 1998. We measured blood pressure by automatic sphygmomanometer (A & Dco.LTD.TM-2654) and divided them into four groups by blood pressure (Group I:systolic blood pressure[SBP]< 120 mmHg and diastolic blood pressure[DBP]<80 mmHg, n=87. Group II:SBP 120-139 mmHg or DBP 80-89 mmHg, n=109. Group III: SBP 140-159 mmHg or DBP 90-99 mmHg, n=127. Group IV: SBP> or =160 mmHg or DBP> or =100 mmHg, n=57). We measured thier body mass index (BMI) and waist-hip ratio (WHR). We also measured their levels of glucose, total cholesterol and TG by automatic biochemical analyzer (Hitachi 7150) and HDL cholesterol by direct method using AutoDAIICHI reagent. We calculated LDL cholesterol levels by Friedewald formula. RESULTS: MI, WHR and glucose level of group IV were significantly higher than those of group I. But there were no significant differences among the groups in the levels of total cholesterol, LDL cholesterol and apolipoprotein B. HDL cholesterol level of group I was statistically higher than that of group IV. TG level and LDL cholesterol/apo B ratio of group IV were significantly higher and lower than those of other groups respectively. CONCLUSION: Postmenopausal women who had moderate or severe hypertension show the trend toward abdominal obesity and atherogenic dyslipidemia. We might have to pay attention to these metabolic abnormalities in postmenopausal women with hypertension.

10.A Study of Effect on Pulmonary Function of Pleural Effusion in Tuberculous pleurisy patients.

BACKGROUND: Pleural effusion is a common disease in clinical practice but its effect on pulmonary function and altered pulmonary mechanics after removal of effusion are not still largely understood. Previous studies have shown that there is little or a relatively small improvement in pulmonary function and arterial blood oxygenation after therapeutic thoracentesis. The present study was designed to assess the effect on pulmonary function of pleural effusion and to test whether there was a significant improvement in pulmonary function and arterial oxygenation after thoracentesis and to observe long term effect after thoracentesis. METHOD: We examined flow-volume curve, body box and arterial blood gas analysis according to severity of effusion, present symptom, and symptom duration. Then, we measured changes of pulmonary function after thoracentesis and observed longterm effect after thoracentesis. RESULT: 1) Pleural effusion cause restrictive pulmonary insufficiency. Not only functional impairment of small airway but also large airway is provoked. 2) MMFR, FEV1, Raw, PO2 are earlier improved than FVC and TLC after thoracentesis and patients without complication have mild restrictive pulmonary insufficiency after longterm observation 3) FVC, FEV1, & TLC are similarly restricted as severity of pleural effusion and PO2 is relatively decreased. 4) Cases with symptom duration 1 week or less and cases with dyspnea have more severe pulmonary insufficiency than others. 5) The flow volume curves show a relatively greater improvement in flow rates at large lung volumes than small airway. 6) Significant relationship is shown between first thoracentesis amount and changes of FEV1, FVC, TLC. CONCUSION: Pleural effusion cause restrictive pulmonary insufficiency and not only functional impairment of small airway impairment but also large airway is provoked. Then, Pulmonary function is progressively improved after thoracentesis and remained mild restrictive pulmonary insufficiency after recovery
