3.Dermographism ( IV ): The Prevalence in Atopic Dermatitis and Allergic Rhinitis.

The prevalences of dermographism in 106 patients with atopic dermatitis and 45 patients with allergic rhinitis were studied using a dermographic tester. The results are as follows : 1. Dermographism was more common in atopic dermatitis(17.0%) than in the general population(6.7%), but no significant difference was observed between allergic rhinitis(6.7%) and general population. 2. The prevalence rates of dermographism were not significantly correlated with the level of serum IgE in both atopic dermatitis and allergic rhinitis.

4.A Case of Papillary Adenoma of the Nipple Showing Histopathologic Findings of Syringocystadenoma Papilliferum.

A 24-year-old woman was seen for evaluation of an skin lesion on thiright nipple. Examination revealed a solitary 1.5 x 1cm-sized, erythematous, oozing, eroded andruted, verrucous nodule on the right nipple. A small amount of serous discharge was found but there was no pain or tenderness. Histopathologic findings showed cystic invaginations extending down vard from the epidermis and papillary projections extending into the lumina of the invagination. Pupiae were lined by two rows of cells. In stroma, there were marked inflammatory infiltrates containing any plasma cells. The overlying dermis showed an adenomatous growth pattern with a prolifertion of round, oval or irregular shaped ducts and fibrotic stroma.

5.Food and house dust mite allergens in children with atopic dermatitis.

Although basic mechanisms of atopic dermatitis remain largely speculative, many studies on pathogenesis suggest the importance of food and inhalent allergens. To evaluate the frequency of food and house dust mite hypersensitivity and differences in this frequency according to ages, we measured the levels of specific IgE antibodies to egg white, egg yolk, milk, soy, and house dust mites in 119 children with atopic dermatitis. The results showed that 53% of patients had positive RAST to any one kind of allergens. The frequency of food and house dust mite hypersensitivity were 34.5%, 30.3 %, respectively. Among allergens, house dust mites and egg white are the most prevalent allergens in all atopic dermatitis patients. The Prevalence of egg white is most common under the age of 2 years, but those of house dust mites are the dust mites are the highest in the ages of 5-12 years. In conclusion, we recommend an egg restriction diet in atopic dermatitis patients who are less than 2 years old when their symptoms do not improve with general skin care.

6.A Case of Factitial Panniculitis.

We report a case of factitial panniculitis in a 50-year-old female patient who had multiple variable sized subcutaneous nodules on area accessible to the hands and disappeared by only prohibition of habitual skin massage. Histologic findings showed mostly lobular panniculitis with hemosiderin deposits, many foamy histiocytes and pseudomicrocysts.

7.The Toxic Effects of Diclofenac Sodium on Cultured Rabbit Corneal Epithelium.

Diclofenac sodium is a widely used drug for reducing inflammation and pain after cataract, surgery or excirner laser photorefractive keratectomy. However,we could find epithelial defect while using this drug postoperatively. To investigate the toxic effects of diclofenac sodium, we cultured corneal epithelial cells and tissues, then exposed them to various concentratiotration of drugs and measured the cell number and expanding area from. the original corneal tissue. In cases of cell culture, the cell number was counted at 24, 48, and 72 hours` exposure to l2.5, 25, 50, and 100 ug/ml solution of diclofenac sodium and at. 5 days after two minute exposure to 0. I and 0.2% solution. The cell number was reduced significantly at 50ug/ml (72 hrs later) , 100ug/ml(48 or 72 hrs later) and 0.1, 0.2% of solution than that of the control group. In tissue culture, we exposed the epithelial tissue to the same condition of cell culture. In long duration exposure conditions, the size of the epithelium grown from the tissues was smaller than that in the control group, especially at the dosage of 50 and 100ug/ml. In short duratio(2 minutes) exposure conditions, no significant difference from the control group was found. In conclusion, diclofenac sodium has some suppressive effects on the growth and migration of the rabbit corneal epithelium and its postoperative high dose or frequent instillation might injure the corneal epithelial cells.

8.A case of hyperimmunoglobuline E syndrome.

The hyperimmunoglobulin E syndrome is a primary immunodificiency disorder characterized by recurrent staphylococcal infections and markedly elevated serum IgE level. Clinical features are coarse face and severe infections of the skin-furunculosis or chronically pruritic dermatitis and sinopulmonary tract infection from infancy by coagulase positive Staphylococcus aureus, or Candida albicans etc. The patients's serum IgE level is elevated but the basic immunologic pathogenesis not fully understood. We have experienced a case of hyperimmunoglobulin E syndrome in a 26/12-year-old who had suffered from recurrent staphylococcal pneumonias and abscesses and chronically pruritic dermatitis from 1 month of age with elevated serum IgE level. A brief review of the related literature is presented.

9.Periodontal Management strategies for the future in Korea.

In periodontics, much progress was made in the understanding of periodontal disease from 1960s to 1980s and in prevention and management of periodontal disease since the end of 1980s. This presentation will discuss about the prevalence of periodontal disease, treatment need, and provision of periodontal treatment in Korea, and how we could manage the periodontal disease efficiently in the future. According to an epidemiological study in Korea, periodontal disease(including gingivitis) was present in 82% of general population and periodontitis in 30-40% in adult population over 30y and juvenile periodontitis in 0.1% of adolescents. If we consider that at least 17% of these patients may have recurrent or refractory forms, there is obviously an abundance of disease that needs treatment. As a result of increase in life expectancy, senile population over 65 y will be increased from 6% in 1996 to 6.9% in 2000, and tooth retention rate and periodontal treatment need are expected to increase. Periodontists need all the help they can get from the general dentists to control periodontal disease. As for provision, postgraduate course in periodontics started in 1957 in Korea and produced over 700 specialized dentists in periodontics. One report indicated that the periodontists as well as general practitioners did periodontal therapy on only a few periodontal patients, because of specific control by current medical insurance system in Korea. Comprehensive periodontal examination is rarely done in local dental clinic. Therefore, enhancement of periodontal care in medical insurance system and education of simplified periodontal examination such as Periodontal Screening & Recording will make dentists diagnose and manage the management of adult patients is based on the recognition that there are multiple diseases, including gingivitis, chronic adlt periodontitis, and other more aggressive forms of periodontitis, and requires the earliest possible recognition of these three disease categories. In this presentation, we discuss practical approach using PSR to diagnose, manage and refer the patients, to facilitate the separation of the simple from the complex and the predictable from the unpredictable form of periodontal diseases and to integrate diagnostic and therapeutic techniques into private practice today.

10.Observations of Arrhythmias by 24 Hour Ambulatory ECG Monitoring in Early Convalescent Phase of Acute Myocardial Infarction.

In order to observe the arrhythmia which occured in early convalescent phase of acute myocardial infarction, a 24 hour ambulartory ECG monitiring was performed in 15 patients who is admitted at dept of Int Medicine, Ewha Womans University hospital from Aug. 1985 to Sept. 1986. during 7.5+/-3.4 days after admission. In all cases one or more of the following arrhythmias were observed;atrial premature beat, ventricular premature beat(VPB), transient paroxysmal ventricular thchycardia, atrial fibrillation, sinus arrest and junctional beat. Of 15 cases, 11(77.3%) had VPB's:Simple VPB's in 4 cases(26.7%) and complex VPB's(6.52+/-2.52)(P<0.05). But there were no difference of premature and abberancy index between complex VPB's and simple VPB's(P>0.1). In addition, we compaired the arrhythmias in CCU with those of early convalescent phase of acute myocardial infarction. But there was no correlation between them.
