1.Balloon Catheter Dilatation in Esophageal Achalasia: Long Term Follow-Up.

PURPOSE: To evaluate the clinical efficacy of balloon catheter dilatation in the treatment of esophageal achalasia. MATERIALS AND METHODS: Seven patients(three males and four females) with esopha-geal achalasia were treated with balloon catheter dilatation. Balloon catheters of variable sizes were used depending on patient's conditions. The patients were followed up over a period of 12-39months. RESULTS: Balloon catheter dilatation in esophageal achalasia was successful in all patients without esophageal perforation. All patients were relieved from dysphagia. Recurrence was not found in 5 patients on long term follow-up study, but was seen in 2 patients after 18 and 21 months, respectively. CONCLUSION: Balloon catheter dilatation was a safe and effective method in the treatment of esophageal achalasia with low recurrence rate of 29% on follow-up study.

2.Perineal Endometriosis.

Endometriosis is an enigmatic disease of menstruating females affecting the reproductive life. We report herein a case of perineal endometriosis in a 33-year-old woman. The patient presented two rounded, thickened plaques bilaterally on the perineum for about four year. The plaque on the right side was located mainly at the episiotomy scar. Histologic examination showed confirmative feature of glandular structures embedded in the cellular stroma and predecidual changes of late secretory phase of endometrium. Radical operation of total hysterectomy with bilateral oophorectomy and partial excision of perineal lesions were performed. There was complete resolution with no relapse by the 6-month follow-up. This is of a very rare occurrence and three cases have been reported in the Korean literatures up to date.

3.The Use of Atypical Antipsychotics in Bipolar Disorder.

Atypical antipsychotics are the more effective and safer alternative to the common practice of maintenance adjunctive treatment as well as acute adjuvant treatment with traditional antipsychotics in patients with bipolar disorder. A few double-blind controlled studies of acute mania found olanzapine or ziprasidone monotherapy to be more effective than placebo, and the combination treatment of risperidone and mood stabilizer was also more effective than placebo. Furthermore, clozapine has antimanic and mood stabilizing effect for the treatment-refractory patients in acute manic and maintenance phase. Olanzapine and risperidone are reported that they have long-term mood stabilizing effect when they are used with mood stabilizers. At present, combination treatment of atypical antipsychotics and mood stabilizer is generally used. However, because each drug of such combinations causes sometimes bothersome and potentially dangerous events as well as their interactions, the consideration for their risk are needed.

5.Surgical Repair of Hypospadias.

A clinical evaluation was made with particular attention to the technique of surgical repair on 61 patients with hypospadias who had been admitted to the Department of Urology, Kyungpook National University Hospital during 10 years periods from January, 1976 to June, 1986. The results were as follows: 1. The patients were between 1 to 29 years. Only 15 patients (24.6 %) were found in the age group under 6 years and 10(16.3%) in the age group over 20. 2. The most common type of hypospadias was penoscrotal in 33 patients (54.l%) and associated anomalies were found in 16 (26%) in which the most common one was cryptochism in 9. 3. 61 patients underwent hypospadias repair in which one stage repairs were used 13 and two stage repairs on 48. In two stage repairs, chordectomies were performed on 35 patients and second stage urethroplasties on 35. 35 patients underwent chordectomy in which Byars technique were used on 32 and Nesbit technique on 3. In the 32 patients who underwent chordectomy using Byars technique, 29 (9l%) were successful, while 3 others had skin necrosis or incomplete chordee repair. 35 patients underwent second stage urethroplasty in which Byars technique were used on 30 and other techniques on 5. In the 30 patients who underwent second stage urethroplasty using Byars technique, 20 (67%) were successful, which 8 having fistulas and 2 urethral stricture. 13 patients underwent one stage repair in which Duckett techniques were used on 4, MAGPI techniques on 3 Devine-Horton techniques on 2 and other techniques on 4. In the 13 patients performed one stage repair, 9 were successful, while 4 had fistulas or skin necrosis. 4. Of 83 surgical procedures performed with chordectomy, second stage urethroplasty or one stage repair, complication was encountered in l7 (20.6%). Of which the most frequent one was fistula. Of the 10 observed fistulas, 5 responsed nicely to one additional repair, while 3 required more additional repair and 2 had been lost in follow-up.

6.Hyperinfection of strongyloides stercoralis.

Strongylodiasis is universal in distribution but is most abundant in countries with a tropical climate. Although infestation by Strongyloides stercoralis is usually limited to the intestines, dessemination of this helminth in debilitated host can be lead to death with various clinical disorders. characterized by profound malabsorption, diarrhea, electrolyte imbalance, gram negative or opportunistic fungal sepsis, coma and death. Cell-mediated immunity contributing significantly to the control of helminthic infections, may be suppressed by carcinoma, immunosuppressive chemotherapy and use of corticosteroids. Diagnosis of Strongyloidiasis is achieved by an examination of samples of feces, duodenal aspirates and sputum of patients for Strongyloides stercoralis. Treatment of strongyloidiasis is twofold : correction of the immunosuppressive state by withdrawal of immunosuppressive drug, if possible, and vigorous treatment with thiabendazole. Testing for strongyloidiasis is especially recommanded before treating a patients should be monitored for infection by Strongyloides stercoralis and other opportunistic infection. We are reporting a case patient with Strongyloides stercoralis hyperinfection and pulmonary tuberculosis who had been. used corticosteroid for persisting polyarthritis.

8.Repair Integrity and Functional Outcomes after Arthroscopic Repair of Transtendinous Full-thickness Rotator Cuff Tears Minimum Two-year Follow-up.

BACKGROUND: To evaluate the clinical outcomes and associated repair integrity in patients treated with arthroscopic repair for a transtendinous rotator cuff tear followed by resection of the remnant rotator cuff tendon. METHODS: Between July 2007 and July 2011, we retrospectively reviewed patients who were treated for transtendinous full-thickness tears in the tendinous portion of the rotator cuff by arthroscopic repair. Clinical outcomes were evaluated using the American Shoulder and Elbow Surgeons (ASES) score, the Shoulder Rating Scale of the University of California at Los Angeles (UCLA), the Constant-Murley score, a visual analogue scale (VAS) pain score, and range of motion (ROM). The repair integrity was determined by magnetic resonance imaging or ultrasonography. RESULTS: There were 19 shoulders with transtendinous full-thickness tears in the tendinous portion of the rotator cuff. The ASES, UCLA, Constant-Murley, and VAS pain scores showed significant improvements in function and symptoms (all p<0.001). The active ROM for forward flexion and abduction was also significantly improved (p=0.002 and p<0.001, respectively). The postoperative radiological examination showed cuff integrity without a re-tear in 68.4% of patients. However, the UCLA, ASES, and Constant-Murley scores were not significantly different between healed and re-torn group (p=0.530, p=0.885, and p=0.262, respectively). CONCLUSIONS: Although repair of transtendinous rotator cuff tears followed by resection of the remnant rotator cuff tendon in the footprint has a relatively high re-tear rate, no significant difference was observed in the short-term clinical results between the re-tear and healed groups.
