2.Clinical Characteristics of Nontraumatic Acute Renal Infarction.

Purpose: Early diagnosis and treatment are essential features to save the kidney in patients with an acute renal infarction. To reduce the delay in diagnosis, the clinical features and treatments of an acute renal infarction were evaluated. Materials and Methods: The medical records of 19 patients (14 men and 5 women) diagnosed with an acute renal infarction were retrospectively analyzed. Symptoms, findings of a cardiologic evaluation, laboratory findings, presumptive diagnosis and treatments were evaluated. Results: All patients complained of flank or abdominal pain. Fifty-three percent of the patients had a history of cardiovascular disease and 37% had no history of specific illnesses. The level of serum LDH was markedly elevated in all patients that performed the test. Initially, only 4 patients were diagnosed as acute renal infarction. In the others, the presumptive diagnoses were urinary stone, acute abdomen, aortic dissection and so forth. Seven patients were managed with conservative care due to the delay in diagnosis or poor general condition. Nine patients were managed with systemic anticoagulation and 3 with intra-arterial thrombolytic therapy. Conclusions: An acute renal infarction must be considered as one of the presumptive diagnoses in patients presenting with flank or abdominal pain and a history of cardiovascular diseases. Prompt CT scanning and testing of the serum LDH level will help in the accurate and immediate diagnosis of an acute renal infarction.

3.Clinical Evaluation of Sterile Intermittent Catheterization in Spinal Cord Injury Patients on Indwelling Catheter Drainage.

Sterile intermittent catheterization was used in spinal cord injury patients who had indwelling catheters. This study included 17 men who had indwelling catheters for from 6 months to 5 years, an average of 22 months. Incidence of achieving catheter-free state was 12 cases among 17 cases (70.6%) and the mean period of intermittent catheterization of successful trials was 31 days. Among successful 12 patients follow-up studies up to 1 year, 9 patients (53.0%) were complete catheter-free state and 3 patients (17.6%) needed alternative intermittent catheterization. For the period of 1 year follow-up studies, there were not been any urologic complications except 2 cases of acute pyelonephritis and some trivial urinary tract infections. As these results, sterile intermittent catheterization was the useful method for management of neurologic bladder due to spinal cord injury and eliminating indwelling catheter.

4.Clinical Evaluation of the Urinary Tract Infection in Spinal Cord Injury Patient.

81 spinal cord injury Patients divided to 4 groups; The 1st. group was 12 patients with vesicostomy, the 2nd. group was 27 Patients with urethral indwelling catheterization, the 3fd. group was 15 Patients with intermittent catheterization and the 4th. group was 27 Patients with self voiding state. In 81 spinal cord injury Patients of 4 groups, urine culture, colony count and drug susceptible test were done from the department of Urology, Veterans Hospital during the period from Mar. 1983 up to Jun. 1983. 1. The incidence of bacteriuria was totally 54 cases (66.7%) among 81 cases, 10 cases (83.3%) among 27 cases in 2rid. group, 8 cases (53.3%) among 15 cases in 3rd. group and 12 cases (44.4%) among 27 cases in 4th. group. 2. The frequency of isolate organisms by culture were as followings; Pseudomonas: 34.8%, Proteus: 17.4%, Serratia: 14.5%, Providence: 13.0%, Acinetobacter: 5.8%, E. coli. 4.3% Alkaligenes: 2.9%. 3. Mixed infection of 2 different organisms was 12 cases (30.8%)among 39cases in 1st. and 2nd. groups and 2 cases (4.8%) among 42 cases in 3rd. and 4th. groups. 4. On the whole, all isolated organisms showed considerable susceptibility to Amikacin (73.9%) and then frequency of susceptible drugs were Tobramycin and Velocef. The incidence of resistance for all drugs was 13 cases (18.8%) among 69 cases.

6.Overactive Bladder and Urinary Incontinence in Adult Women: Prevalence and Effects on Daily Life and Sexual Activity.

The purpose of this study was to investigate the prevalence, relative factor and the effects that overactive bladder or urinary incontinence have on women's lives. Data was gathered through an internet survey. The subjects included 3,372 women ranging from 20 years old to 49 years of age. The questionnaire included general characteristics (age, parity, family history, and history of UTI), overactive bladder symptoms (urgency, frequency, nocturia, urgent incontinence), urinary incontinence, discomfort in ADL and sexual activity. The data was analyzed by the SPSS/PC+ program using frequency, X(2) test, and multi- variate logistic regression. The results were as follows; 1. The prevalence was 12.7 % in overactive bladder and 21.0% in urinary incontinence. 2. 19.2% of the subject have urge incontinence, 5% of them have coped with the use of a pad. 3. The significant factors to overactive bladder were age(X(2)= 6.6, p<0.05), history of urinary tract infection (X(2)= 50.8, p<0.01) and family history (X(2)= 26.1, p<0.01). The significant factors to urinary incontinence were age (X(2)= 6.2, p<0.05), occupation (X(2)=11.0, p<0.05), history of urinary tract infection(X(2)= 20.2, p<0.01), parity (X(2)= 8.6, p<0.01), and family history (X(2)= 4.9, p<0.05). 4. Overactive bladder impacts on individual daily life was 5.0 times, urinary incontinence was 2.9 times higher than in non-symptoms. Also, overactive bladder impacts on their sexual activity disturbance was 4.3 times, urinary incontinence has 3.9 times higher than in non-symptoms. In conclusion, overactive bladder symptoms and urinary incontinence were health problems that disturbed women's lives. Also the most problem among overactive bladder may be urgency in Korean. Intervention based on these results needs to be provided for these women.

7.CT findings of cervical lymphadenopathy: morphological analysis.

To evaluate the effectiveness of computed tomography for the differential diagnosis of cervical lymphadenopathy, we reviewed CT scans of 26 patients with cervical lymphadenopathy retrospectively. These included 10 patients with tuberculous lymphadenitis, 11 patients with metastasis and 5 patients with lymphoma, We evaluated the CT scans with a special attention to internal nodal density, feature of contrast enhancement and location of lymphadenopathy. Tuberculous lymphadenitis involved multiple nodes unilaterally and showed central low density with even or uneven rim enhancement, usually occurring in young patients (mean: 31.6 years). Two cases with tuberculous lymphadenitis showed calcifications within the lymph nodes. Lymphoma involved unilateral or bilateral nodes and appeared as conglomerated isodense mass with even rim enhancement. Metastasis involved multiple nodes unilaterally and showed focal, diffuse of mixed pattern of central low density with variable rim enhancement, usually occurring in old patients (mean: 59.4 years). Locations of most frequent lymph node involvements were internal jugular group (76%), spinal accessory group (54%) and retropharyngeal group(12%).

Leiomyorrw is the mest cunmon in the uterine hunor. Smooth muscle tumors of the female genital taact arise mostly from the uterine myometrium and only rarely from the broad ligament. We experienced a cases of huge leiomyoma of the broad ligarnent with secondary cystic degeneration. In the respect of rarity, we report this case with a brief review.

9.Norm-referenced standard of health-related physical fitness of ROKAF pilots.

BACKGROUND: The present study is undertaken to acquire normative data of health-related physical fitness(HRPF) of Republic of Korea Air Force(ROKAF) aircrews and to provide norm-referenced standard of HRPF. METHODS: From 15 May 1998 through 14 December 1999, 1284 KAF pilots are checked muscular strength, muscular endurance, flexibility, and body fat percentage. RESULTS: The grip strength was relatively maintained, but flexibility, back strength, and muscular endurance were declined according to promotion and aging. And the incidence of obesity determined by bioelectrical impedence analysis was higher than that assessed on the basis of height and weight. CONCLUSION: It is suggested that many pilots who were normal in BMI have abdominal or visceral obesity. Further studies must be made on the health promotion program for the pilots, on the changes in physical fitness, and on the criterion-referenced standard of HRPF of the pilots.

10.HBsAg and anti-HBs prevalence in North Korean defectors.

BACKGROUND: Hepatitis B is a worldwidely distributed disease. The hepatitis B prevalence of South East nations is reported to be higher than that of America or Europe. Korea is known to be one of high prevalent nation among Asia-Pacific nations. But hepatitis B prevalence in North Korea, separate from South Korea due to politics and ideology, is not reported. This study was performed using the data of North Korean defectors to estimate the prevalence of hepatitis B in North Korea. METHODS: We analyzed the data of 107 persons (98, mals, 9 females) who defected from North Korea and underwent screening exammination in a general hospital from July 1987 to March 1996. Twenty nine persons(25 males, 4 femals) were excluded because of incomplete data. The HBsAg test was carried out by reversed passive hemagglutination(RPHA) and the anti-HBs test was carried out by passive hemagglutination(PHA). The liver function tests were carried out by biochemical quantitative analysis method of Dimension. RESULTS: The study group were 73 males(93.6) and 5 females(6.4%). Mean age was 33.0+/-8.4 years and third decade was the largest as 35 persons(44.9%). The overall HBsAg positive rate was 15.4% and anti-HBs positive rate was 83.9%. Those who need vaccination was 51.4%. The only item amorg liver function tests which was statistically different by the hepatitis B viral marker groups was alkaline phosphatase. Mean total cholesterol was 176.4+/-38.0mg /dL. But no other item of liver function tests was statistically different in abnormal value by the hepatitis B viral marker groups. CONCLUSIONS: The overall HBsAg positive rate of North Korean defectors was much higher than that of South Korean and anti-HBs positive rate was much lower. So HBsAg positive rate of North Korean seems to be higher than that of South Korean. We think that a large study of hepatitis B for North Korean should be performed at every opportunity and take a measure to meet the situations.
