2.Evaluation of Meniscal Tears of Knee by Post

Tentative diagnosis of meniscal pathology is very helpful for the successful operative management including arthroscopy. Conventional arthrography has long been used to meet this purpose, but it has some limitations due to complex views and some difficulties in imaging the whole anatomical structures. Recent diagnostic advancement produced post-arthrographic HRCT and MRI for the compensation of these problems of conventional arthrography. Arthrography and post-arthrographic HRCT has been carried out on 49 knees in 43 patients. 38 knees among them could be confirmed by arthroscopy and following results were obtained. 1. Diagnostic accuracies of post-arthrographic HRCT for medial and lateral meniscal injuries of knee were 94.7% and 92.1% repectively. 2. Shape, contour of torn meniscus and the relationships between the torn fragments were better demonstrated with HRCT. 3. Horizontal tear could also be suspected by HRCT. 4. Peripheral detachment could be differentiated with synovial recess on the HRCT, especially in the reformat view, but was more clearly visible in arthrography. 5. Arthrography and HRCT can be used as a good complementary study for the evaluation of meniscal injuries.

4.The SEM Observation of The Various Root Treatment Effect On Furcation Area.

In periodontal regeneration treatment, access to the frucation area is very difficult. Thus complete removal of plaque, calculus and endotoxin is somewhat impossible. In this study, teeth that were extracted due to periodontal disease were used. The furcation area was treated with periodontal curette, ultrasonic scaler, roto bur and they observed using SEM. The result was follows 1. The group treatment with curette showed remaining plaque, the cementum existed in most of the surface and partial dentinal tubule orifice could be seen. 2. The group treatment with ultrasonic scaler showed less removalof plaque compared to curette and irregular surface could be seen. 3. The group treatment with roto bur showed cleaner surface and many dentinal tubule orifice could be seen compared to the curette and ultrasonic scaler groups. Thus when suing treatments such as bone grafting or guided tissue regeneration, it is considered that the furcation area should be treatment with Roto bur.

6.Death by Subdural Hematoma with Metastatic Carcinoma of Unknown Origin: An Autopsy Case Report.

Dural metastasis associated with chronic subdural hematoma is very rare in patients with malignant neoplasms; it may be difficult to distinguish malignant neoplasms from chronic subdural hematoma. Chronic subdural hematoma is usually a late manifestation of malignant tumors, which contributes to the severity of the prognosis. The scans obtained by using magnetic resonance imaging or computed tomography may be misleading when a subdural hematoma masks the underlying tumor. Herein, we report a case of a subdural hematoma with dural metastasis of unknown origin in a 45-year-old woman; however, the neoplasm was not detected until autopsy.

7.Two-Phalanged Fifth Toe in Korean.

Except for the hallux, the human toes classically present three phalanges, distal, middle and proximal. However in 5th toe, only two phalanges are frequently observed. In this condition, known as synarthrosis of the distal interphalangeal joint, the middle and distal phalanges are fused together to appear as symphalangism or biphalangeal 5th toe. Phalanges of 5th toe was investigated in 1,187 cases of Korean radiographs. The incidence of symphalangism was found to be 74% in 1,150 adult. The bilaterality was 99%. To prove proove the genetic basis of the symphalangism, pedigree studies were performed. The symphalangism of the 5th toe was supposed to be an autosomal dominant trait. As an phenotype of recessive homozygote the triphalaneal subjects were traced to investigate their families. Pedigrees of four families in which both parents had triphalangea of 5th toe showed that their offsprings always showed the triphalangea. Therefore, it suggests the symphalangism inherit as a Mendelian dominant trait and it seems to be an example of microevolution or genetic adaptation to bipedalism.

8.Hypotensive Efficacy and Safety of Manidipine on the Patient with Essential Hypertension.

Thirty patient with essential hypertension were administered Manidipine, a new calcium antagonist, 10~20mg once daily to evaluate the hypotensive efficacy and safety for 8 weeks. And the followings were the result. 1) Patients were consists of 14 male, 16 female, aged 53 in average and classified as mild in 21 and moderate in 9 patients. 2) Optimum intial dose was 10mg and 10 to 20mg were the doses recommended. 3) Blood pressure dropped after 8 weeks 24/13mmHg in average, rewarding 80% effectiveness and normalized in 87%. 4) Most frequent side reaction was facial flushing in 5 patiens followed by palpitation and dizziness, all of which did not disturb the continuation of medication. 5) Most of routine laboratory parameter were normal and unchanged between before and after the trial. 6) Overall rating of usefulness was 77%. In conclusion, Manidipine 10 to 20mg once daily regimen is well tolerated and effective in the treatment of mild to moderate essential hypertension.

9.The Effects of Shift Rotation Directions on the Circadian Rhythms of Urinary Free Cortisol, 17-OHCS in Shift Workers.

To determine effects of the direction of the rotational work shifting on the circadian rhythm and adaptation of physiological and psychological components were investigated with nursing students. Two groups of seven volunteers participated as experimental subjects, and eight nursing students participated as a control group. The directions of rotation work shift were as follows : CW(clockwise)-shift system rotation in the direction of day shift(3 lays), evening shift(3days), off duty(1day) and night shift(5days). and CCW(counterclockwise)-shift system rotation in the reverse direction. Urinary free cortisol and 17-hydroxycorticoster oids in the urine were measured by the solid-phase radioimmunoassay and the colorimetry methods, re spectively. Plasma melatonin was measured by radioimmunoassay. The urinary free cotisol rhythm changed to ir-regular on the evening and night shift in the CCW shift system, whereas the rhythm seems to adapt on night shift in the CW shift system. The daily rhythms of urinary 17-OHCS indicate that they are well synchronized to shift work. These results show that the rotation of shift in the CW direction is more acceptable in terms of the adaptation of hormonal rhythms. These results indicate that the CCW rotation of shift work makes it more difficult for the workers to adapt on a physiological level, and has many more negative effects on nurses when compared with the CW rotation. Thus, a clockwise rotating shift systern seems to be more desirable for the optimal health and performance of nurses.

10.Unresolved Suffering Lived Experiences of College Students.

PURPOSE: This study was done to discover the structure of the lived experiences of unresolved suffering of college students and to gather information to develop therapeutic educational interventions for nursing students. METHODS: The research question, 'What is the structure of the lived experience of unresolved suffering?' was examined based on Parse's Human becoming research method. Twelve nursing students were recruited from K University. From May, 1 to June 30, 2009, Data were gathered from writings and engagement in dialog. RESULTS: The structure found in the college students' lived experiences of suffering was as follows: negative self-concept from being discriminated and ignored by parents, feelings against people and difficulties in interpersonal relations, feelings of isolation, betrayal, guilt, and loss. Their suffering was emotional grief and social withdrawal from damaged human dignity and low self-esteem from psychological trauma. Conceptual integration found to be in process of transferring the enabling-limiting, connecting-separating values. CONCLUSION: It is necessary to develop therapeutic educational interventions for college students for further development as individuals and future health professional by developing awareness of the structure and the meaning of their suffering experience.
