2.Survey of Oncology Nurses' Awareness of Patient Safety Culture and Nursing Care Activities for Patient Safety.

PURPOSE: The purposes of this study were: 1) To describe the level of awareness of patient safety culture and nursing care activities for patient safety and, 2) To identify factors affecting nursing care activities for patient safety among oncology nurses working at inpatient care units. METHODS: The study sample included 221 nurses working in oncology wards. After obtaining IRB approval, data collection was done with structured self-administered questionnaires, from September 13 to October 13, 2013. Data analysis was done using SPSS WIN 18.0. RESULTS: The mean score of awareness of patient safety culture was 3.46 and the mean score of nursing care activities for patient safety was 4.31 both on a scale of 5.0. Significant factors affecting nursing care activities for patient safety were the open attitude of the supervisor toward patient safety (beta=.19, p=.005) and the degree of patient safety within a ward perceived by the oncology ward nurses (beta=.23, p=.003). These factors explained 31% of the subjects' nursing care activities for patient safety (F=6.08, p<.001). CONCLUSION: To increase nursing care activities for patient safety in the oncology ward, a strategy such as an education program for increasing the awareness of patient safety is needed.

4.Effect of 17beta-Estradiol and 1,25-Dihydroxyvitamin D3 on Interleukin-6 Production of Periodontal Ligament Cells.

Interleukin-6(IL-6) stimulate osteoclast differentiation. 17beta-estradiol, 1,25-dihydroxyvitamin D3(1,25-(OH)2D3) and interleukin-1beta inhibit or stimulate osteoclast differentiation by decreasing or increasing the synthesis of interleukin-6(IL-6) from stromal/osteoblastic cells, respectively. Periodontal ligament(PDL) cells reside between the alveolar bone and the cementum and have osteoblastic characteristics. To estimate the effect of 17beta-estradiol and 1,25(OH)2D3 on IL-6 production of PDL cells, PDL cells were treated with 17beta-estradiol or 1,25-(OH)2D3 in the absence or the presence of IL-1beta. The concentration of IL-6 produced form PDL cells was determined by enzym linked immunosorbent assay(ELISA). In unstimulated PDL cells, we detected constitutive production of IL-6 at 1st and 2nd day. IL-1beta increased IL-6 synthesis at 1st day and 2nd day. 17beta-estradiol had no significant effect on the secretion of this cytokine, either constitutively or after stimulation with IL-1beta(0.05 ng/ml). 1,25-(OH)2D3(10(-8)M) decreased not only constitutive IL-6 production but also IL-1beta-induced IL-6 production at 2nd day. These results suggest that 1,25-(OH)2D3 may control IL-1beta-induced osteoclast differentiation by decreasing IL-1beta-induced IL-6 secretion of PDL cells.

Invasive pulmonary aspergillosis (IPA) has been known to occur in immunocompromised patients, but has been rarely reported in immunocompetent patients. In immunocompetent patients, pulmonary fungal infections are not initially considered. This results in diagnosis and treatment delays, as well as poor prognosis. We report a case and serial CT findings of IPA in an immunocompetent 29-year-old male after congenital heart disease surgery.
6.Dantrolene and Post-operative Hyperthermia: A Case Report.

Malignant hyperthermia is a rare, fatal pharmacogenetic disorder that occurs during general anesthesia following exposure to a depolarizing muscle relaxant, such as succinylcholine, or volatile anesthetics. Clinical findings in malignant hyperthermia include muscle rigidity, sinus tachycardia, increased CO2 production, skin cyanosis with mottling, and marked hyperthermia. For treatment, cooling techniques must be accompanied by discontinuation of the provocative medication. Furthermore, dantrolene administration is the mainstay of treatment for malignant hyperthermia, and should be initiated as soon as the diagnosis is suspected. We recently experienced a case with post-operative fever of 41.0degrees C refractory to conventional anti-pyretic measures and finally resolved with dantrolene administration, in a patient with methicillin-sensitive Staphylococcus aureus monoarthritis of the knee and rapid progression of diffuse septic pneumonia requiring mechanical ventilation.

7.Dynamic MR Imaging of Internal Derangements of the Temporomandibular Joint: Correlation with Clinical Signsand Symptoms.

PURPOSE: To evaluate the correlation between findings of dynamic magnetic resonance (MR) imaging of thetemporomandibular joint (TMJ) and clinical findings in patients with internal derangement of the TMJ. MATERIALS AND METHODS: Dynamic MR images of 130 joints of 65 patients (17 men and 48 women aged 14 to 74 ; mean age, 34)with internal derangement of the TMJ were obtained using a 1.5-T MR imaging system. MR findings of anteriordisplacement, disc deformity and degenerative change were correlated with clinical findings including joint pain,clicking sound, crepitation and maximal mouth opening (MMO). RESULTS: Among 62 joints with TMJ pain, 32 showedanterior displacement without reduction, 15 showed disc deformity and 19 showed degenerative change. Among 49joints in which there was a clicking sound, 24, 8 and 12 joints, respectively, showed the above-mentionedfindings, while in seven with crepitation, these same findings were evident in five, six and four joints,respectively. Bilateral and unilateral joint disorders were observed in 17 and five patients, respectively, among22 with MMO less than 30mm and in 11 and 18 of 42 patients with MMO between 30mm and 60mm. CONCLUSION: On MRimaging, patients with MMO less than 30mm usually showed bilateral joint disorders and those with crepitationshowed chronic change. This indicates that there is high association between clinical findings of internalderangement of TMJ and MR findings.

8.The Study on the Clinical Features of Gouty Arthritis.

OBJECTIVES: The aim of this study is to enhance understanding the clinical features, pathogenesis, diagnosis and treatment of gouty arthritis in Korea by analyzing the clinical manifestations of the patients with urate crystal-proven gouty arthritis. METHODS: 78 cases who had been diagnosed as gouty arthritis by confirming the urate crystals in synovial fluids or tophi in Seoul National University Hospital between January 1, 1989 and July 31, 1995 were analysed for their histories, symptoms, signs, laboratory data, and X-ray findings. RESULTS: 1) Male to female ratio was 18.5:1. The mean age of onset is 49.3 +/- 14.5 years(range 11-83 years); the mean duration of disease 6.5 +/- 7.0 years(range 0-30 years); the mean duration of gouty attack 7.2 +/- 5.5 days(range 1-30 days). 2) The frequent precipitating factors of gouty arthritis were hospitalization(37%) and alcohol drinking(15%). The most frequent accompanying disease was hypertension(24%). Obesity, diabetes, chronic renal failure, hyperlipidemia, ischemic heart diseases, or cerebrovascular diseases were also accompanied by gouty arthritis. 3) The patterns of joint involvement were devided into 3 groups: monoarthritis; 42%, oligoarthritis; 35%, polyarthritis; 23%, The most frequent site of the first gouty attack was the 1st toe(65%). The most frequently involved joint at gouty attack was also the 1st toe(68%). While only lower extremities were involved in most cases with monoarthritis and oligoarthritis(91% and 78%, respectively), both lower and upper extremities were involved in most cases with polyarthritis(78%). 4) Hyperuricemia was found in 74% of the cases at gouty attack. But serum uric acid level was normal in 26%. With respect to pathogenesis of hyperuricemia, 14% of the cases had uric acid overproduction and 86% had uric acid underexcretion. 5) Bony changes in radiologic findings were found in 47% of the cases and tophi in 33%. Bony changes and tophi was significantly related to the younger age of gouty onset and higher serum uric acid level at gouty attack. 6) Acute gouty arthritis responded well to colchicine and NSAIDs. There was no difference in efficacy and the frequency of side effects between them. CONCLUSION: The clinical features of the gouty arthritis in Korea showed no difference from those in foreign studies except higher prevalence of oligo-/polyarthritis and tophi. To be remarkable, 26% of the patients with gouty arthritis did not have hyperuricemia at gouty attack. This finding indicates that urate crystals should be confirmed by synovial fluid examination for diagnosis of gouty arthritis.

9.Evaluation of the Real-Q HCV Quantification Kit.

BACKGROUND: Hepatitis C virus (HCV) RNA quantification is necessary for predicting the therapeutic response and assessing treatment results in patients with chronic HCV infection. Recently, real-time PCR technology for HCV RNA quantification displayed good linearity within the dynamic range. Thus, it is gradually replacing branched-DNA (bDNA) and PCR- hybridization assays. In this study, we evaluated the performance of the Real-QTM HCV quantification kit (biosewoom. Inc., Seoul, Korea) developed in Korea. METHODS: We evaluated the HCV quantification kit for detection limit, specificity, linearity, accuracy, and recovery rate of HCV RNA standard material. The results were analyzed for a correlation with those of Cobas Amplicor HCV Monitor 2.0. RESULTS: The HCV quantification kit showed a high recovery rate of HCV RNA standard material of various concentrations and amplication of HCV RNA equally in all genotypes. Hepatitis B virus and human immunodeficiency virus showed no cross-reactivity with HCV. Within-run and between-run coefficients of variation (CV) were 9.52~15.84% and 9.40~17.53%, respectively. Between-day coefficients of variation were 11.62~18.04%, and detection limit was 44 IU/mL. It showed a good correlation with Cobas Amplicor HCV Monitor 2.0 (R2=0.8954). CONCLUSION: The Real-Q HCV quantification kit showed a good specificity, sensitivity, linearity, and accuracy; therefore, we propose that it is fully adequate for monitoring antiviral therapy in patients with chronic HCV infection.

10.Arachnoid Cyst in Cavernous Sinus: Case Report.

Arachnoid cyst of the cavernous sinus is very rare. When present, its anatomic location frequently gives rise to cranial nerve palsy. We report a case of arachnoid cyst of the cavernous sinus in a 38-year-old man with impaired eyeball movement and diplopia.
