1.Drug Abuse Status and Its Determinants of Male High School Students in Taegu.

To identify the drug abuse status and its determinant factors in high school boys in Taegu, the study was performed from April to May, 1995. Study population were selected by cluster sampling method and total 5,665 students replied to the self-administered questionnaire survey(2,207 in academic high school, 3,458 in business high school). The major findings were as follows; The proportion of drinking, smoking experience was 55.0%, 45.8%, respectively, and the proportion of current drinker, current smoker was 27.2%, 27.5%. The drinking, smoking experience rate of second grade students was higher than first grade and it was higher in business high school boys. The proportion of a stimulant, a hallucinogen, hemp leaf cigarets experience was 3.2%, 1.6%, 0.1%, respectively. Drug abuse had significant association with home environment(lower economic status, frequent move, death of father or mother, apart from family), parents environment(parents' indifference, parents' drinking and smoking, etc.), school life(lower school grades, intimate friend's drug abuse, etc.), generous attitude to drug abuse, higher level of stress. Students who replied that the law prohibited immature person(students) from drinking and smoking showed lower drug abuse rate. In multiple logistic regression analysis, second grade students, business high school students, parents' indifference, lower school grades, intimate friend's drug abuse, no recognition of the fact that the law prohibits high school students from drinking and smoking, generous attitude to drug abuse, higher level of stress were significantly related with alcohol abuse and smoking. Other drugs abuse were related with above factors. On consideration of above findings, to prevent students from drug abuse, we have to try together in house, school, and society.

2.The Process of Acceptance of Complementary and Alternative Therapies (CATs) among Nurses: Grounded Theory Approach.

PURPOSE: This study was done to explore the process of accepting CATs among nurses who experienced CATs in Korea. METHODS: Grounded theory methodology was utilized. Data were collected from 10 nurses during individual in-depth interviews. Theoretical sampling was used until the data reached saturation. Data were analyzed using the constant comparative analysis method. RESULTS: The core category emerged as "resolving the doubt and integrating" explaining the process of accepting CATs. The nurses engaged in three stages: need awareness, look for solution and integration. Causal conditions were interest as a nursing intervention and orthodox medical limitations. Context was lack of basis for application and increase in social interest. Strategies were new knowledge acquisition, having a strong will, combined with existing knowledge, and individualized intervention. Intervening conditions were others' eye, exhaustion for nurses and physical environment. Consequences were expanding of the nursing role and improved nurse satisfaction. CONCLUSION: The results of the study should facilitate application of CATs in nursing practice. To help nurses who are interested in CATs, there is a need for education programs, and further research on CATs.

3.Crossmapping of Nursing Problem and Action Statements in Telephone Nursing Consultation Documentations with International Classification for Nursing Practice.

OBJECTIVES: This study is to cross-map telephone nursing consultation documentations with International Classification for Nursing Practice (ICNP; ver. 1.0 concepts). METHODS: The narrative telephone nursing consultation documentations of 170 ophthalmology nursing unit patients were analyzed. The nursing statements were examined and cross-mapped with the Korean version of the ICNP ver. 1.0. If all the concepts of a statement were mapped to ICNP concepts, it was classified as 'completely mapped'. If any concept of a statement wasnot mapped, it was classified as 'partially mapped'. If none of the concepts were mapped, it was classified as 'not mapped'. RESULTS: A total of 738 statements wereused for documenting telephone nursing consultations. These statements were divided into 3 groups according to their content: 1) 294 nursing phenomena-related statements (72 unique statements), 2) 440 nursing actions-related statements (76 unique statements), and 3) 4 other statements (2 unique statements). In total, 189 unique nursing concepts extracted from 150 unique statements and 108 concepts (62.44%) were mapped onto ICNP concepts. CONCLUSIONS: This study demonstrated the feasibility of computerizing narrative nursing documentations for electronic telephone triagein the ophthalmology nursing unit.

4.Sensory Testing for Binocular Suppression in Accommodative Esotropes.

It is important to detect binocular suppression of pediatric strabismic patients, especially in accommodative esotropes and intermittent exotropes. However, it may take a long time and false results may be obtained in young children. In order to examine binocular suppression, we selected 103 accommodative esotropes with binocular suppression. Vectograph test, Worth 4 dot test, and 4 prism base-out test were performed at distance, and Worth 4 dot test, TNO test, and Bagolini test at near. Titmus fly test and Randot stereotest were administerd to know the relationship between stereoacuity and suppression. In detecting binocular suppression, the true rate of vectogram was 95%, which was the highest result of all tests and 4 prism base-out test was the next with 88% of the true rate. The stereoacuity was increased in proportional to the absence of suppression. The vectographic porject test was the easiest and the most accurate test to identify normal fusion or binocular suppression at distance in young strabismic children. The accuracy will be increased in the combination with 4 prism base-out test. Therefore the vectograph also can be used in the screening of small amount of deviation, monocular amblyopia and low vision in kindergarten and elementary school.

5.Development of Telephone Consultation Algorithm for Patient Discharged with Ophthalmic Disease.

PURPOSE: This methodological study was done to develop a telephone consultation algorithms and practice guidelines for patient discharged with ophthalmic diseases. METHODS: The ophthalmic problems of the patients were identified and expert knowledge on managing the problems was acquired. Algorithms and practice guidelines were developed based on the expert knowledge. The content validity of algorithms and practice guidelines was evaluated by the experts. RESULTS: The preliminary algorithms and practice guidelines were developed from 60 detailed signs and symptoms and 45 nursing interventions. The experts agreed that 57 detailed signs and symptoms linked with nursing interventions were valid, with the content validity index over 80%. Meeting with nurse experts and ophthalmologists was convened to review the rest of the 3 detailed signs and symptoms linked with nursing interventions. Finally, 60 detailed signs and symptoms and 46 nursing interventions were confirmed. CONCLUSION: This study suggests that the algorithms and practice guidelines are effective decision-making tools and utilization of these algorithms and practice guidelines is expected to improve the quality of clinical nursing and patient satisfaction.

6.Factors Influencing Infection Control Awareness and Implementation Levels among Dental Hygienists.

A total of 228 dental hygienists working in dental hospitals and clinics in the Busan and Gyeongnam areas were surveyed between August 1, 2015, and October 15, 2015. The factors influencing infection control awareness and implementation levels among the dental hygienists were investigated to prepare basic data with the goal of establishing guidelines for systemic infection control. Treatment preparation support for infection control positively correlated with equipment and facility support (r=0.4343, p<0.01), treatment skill and information support (r=0.231, p<0.01), infection control education support (r=0.266, p<0.01), infection control awareness (r=0.354, p<0.01), and infection control implementation levels (r=0.442, p<0.01). Equipment and facility support positively correlated with treatment skill and information support (r=0.418, p<0.01), infection control education support (r=0.422, p<0.01), infection control awareness (r=0.404, p<0.01), and infection control implementation levels (r=0.454, p<0.01). Infection control education support positively correlated with infection control awareness (r=0.348, p<0.01) and infection control implementation levels (r=0.405, p<0.01). Infection control awareness positively correlated with the infection control implementation level (r=0.879, p<0.01). The factors influencing the awareness of infection control include treatment preparation support, equipment and facility support, treatment skill and information support, and infection control education support. The influencing the infection control implementation level include treatment preparation support, equipment and facility support, infection control education support, and treatment skill and information support. To enhance the awareness of infection control and implementation levels amongdental hygienists, an infection control system must be established and implemented A rigorous system for evaluating dental clinics must also be established to ensure an ideal dental treatment environment and to protect patients' health and safety.

7.Management of Diabetes Mellitus and Factors Associated with Poor Glycemic Control in an Urban Area.

BACKGROUND: We evaluated the current status of diabetes management and the predictors for poor glycemic control in an urban area. METHODS: This study included 1,138 community-dwelling adults (> or =50 years) with diabetes, of which 584 participated in the diabetes care survey. Logistic regression was used to identify the factors predicting poor glycemic control (hemoglobin A1c[HbA1c]> or =7%) in the total sample and to evaluate the relationship between the history of diabetes management checkup and poor glycemic control in the diabetes care survey sample. RESULTS: Of the 1,138 patients, 53.2% had blood pressure less than 130/80 mmHg, 41.7% had fasting glucose between 70 and 130 mg/dL, 48.6% had HbA1c below 7.0%, 60.1% had triglycerides below 150 mg/dL, 41.4% had low density lipoprotein cholesterol below 100 mg/dL, and 59.1% had normoalbuminuria (urine albumin-to-creatinine ratio <30 mg/g creatinine). Of the 584 patients completing the diabetes care survey, 63.9% had one or more lipid tests, 32.0% had one or more HbA1c tests, 43.8% had one or more microalbuminuria tests, and 42.5% had one or more fundoscopic examinations annually. Female gender (odds ratio [OR], 1.71; 95% confidence interval [CI], 1.23-2.36), duration of diabetes (OR, 1.07; 95% CI, 1.06-1.09), and alcohol use (OR, 1.40; 95% CI, 1.06-1.85) were associated with an increased risk for poor glycemic control while age (OR, 0.97; 95% CI, 0.96-0.99) and antihypertensive medication (OR, 0.64; 95% CI, 0.50-0.83) were associated with a decreased risk. CONCLUSIONS: This study shows that glycemic control is likely to be poor in urban areas. We need to develop appropriate community-based strategies to achieve optimal glycemic control and prevent diabetes complications.

8.Significance of APACHE Score in Patients with a Gastrointestinal Perforation.

BACKGROUND: The APACHE scoring system of the Health Care Financing Administration (HCFA) has been being used for serious patients. The scoring system is composed of acute physiologic variables and chronic disease. METHODS: Among patients who underwent emergency operations from 1992 to 1997 because of gastrointestinal perforation, we analyzed 110 cases with five kinds of diseases: duodenal ulcer perforation, small bowel perforation, perforated appendicitis, gastric ulcer perforation and colon perforation. RESULTS: The results were as follows: 1) The preoperative APACHE II scores ranged from 0 to 21. The scores of 64 cases (60.9%) were from 0 to 5. 2) There were no death in case for which pre-peration APACHE II score was from 0 to 10, 25% of the mortality occurred in cases with scores from 11 to 15, 50% in those with scores from 16 to 20, and 100% in those with scores above 21. 3) The APACHE II score decreased continuously from the 3rd to the 7th postoperative day. 4) The preoperative APACHE II scores in gastric ulcer perforation patients were significantly higher than those in duodenal ulcer perforation patients. In the cases of gastric and duodenal ulcer perforations, the APACHE II scores in patients who underwent primary closure were higher than the scores in those who underwent a more definitive operation. 5) In death cases, all of their APACHE II scores were higher at the 3rd postoperative than at the 7th postoperative day, but their APACHE III scores continuously increased postoperatively. CONCLUSIONS: It is thought that the APACHE scoring system is more reliable than clinical experience in the classification of patients by operative risk and in estinating the result and giving a prognosis. Thus, the principle of treatment should be established by estinating patient's score before the operation. Careful preoperative management is necessary for patients with scores more than 10. Because patientswith scores more than 21 have very a high mortality, operative time and method must be carefully decided. The APACHE III scoring system seems to be more sensitive than the APACHE II scoring system in predicting deaths and further investigations and clinical applications should be performed.

10.Detection of Enteroviruses and Mammalian Reoviruses by RT-PCR and Integrated Cell Culture-PCR in CPE-positive Surface Water Samples.

The environmental water samples assayed by total culturable virus assay (TCVA) were evaluated by reverse transcription-polymerase chain reaction (RT-PCR) and integrated cell culture-PCR (ICC-PCR) method for enteroviruses and reovirus since they are the usually detected virus groups by culture assays. The detection sensitivities of the TCVA, RT-PCR, and ICC-PCR were compared and the overall reliability of the detection was analyzed to confirm environmental samples for enteric viruses. A total of eight samples from different areas was analyzed by performing TCVA, RT-PCR, and ICC-PCR. Virus concentrations in surface water samples ranged from 1.03 to 47.3 most probable numbers of infectious units (MPN) per 100 liters. When primers specific for both enteroviruses and reoviruses were used in both RT-PCR and ICC-PCR, all the samples (100%) were positive for the viruses. Reoviruses were the most frequently detected ones among the samples. According to the sequence results of enteroviruses, five of the samples were contaminated by coxsackievirus type B3, and the rest by coxsackievirus type B6, echovirus type 30, or vaccine strain poliovirus type 3. It was observed that both enteroviruses and reoviruses were detected concurrently in all CPE-positive environmental water samples.
