Benign idiopathic neonatal convulsion is a rare disorder which has no family history of convulsion and develops before and after the 5th day in a healthy full-term neonate. Its characteristics appear focal, or multifocal clonic seizures but rare tonic seizures lasting about several minutes. It reveals non-specific findings in neurologic examination, neuroimaging and EEG(electroencephalography) so that it should be differentiated from those diseases such as eletronic imbalance, inborn errors of metabolism, other neonatal epileptic syndromes. We report two healthy full-term female neonates presented with multifocal clonic seizures before and after the 5th day after birth. They had no family history of convulsion, fetal asphyxia, fetal and maternal problems and the neurologic examination and neuroimagings were normal. The convulsions were controlled by intravenous phenobarbital injection. They had no more convulsions ever since and showed normal development at the follow-up performed one year later. We experienced a rare disorder, benign neonatal convulsion in healthy full-term neonates. We hope this report will help its diagnosis and treatment and prevent unnecessary long- term anticonvulsant medication.

Carbon Monoxide poisoning is one of the most serious Public health problems in Korea. The incidence rate officially reported has been known to be the highest in the world. This high incidence is mainly due to the wide prevalence of anthracite coal briquette as the domestic fuel for unique Korean heating system called "ondol." The coal briquette gas contains around 3-5% of Carbon Monoxide. A nation-wide effort to eliminate or reduce this serious hazards has produced little effect and the most hospitals are offering very ineffective measures such as oxygen inhalation through nasal catheter. Author has believed that this preventable accident should be approached by the secondary preventive measure because of our socio-economic status do not allow us optimistic results from primary preventive measure as far as the problem of CO poisoning is concerned. Author has treated 466 patients during 30 months period by Hyperbaric Oxygenation at Seoul National University Hospital. The results found are as follows. 1. Female has a higher incidence rate than male and the age group between 15-29 years showed highest incidence. 2. The recovery time depends on the time when the patients arrived at hospital. Earlier the arrival time, shorter the recovery time. 3. Some objective signs are representing typical physiological response to tissue hypoxia. 4. Therapeutic effectiveness of Hyperbaric Oxypenation is confirmed by such indices as recovery rate, Admission rate and average stay in hospital. Futher, these results are cocordant with other reports on the clinical value of Hyperbaric Oxygenation in the treatment of CO poisoning.

The osteochondroma is a common benign tumor with osteocartilaginous exostosis. It may arise from any bone, but is observed most often on long tubular bones, particularly on their metaphyseal region. Osteochondroma may also arise from the rib but seldom grow to giant size. We experienced a rare case of giant osteochondroma arising from the left 5th rib. The patient, 41 years old man, had an adult fist sized mass on the left lower chest with mild aching pain on respiration. The large mass was examined by biopsy. It showed characteristic microscopic findings of osteochondroma. No evidence of malignant change was seen in this large tumor.

Debonding of cement-femoral stem interface has been suggested as a initial focus of loosening mechanism in many previous studies of cemented total hip replacement. The purpose of this study was to investigate the effect of debonding of cement-femoral stem interface to the cement-bone inter- face by using three-dimensional non-linear finite element analysis. Three cases of partial debonded, full debonded, and full bonded cement-bone interface were modelled with partial bonding of distal 70mm from the tip of femoral stem. Each situation was studied under loading simulating one-leg stanced gait of 68kg patient. The results showed that under partial and full debonded cement-stem interface conditions the peak von Mises stress (3.1 MPa) were observed at the cement of cement-bone interface just under the calcar of proximal medial of femur, and sudden high peak stresses (3.5 MPa) were developed at the distal tip of femoral stem at the lateral bone-cement interface in all 3 cases of bonding. The stresses were transfered very little to the cement of upper lateral bone-cement interface in partial and full debonded cases. Once partial or full debonded cement-femoral stem interface occured, 3 times higher stress concentration were developed on the cement of proximal medial cement-bone interface than full bonded interface, and these could cause loosening of cemented total hip replacement. Clinically, preservation of more rigid cement-femoral stem interface may be important factor to prevent loosening of femoral stem.

Fifty-eight primary total hip arthroplasties with hyhrid cemented femoral component using contemporary cementing technique and cementless acctahular cup were followed for a minimum 3 year(ranged, 36 to 67 months, mean 4.1 years). Thc average age of the patients at the time of the operation was 54 years(ranged, 22 to 73 years). The preoperative common diagnosis were avascular necrosis of femoral head in 38 hips and miscellaneou. in 20 hips. The mean Harris hip score was 61 in preoperative and 91 in postoperative at last Follow-up. 5-1 hips(92%) were rated excellent and good clinically. One hip was revised for aseptic loosening of the femoral stem at 52 months follow-up. Radiographically no acetahular component was loose. Three femoral stems were definitely loose, one stem was classfied as prohahle loosenine and another one was ratde as possible loosening. There was no progressive radiolucent dermaation ol' cement-hone interf'ace in stable fermoral steni. Non progressive cement-bone radiolucency was lound seven hips in zone 1, two hips in zone 6, one hip in zone 2 and one hip in zone 7 at last follow-up. There were two postoperalive dislocations. Other complications included superficial infection(two hips) and sciatic nervc palsy(one hip). Our experience showed that the hyhrid hip replaccment with improved cementing technique provided satisfactory results at minimum 3 year follow-up study and emphasized the importantance of careful cementing techniques in survival ot cemented I'cmoral stem.
