1.The Effect of Sciatic Nerve Transection on the Somatostatin and Substance P Cells in Dorsal Root Ganglia of Rats.

The authors investigated the morphometric analysis of substance P(SP)- and somatostatin(SOM)- containing nerve cells in dorsal root ganglia. For this purpose, immunohistochemical method was used to determine the number, size and the morphological characteristics of SP- and SOM-reactive cells in L5 dorsal root ganglia of rats. In addition, changes in type A, type B, SP- and SOM-containing nerve cells in ganglia after sciatic nerve transection were also determined. The results were as follows : 1) SP- and SPOM-reactive nerve cells belong to the population of type B cell, but N/C ratios of immunoreactive cells were higher than others ; 2) in normal group, SP- and SOM-reactive nerve cells were 12.5 and 3.2% of total nerve cells in ganglia, respectively ; 3) the case of coexistence of SP and SOM in one cell was not found ; 4) and there was a marked reduction in the number of SP- and SOM-reactive cells at 2 weeks after nerve injuries. And SP-reactive nerve cells were increased in number at 6 weeks after operation, but SOM-reactive cells were not. According to these results, SP- and SOM-reactive nerve cells belong to type B cells, but do not coexist in one cell. These nerve cells were decreased in number after nerve transection. SP-reactive nerve cells were recovered at 6 weeks after operation but recovery of SOM-reactive cell was not found.

2.Treatment of Unstable Intertrochanteric Fracture of the Femur: Cpmparision between Sliding Compression Hip Screw and Jewett nail Plate

Commonly used internal devices to fix the stable and unstable intertrochanteric fractures are fixed nail plate (Jewett nail, Holt nail), sliding nail plate (compression hip screw) and intramedullary device (Ender, Harris nail). The choice of internal device is influenced by the general coditions of patients, the pattern of fracture and the personal preference of surgeon. There are many reports that sliding nail plate appears to give better result than fixed nail plate because the latter device leads to high failure rate. Twenty nine unstable intertrochanteric fractures treated with Jewett nail plate (group I) and sliding compression hip screw (group II) between 1981 and 1988 were reviewed after minimal twelve months follow up. Group I comprised of fourteen cases, Group II, of fifteen. There were seventeen males and twelve females. The ages ranged from thirty-two to seventy-eight years, the average age being 57.8 years. Eighteen cases were caused by slip down; eight, traffic accident; and three, fall down. The average operation time was 124 minutes in group I and 148 minutes in group II. The average amout of blood loss was 1024cc in group I and 1040cc in group II. The average time to union was 13 weeks in group I and 12.9 weeks in group II. In Jewett nail plate fixed group, there were three complication; one coxa vara, and superior migration of nail, and one subcapital fracture and one superficial wound infection. In sliding compression hip screw group, one was complicated with coxa vara. Above results suggest that the use of Jewett nail plate in unstable intertrochanteric fracture could be got good results with method using compression hip screw.

4.The Relationship of Anger Expression and Alexithymia with Coronary Artery Stenosis in Patients with Coronary Artery Diseases.

This study examined the relationship between anger expression or alexithymia and coronary artery stenosis in patients with coronary artery diseases. 143 patients with coronary artery diseases (104 males and 39 females) were enrolled in this study. The severity of their coronary artery stenosis was measured by angiography. The Anger Expression Scale and the Toronto Alexithymia Scale were used to assess the level of anger expression and alexithymia. The more stenotic group (occluded by 75% or more) exhibited a significantly higher level of alexithymia than the less stenotic group (occluded by less than 25%). Multiple regression anaylsis on the extent of stenosis also revealed that regardless of gender and age, the coronary artery disease patients with higher alexithymia were likely to show a greater level of stenosis. However, no significant differences were found on either the anger-in or anger-out subscale scores between the two groups. These results suggest that alexithymia is associated with the severity of coronary artery stenosis in patients with coronary artery disease. However, both anger expression and anger suppression were not shown to be associated with the severity of coronary artery stenosis.

5.Clinical Experience of Cleft Lip and/or Palate Repair in Complex Congenital Heart Disease.

In cleft lip and/or palate patients with the complex congenital heart diseases, surgical repair of the cleft lip and/or palate has been postponed after the open heart surgery because the heart problem of the patient might cause more complications associated with anesthesia and surgery. There has been little report about experiences in the surgical management of these patients and optimal time of surgical intervention. Authors are introducing the experiences of performing corrective surgery of cleft lip and/or palate in the patients with congenital heart diseases before and after the open heart surgery. We managed five patients from May 1992 to March 2004. Two patients were male and the rest were female. One of them had cleft lip alone and others had cleft lip and palate. Two of them underwent delayed cleft lip and/or palate surgery after open heart surgery, and the rest had immediate intervention for cleft lip and/or palate. There was no complication during the operation and postoperative period. There would be no need to delay the corrective surgery of the cleft lip and/or palate after the open heart surgery, if solid medical team approach was available with the pediatric cardiologist and the anesthesiologist.

6.A case of small intestinal obstruction due to phytobezoar, revealed after operation in elderly.

The major causes of small bowel obstruction are postoperative adhesion, hernia, intussusception and malignancies. However, in cases of uncommon causes of small bowel obstruction, surgeons are in a dilemma because the preoperative diagnosis and making a decision to operate are frequently difficult and delayed. Phytobezoars are unusual cause of small bowel obstruction. They are mostly due to ingestion of stringent immature fruits following gastric surgery. They are usually formed in stomach and do not migrate to the other intestinal tract, but can lead to the serious complication of acute small bowel obstruction. We are reporting a case of acute small bowel obstruction due to phytobezoar about 2x2x3 cm3 size in a 90 year old female.

7.Cerebral Sparganosis with Apoplectic Presentation.

Cerebral sparganosis is a rare parasitic infestation of the brain, usually presenting with seizure associated with chronic recurrent inflammatory reaction. Recently we have treated a very unusual form of this parasitic disease, of which clinical onset was apoplectic. A 60-year-old man was transferred to our emergency room complaining of sudden onset of right hemiparesis and speech impairment. Computed tomography(CT) of the brain demonstrated high density lesion, suggestive of a hematoma in the left parietal subcortical region. Unusual location and shape of the hematoma made us took CT angiogram to confirm any vascular abnormalities in or around the hematoma. We performed an explorative craniotomy and evacuation of the hematoma under the guidance of stereotactic system. At surgery, a live parasite, a larva of a tapew (Spirometra mansoni) was found and removed with the hematoma.

8.Cerebral Vasospasm after Surgical Clipping Versus GDC Embolization in Ruptured Intracranial Aneurysm.

OBJECTIVE: The purpose of this study is to analyze the influence of the treatment modality(surgical cilpping versus GDC embolization) on development of cerebral vasospasm in a non-randomized retrospective analysis of 93 patients of aneurysmal subarachnoid hemorrhage. METHODS: The following parameters were retrospectively reviewed in our institution's data base and analyzed in association with vasospasm-related ischemic infarctions: 1) Hunt and Hess(H&H) grade, 2) Fisher group, 3) location of aneurysm, 4) treatment modality(surgical cilpping versus endovascular treatment). RESULTS: Of the 93 patients, 39 (41.9%) patients suffered delayed ischemic infarctions. The incidence of delayed ischemic infarctions were increased as higher H&H grade and Fisher group but was not related with the location of aneurysms and the treatment modalities. CONCLUSION: Even with vigorous removal of the subarachnoid blood clots in the surgical clipping group, there is no statistical differences in the incidence of cerebral vasospasm between the two treatment modalities.

9.Primary Ewing's Sarcoma Arising from the Mandible: A Case Report.

Ewing's sarcoma is an uncommon malignancy that usually occurs in children. It is composed of monotonous population of small cells with large nuclei and little cytoplasm. These cells fall into the category of the "small, round, blue cell tumors of children". Based on cytogenetic studies, Ewing's sarcoma appears to be of neuroectodermal origin and shares the same reciprocal 11:22 translocation as primitive neuroectodermal tumor. Long bones and pelvis are the preferential sites. The incidence of Ewing's sarcoma in all primary malignant bone tumors is four-seven percent. Only seven percent of Ewing's sarcoma occurs in the mandible either as primary or metastatic disease. The rarity is due to the paucity of hematopoietic marrow of the mandible. Soft tissue swelling and pain are the most common presenting symptoms. Differential diagnosis of Ewing's sarcoma of the mandible includes other mass lesions of the jaw such as osteosarcoma, Burkitt's lymphoma, retinoblastoma and neuroblastoma. Treatment for mandibular Ewing's sarcoma consists of various single and combination modalities including radiation, chemotherapy, and surgery. We report a case of a 24-year-old male with mandibular Ewing's sarcoma who was treated with chemotherapy (vincristine, adriamycin, cyclophosphamide and ifosfamide, etoposide) and concurrent radiation therapy (6MV, 180-200 cGy/d, 32 fractination, total dose: 5900 cGy).

10.Flexor Carpi Radialis Tendon Rupture due to Repetitive Golf Swing

Flexor carpi radialis (FCR) muscle is located in the forearm anteriorly that runs through a synovial fibro-osseous tunnel in the forearm. We described a case of FCR tendon rupture due to repetitive overuse injury. A 55-year-old man, right-hand dominant, presented with right forearm pain and swelling which started 3 days ago while playing amateur golf. Focal tenderness and bruising over volo-ulnar region of the right forearm were examined. Plain radiographs showed soft tissue edema around lesion area and no detectable fracture. Ultrasonography showed multiple hypoechoic lesions suspected as hematoma of the flexor muscle group. After done magnetic resonance imaging, he was diagnosed with rupture of FCR tendon at proximal origin and strain of flexor digitorum superficialis and palmaris longus muscle. He received compressive dressing and restriction of wrist range of motion for three weeks. Two months later, remaining traces of lesions were observed at the follow-up ultrasonography and the pain disappeared.
