1.Catamenial Hemoptysis: Report of one case.

Pulmonary endomertiosis is a rare disorder with the typical symptom of hemoptysis during menstruation (catamenial hemoptysis). We report a case of a 19-year-old woman, gravida 0, with 3-month history of catamenial hemoptysis which was confirmed with chest computed tomography. She was treated by means of thoracoscopic wedge resection for the right lesion and fuperior segmental resection through the left thoracotomy, successively. Preoperative fluoroscopy-guided hooking for thoracosopic target lwsion was helpful in circumstances with one lung anesthesia. Four months of follow-up after an uneventful discharge revealed out no recurrence of catamenial hemoptysis in symptoms and images.

2.Techniques of Clinical Digital Photography in Plastic Surgery.

Nowadays, clinical digital photography is very prevalent, though the pitfalls, problems, and standardizing works of its techniques have not yet been discussed well enough in Korea. Digital single lens reflex cameras and macro capacity lenses are still recommended for plastic surgeons for its reproducibility and high quality of developed photographs. Since the selection of digital camera itself is as important as the choice of the film in film cameras, the selected digital equipments should not be exchanged in a center or a clinic. Eighteen percents gray card(Eastman Kodak) could be used for the strict setting of the white balance as the white balance is essential in digital imaging. Studio flash lighting is good enough in clinical digital photographing settings and, soft, even lighting effect should be achieved using flash accessories (e. g. bouncer) minimizing loss of details. Taken images could be optimized through the software working (image processing) after the monitor color balancing work. They must be always developed or printed under the uniform setup status of the whole equipments to get accurate, constant pictures.

3.The Mechanisms of Resistance to TNF in TNF-Sensitive Cancer Cells Transfected with TNF-alpha Gene Using Retroviral Vector.

BACKGROUND: Tumor necrosis factor(TNF) has been considered as an important candidate for cancer gene therapy based on it9 potent anti-tumor activity. However, since the efficiency of current techniques of gene transfer is not satisfactory, the majorities of current protocols is aiming the in vitro gene transfer to cancer cells and re-introducing genetically modified cancer cells to host In previous study, it was shown that TNF-sensitive cancer cells transfected with TNF-α CDNA would become highly resistant to TNF. Understanding the mechanisms of TNF-resistance in TNF-α gene transfected cancer cells would be an important step for improving the efficacy of cancer gene therapy as we]1 as for better understandings of tumor biology. This study was designed to evaluate the role of new protective protein synthesis in the acquired resistance to TNF of TNF-α gene transfected cancer cells. METHOD: We transfected TNF-α c-DNA to WEHI l64, a murine fibrosarcoma cell line, using retroviral vector (pLT12SN(TNF)) and confirm the expression of TNF with PCRf ELISA, MTT assay. Then we determined the TNF resistance of TNF gene transfected cells(WEHI 164-TNF) and the changes of TNF sensitivities after treatments with actinomycin D(transcription inhibitor) and cycloheximide(translation inhibitor). RESULTS: WEHI 164 which was sensitive to TNF became resistant to TNF after being trsnsfected with TNF-α gene and the resistance to TNF was partially reversed after treatment with actinomycin D, but not with cycloheximide. CONCLUSION: The acquired resistance to TNF after TNF-α gene transfection may be associated with synthesis of some protective proteins.

5.Correlation between Arterial Vascularity by Radiologic Study and Structural Histopathologic Type in Resected Hepatocellular Carcinoma(HCC).

PURPOSE: To analize the correlation between dynamic vascular pattern and structural histologic type of HCC. MATERIALS AND METHODS: The materials consisted of 35 cases of HCC, which could be classified by structural histologic type and had available preoperative arterial dominant images. Excluding one case of pseudoglandular type, we compared structural histologic type of the tumors(trabecular group;13 cases, mixed group;10 cases, and nontrabecular group ;11 cases) with dynamic vascular pattern such as degrees of tumor staining, severity of tumor vessels on hepatic angiography, and with enhancing degrees of early phase relative to late phase on two-phase incremental bolus dynamic CT. RESULTS: In hepatic anglogram, trabecular group showed stronger tumor staining and more prominant dilatation and irregularity of tumor vessels than those of nontrabecular group, and In two-phase incremental bolus dynamic CT, and trabecular group showed more prominant tumor enhancement in early phase than those in late phase. CONCLUSION: The trabecular type(typical) and non-ti'abecular type of HCC showed radiologically different vascular patterns and therefore we believe that this maybe the basis with which diagnosis and treatment policy of radiologically atypical HCC could be made.

6.Clinical Feature of Esophageal Papilloma.

In human, Human Papilloma Virus(HPV) is associated with benign squamous tumors in a variety of body sites. But the relationship between HPV infection and malignant epithelial lesions is not clear. Esophageal squamous papilloma is relatively rare tumor but it is reported with increasing frequency recently. We reviewed twenty six patients of esophageal papilloma diagnosed by endoscopic biopsy from 1990 to 1994. The results are as follows; The peak age is fifth decade, men and women ratio is 1: 1.2. Most papillomas located in distal esophagus and have no specific symptoms related to papilloma. All case are sessile form and no malignant change in follow up endoscopy.

7.Loffler's Syndrome Associated with Clonorchis Sinensis Infestation.

In 1932, Loffler described a syndrome of self-limiting, transient pulmonary infiltrates associated with peripheral blood eosinophilia and mild pulmonary symptoms. A number of conditions are related to pulmonary eosinophilia or pulmonary infiltration with eosinophilia. Especially, parasitic infestations are often related to pulmonary eosinophilia, but only two cases associated with Clonorchis sinensis have been anecdotally reported in English literature. Here we report a case of migrating pulmonary eosniophilic infiltrations associated with Clonorchis sinensis that was successfully treated with praziquantel. Clonorchiasis should be considered in patients with marked eosinophilia and pulmonary infiltrations.

Until the recent development of the Q-switched lasers, it was not possible to remove tattoos without permanent scarring and pigmentary changes. Among the Q-switched lasers which was introduced under the concept of selective photothermolysis, Q-switched Nd-YAG laser has a longer wave(1064nm), deeper penetration, and lesser pigmentary change of skin than other type Q-switched laser. So, proved effectivity in removing pigmented lesion and tattoos without scarring and pigmentary changes. But repeated treatment is essential for a good result and than there is no agreement on the treatment interval in using Q-switched Nd-YAG laser. In this experiment, the author tattooed on the back skin of rabbits and treated them twice using Q-switched Nd-YAG laser in 5 groups of different treatment interval(1,2,4,6,8 weeks). The authors analysed the histology and final therapeutic results of the 5 groups to find a reasonable and shorter treatment interval by which we can obtain an effective therapeutic result without causing permanent tissue injury and change of the skin texture. The result shows that the 4 week treatment interval of Q-switched Nd-YAG laser for the tattoo removal in rabbits is the shortest term by which we can get effective therapeutic results without permanent scar and pigmentary changes.

10.Chronic Subdural Hematoma Secondary to Traumatic Subdural Hygroma.

PURPOSE: Sometimes chronic subdural hematoma can be developed following posttraumatic subdural hygroma. The purpose of this study is to investigate its incidence, the duration required for their conversion, and characteristic CT and MR findings of subdural hygroma and chronic subdural hamatoma. MATERIALS AND METHODS: We studied 8 patients with persistent posttraumatic subdural hygroma which consquently developed chronic subdural hamatoma. The patients were examined with CT initially and foilowed-up with CT in 3 and MR in 5. We analyzed the location of the lesion, the change of the density or signal intensity, the change of the size, and the degree of enhancement and mass effect. RESULTS: The duration required for the formation of hematoma was 48-166 days (mean, 76 days). The characteristic CT findings of subdural hygroma were a crescentric lesion with CSF density along the inner table without contrast enhancement. The mass effect was minimal. The CT findings of chronic subdural hematoma were higher density than that of hygroma in all cases, increase in thickness and size in 3 cases, and contrast enhancement along the inner membrane of the hematoma in 5 cases. The signal intensities of the subdural hygroma were identical to those of CSF on both T1 and T2 weighted images, whereas, those of chronic subdural hematoma were higher. CONCLUSION: The increased signal intensity on T1 weighted MR images and increased attenuation or contrast enhancement of the lesion on CT may suggest the conversion of subdural hygroma into chronic subdural hematoma.
