Reactive mesothelial cells are frequently found in the smear, of body fludid dffusion when associated with liver cirrhosis, pulmonary tuberculosis, congestive heart failure or pulmonary infarction. But it is not easy to differentiate these reactive mesothelial cells from macrophages or various types of malignant cells. In order to investigate the morphology of hyperplastic mesothelial cells, in comparison with the various types of malignant cells in the effusion, this experiment was designed. The tissue reaction was evoked by intraperitoneal injection of 2.5 ml of 2% Talcum suspension. The results obtained were as follows: 1) On light microscopic observation of the tissure, until 5 days, cytoplasm and nucleus of mesothelial cells became larger and nucleoi became prominent. After then, cytoplasm and nucleus became smaller and at 28 days after injection of Talcum suspension, cellular features became similar to those of normal ones. 2) On cytologic observation of touch print specimens, hyperplastic mesothelial cells became round to oval, and their cytoplasm became conspicuous. Nuclei were vesicular, and nucleoi were prominent. N/C ratio was decreased than normal. 3) On electron microscopic observation, hyperplastic mesothelial cells did not have microvilli, but had a few cytoplasmic globular projections, loosely approximated cellular junctions, larger and more abundant intracytoplaomic vesicles when compared with normal one. Chromatin clumping, irregular thickening of nuclear membrane and prominent nucleoi were also noted.

As the productive activities have vastly increased following industrialization and urbanization in the modem society, the resulting high mobility of people and goods have caused a sharp increase in the accidents in the work places as well as traffic accidents. In particular, deaths caused by injuries are generally concentrated in the economically active young peoples, producing incalculable losses to the society and nation as a whole. Advanced nations with superior medical care systems have succeeded in reducing incidents of such deaths by operating trauma centers. Especially noteworthy is the case of the United States where such specialized trauma centers have greatly contributed to reducing deaths from injuries in the non-urban areas with less access to medical facilities. At present Korea has no medical centers specializing in injuries. In large cities, the injuries are being treated tertiary medical facilities while in the provinces they are referred to small and medium-sized hospitals that constitute secondary medical facilities. Currently in Korea the Trauma patients are treated at general hospitals that consist of 726 secondary medical facilities and 40 tertiary medical centers nationwide. The secondary medical facilities which tend to take most responsibility for the treatment of trauma are generally deficient in medical staff and facilities (including operating and intensive care facilities). Despite such deficiency and limitations, no regulations exist regarding treatment or transportation of trauma patients. This article reports the outcome of a comparative analysis of the results of trauma treatments among different types of medical facilities based on objective data in the hope that such study would facilitate a comparison with the treatment systems of advanced countries and thereby contribute to a precise formulation of problems that must be addressed in this area.

OBJECTIVES: The purpose of this study was to reveal psychological traits, risk factors, and predictors of the military maladjusted group. In addition, we attempted to compare the correlation of temperament, character profile, and resilience between maladjusted and adjusted groups. METHODS: A total of 166 maladjusted soldiers who joined the green camp of three divisions and 125 adjusted soldiers participated in the survey from July to December 2013. As self-report questionnaire methods, Beck Anxiety Inventory, Beck Depression Inventory (BDI), and the Korean version of the Connor-Davidson Resilience Scale, Temperament and Character Inventory (TCI) were used. T-test, binary logistic regression analysis, Pearson's correlation, and multiple regression analysis were used as analytical methods. RESULTS: The maladjusted group showed a high level of anxiety, depression, and Harm Avoidance and a low level of resilience, Reward Dependence (RD), Persistence, Self-Directedness, and Cooperativeness compared with the adjusted group. BDI and RD were found to be statistically significant for prediction of military maladjustment. Higher correlation between resilience and TCI was observed for the maladjusted group, but not the adjusted group. CONCLUSION: We suggest that a high depression level and a low RD level should be included as predictors among target symptoms for development of psychological treatment for the maladjusted group. In addition, we suggest that the level of resilience can affect correlation of resilience with TCI.

BACKGROUND: Domestic violence once considered an infrequently occurring event existing only in deviant relationships, it has been more recently recognized as a widely distributed phenomenon resulting from multiple cultural, social, psycological factors. According to declining world economics, family violence rises with a simultaneous curtailment of agencies available to assist battered persons and there is increasing pressure on emergency departments to serve those who might otherwise have sought help from public agency. In Korea, we don't have any clinical data about domestic violence, and authors analysed clinical pictures of domestic violence victims who came to the emergency department of Korean University medical center. METHOD: We reviewed the victims of the domestic violence retrospectively, who visited in ED, Korean Univ. Hospital from Jan. 1996 to the Dec. 1996. We Analysed data to Age, sex, Injury severity score, location of injury, mechanism, perpetrator. RESULTS: During the study period, total 134 domestic violence victims were identified. The young, females were predominantly victimized. And married women were more commonly violated. (about 70%). ISS was relatively mild (2.491), face and head were mainly injured by punching and objects throwing. The Prevalence of domestic violence victims almost presented at Emergency department between 6:00/P.M. and 8:00 A.M.. CONCLUSION: Emergency physicians suspect domestic violence victims who injured at face and head by punching or hitted would. In these patients, we need more detail history and physical exam. And in case of repeated domestic violence, refer to neuropychatry dept and social workers with perpetratrs, and need mere concerte social programs for domestic violence.

BACKGROUND: We studied the incidence of trauma caused by alcohol related accidents, and the effects that has on the occurrence, the extent, and the outcome to the patient. METHODS: In our study we studied trauma patients excluding pediatric patients(15 years old and under) who came to the Emergency department of Korea University Medical School Anam Hospital from the 1st of january 1996 to 30th of June 1996, looked into their medical records, and studied the records in a retrospective manner. The trauma patients were divided into two groups, a) alcohol-related and b) alcohol-non-related. The two groups were then subdivided according to their sex, age, the time they came in, the anatomical part of the trauma, the mechanism of their injury, the extent of the injury, the length of their hospital stay, the length of their ED stay and were seperately compared and analysed. Statistically, ANOVA and logistic regression analysis using SAS were used in the study and then was assessed in Chi-square analysis methods. RESULTS: The total of the trauma patients, added up to 832 people, 577: male and 255: female. Among this sum, 16 trauma patients were alcohol related(male:127 & female 36). 115 people were in the age group of 21-40. Compared to the non-alcohol related trauma group, the alcohol-related group had more facial & scalp injuries and tended to come in the hours between 0-6 AM. The cause of the injuries were mossy by fast-fighting and suicide, compared to mostly accidental-slipping injuries in the non alcohol-related group. There were no difference in the degree of the injury(ISS) and the length of hospital stay between the two groups, whereas the length of the stay at the ED was longer alcohol-related trauma patients. CONCLUSION: Alcohol related trauma patients were mostly in their 20s and 30s, came to the hospital at a late time it the reason for their visit were mossy because of fist fighting. In the Emergency department, because prompt and correct diagnosis is quite difficult to make in this group, their ED stay tended to be longer. We can conclude that measuring the blood alcohol level of these patients, continuing the psychological therapy and educating people is needed.

To fine out the changes of DNA ploidy and cellular proliferation activity during carcinogenesis and evaluate correlation between flow cytometrically determined S-phase fraction and proportion of proliferation cell nuclear antigen(PCNA, PC10) immunoreactive cells, the authors studied on malignant fibrous histocytoma induced by intra-articular injection of 9, 10-dimethy1-1, 2-benzanthracene(DMBA) in the rats. Forty Wistar rats were used. The results obtained were as follows. 1) Firstly, tumors were palpated 5 weeks after the last injection of DMBA and formed in 27 rats at sacrificed. Histologically, these lesions showed storiform, indicative of malignant fibrous histiocytoma. 2) Three cases of DNA aneuploidy were observed at 4 and 5 months after the last injection of DBMA and one of them, which was DNA diploidy at main mass, was found at daughter mass. 3) Flow cytometrically determined S-phase fraction and proportion of PCNA(PC10) immunoreactive cells in malignant fibrous histiocytoma induced by DMBA were much higher than in control groups and slightly increased according to sequential changes after formation of mass. The comparison of flow cytometrically determined S-phase fraction and proportion of PCNA(PC10) immunoreactive cells showed significant correlation(r=0.6092, p<0.001). Above results strongly suggest that ploidy pattern may evolve into aneuploid type during the development of tumor and proliferation activity increases during the carcinogenesis.

PURPOSE: The purposes of the authors' analysis were to assess the values that plastic surgery journals demonstrate in terms of the standardized measures created by the Korea Medical Citation Index, and to assess the relationship between these values and the turnaround time of these journals. METHODS: The overall indexes of J Korean Soc Plast Reconstr Surg(JKSPRS), J Korean Soc Aesthetic Plast Surg(JKSAPS), J Korean Cleft Palate-Craniofac Assoc (JKCPRA) were compared with those of journals related with Korean plastic surgery using the following parameters: impact factor, cited half-life, total articles, and the number of journals. Korean plastic surgery journals were compared with journals from relative fields. In addition, an evaluation of all original articles published in 2007, assessing the time intervals from submission to publication was conducted for Korean plastic surgery journals and various journals which were related with plastic surgery. RESULTS: The number of articles for Korean plastic surgery journals for 2006 ranged from 19 for JKCPRA to 149 for JKSPRS. The time interval from submission to publication of an article among Korean plastic surgery journals for 2007 ranged from 73.7 days for JKSAPS to 176.2 days for JKSPRS. The variation in impact factor of JKSPRS for the period from 2002 to 2005 increased from 0.084 in the year 2002 to 0.168 in 2005. But the impact factor in 2006 has fallen to 0.112. CONCLUSION: JKSPRS demonstrated comparatively high overall index values and a short turnaround time in comparison to relative journals. To improve the status of Korean plastic surgery journals, members of Korean plastic surgeons should quate Korean plastic surgery journals and adjust key word to MeSH. The title written down in Korean should use medical terminology published by Korean medical association.

It awakens interest that the frequency of carcinoma of the uterine cervix is the highest among the carcinomas among the Korean women, althouth it is not so high among Westerns. It is admitted that the exfolicative cytologic technique is useful not only as a screening test for early detection of cervical cancer and its precursor lesions but also as a tool for follow-up, because it is accurate, economical, and easy to perform. But it remains to be solved that the false negativity of the cytologic diagnosis is relatively high. Five hundred and sevently two cases of cytology specimens, confirmed by either colposcopic biopsy or cone biopsy, were studied among 15,844 cases which were submitted to the Department of Pathology, Chonnam University Hospital between October of 1982 and August of 1986. The results obtained were as follows. 1) The accuracy of the cytologic diagnosis was 69.9%, the false nagativity, 19.4% and the false positivity, 6.6%. 2) Among the causes of discrepancies between cytologic diagnosis and histologic diagnosis, error in cytology reading was the most frequent (57%), error in cell collection was the second frequent (43.7%), and error in biopsy location was the least frequent (5.3%). 3) Among the 38 cases of false positive diagnosis, the percentage of error in cytology reading was 82.3%, and that of error in biopsy location was 20.7%. Among the 105 cases of false negative diagnosis, the percentage of error in cytology reading was 50.5%, and the percentage of error in cell collection was 56.6%.

PURPOSE: The differential diagnosis between Modic type I degenerative spine and infectious spondylitis sometimes is difficult, because the affected bone marrows in both disease show similar signal intensity on conventional MR imaging. We evaluate the usefulness of diffusion-wighted MR imaging for differential diagnosis between Modic type I degenerative spine and infectious spondylitis. MATERIALS AND METHODS: The spin-echo and diffusion-weighted MR images of eight patients with Modic type I degenerative spines and 14 patients with infectious spondylitis diagnosed by clinical findings or CTguided biopsies were analyzed. The diffusion-weighted imaging sequence was based on reversed fast imaging with steady-state precession (PSIF). Signal intensity changes of the vertebral bone marrow on conventional spin-echo and diffusion-weighted MR imaging were compared between degenerative spine and infectious spondylitis. RESULTS: On T1-weighted images, the affeted bone marrow in both disease showed hypointense signals. On T2-weighted images, all of type I degenerative spine and 11 of infectious spondylitis showed hyperintensity, and three of infectious spondylitis showed heterogeneous mixed signal intensity. On diffusionweighted MR images, all of type I degenerative spine were hypointense with peripheral high signal intensity to normal vertebral body, but infectious spondylitis was hyperintense (n=11) and hypointense (n=3). CONCLUSION: Diffusion-weighted MR imaging is useful to differentiate Modic type I degenerative spine from infectious spondylitis. On diffusion-weighted images, the high singal intensity of bone marrow suggests infectious spondylitis, whereas the low signal intensity of bone marrow with peripheral focal high signal intensity suggests type I degenerative spine.
