1.5-Fluorouracil, Leucovorin ( FL ) Combination Chemotherapy in Advanced or Recurrent Colo - rectal Cancer.

PURPOSE: We studied the effectiveness and toxicities of 5-fluorouracil+leucovorin, combination chemotherapy in advanced or recurred colo-rectal cancer patients, who didn't have previous chemotherapy and enrolled from August 1993 to July 1998. MATERIALS AND METHODS: All patients were treated with leucovorin followed by 5-fluorouracil for 5 consecutive days every 4 weeks. Among 43 patients who were enrolled, 40 patients received treatment at least 2 courses, and they were evaluable. Male to female ratio was 21 to 19. In serum CEA level, 27 patients were greater than 5 ng/ml and 13 were less than 5 ng/ml. And primary site was colon in 21 patients and rectum in 19 patients. RESULTS: The complete response rate was 7,5% and the partial response rate was 25%. The median survival duration was 14.7 months, the median response duration was 16.0 months, and median time to progression was 7.3 months. In the analysis of response, survival duration, time to progression according to various characteristics of patients, serum CEA level and liver involvement were revealed significant difference in survival duration, time to progression (p=0.0122, 00350 & 0.0202, 0.0123) on univariate analysis, but no significant difference on multivariates. Hematologic and non-hematologic toxicities were mild and tolerable. CONCLUSION: This study indicates that the combination of 5-fluorouracil (370 mg/m) and leucovorin (20 mg/m) is effective and tolerable regimen in advanced or recurred colo-rectal cancer patients without previous chemotherapy.

2.Ulnar Nerve Injury Caused by the Incomplete Insertion of a Screw Head after Internal Fixation with Dual Locking Plates in AO/OTA Type C2 Distal Humerus Fractures.

After dual plating with a locking compression plate for comminuted intraarticular fractures of the distal humerus, the incidence of ulnar nerve injury after surgery has been reported to be up to 38%. This can be reduced by an anterior transposition of the ulnar nerve but some surgeons believe that extensive handling of the nerve with transposition can increase the risk of an ulnar nerve dysfunction. This paper reports ulnar nerve injuries caused by the incomplete insertion of a screw head in dual plating without an anterior ulnar nerve transposition for AO/OTA type C2 distal humerus fractures. When an anatomical locking plate is applied to a distal humeral fracture, locking screws around the ulnar nerve should be inserted fully without protrusion of the screw because an incompletely inserted screw can cause irritation or injury to the ulnar nerve because the screw head in the locking system usually has a slightly sharp edge because screw head has threads. If the change in insertion angle and resulting protruded head of the screw are unavoidable for firm fixation of fracture, the anterior transposition of the ulnar nerve is recommended over a soft tissue shield.

3.A Case of Antipsychotic-Regression Syndrome in Haloperidol Treated Tourette's Syndrome.

Authors report a case of separation anxiety disorder, which developed as a side effect during haloperidol treatment of Tourette syndromes(TS). In this case, 14 years old boys developed attention deficit symptoms during his infancy. At 4th grade of primary school, he developed vocal tic, motor tic, and coprolalia. With 5mg/day of haloperidol treatment his symptoms of TS were subsided. During the treatment, he developed features of separation anxiety disorder, including dependence, pleading, clinging, and sadness. Symptoms of attention deficit and separation anxiety disorder were improved by 25mg/day of imipramine treatment. During haloperidol treatment of TS, careful observation may be needed whether separation anxiety disorder-like symptom develops.

6.Cermline Mutation of RET Gene in A Multiple Endocrine Neoplasia Type 2A ( MEN2A ) Family.

PURPOSE: Multiple Endocrine Neoplasia Type 2A (MEN 2A) is an autosomal dominant disease characterized by development of the medullary thyroid cancer, adrenal pheochromocytoma and parathyroid hyperplasia. Gennline mutations of RET gene, which cause a susceptibility to MEN 2A syndrome, have been reported in MEN 2A families. The identification of germline mutation in family members with hereditary tumor syndrome makes the presymptomatic diagnosis possible. However, there are only a few reports on the germline mutation of RET gene in Korean patients with MEN 2A. This study was performed to investigate the germline mutation of RET gene in a Korean MEN 2A family. MATERIALS AND METHODS: Blood samples were taken from family members of a MEN 2A family. Mutational status was investigated using single strand conformation polymorphism (SSCP) method, and following direct sequencing. Basal level of calcitonin was measured, and calcium provocation test was done when the result of basal level of calcitonin was equivocal. RESULTS: A missense type germline mutation of RET gene was identified at codon 634 (TGC->TGG) in eight patients from the family. All patients with the germline mutation of RET gene showed elevation of calcitonin level either in basal test or in calcitonin provocation test. CONCLUSION: We identified a germline mutation of RET gene in a family with MEN 2A, and it would make the accurate presymptomatic diagnosis possible.

7.Comparison of digital models generated from three-dimensional optical scanner and cone beam computed tomography.

PURPOSE: The objective of this study was to compare the accuracy of digital models from 3 dimentional (3D) optical scanner and cone beam computed tomography (CBCT). MATERIALS AND METHODS: We obtained digital models from 11 pairs of stone casts using a 3D optical scanner and a CBCT, and compared the accuracy of the models. RESULTS: The error range of average positive distance was 0.059 - 0.117 mm and negative distance was 0.066 - 0.146 mm. Statistically (P < 0.05), average positive distance was larger than 70 µm and shorter than 100 µm, and that of negative distance was larger than 100 µm and shorter than 120 µm. CONCLUSION: We concluded that the accuracy of digital models generated from CBCT is not appropriate to make final prostheses. However, it may be acceptable for provisional restorations and orthodontic diagnoses with respect to the accuracy of the digitalization.

8.The effects of hydroxyapatite toothpaste on tooth hypersensitivity.

PURPOSE: The present study was performed to evaluate the effect of hydroxyapatite dental paste on tooth hypersensitivity compared to other materials. MATERIALS AND METHODS: In the general fluoride dental paste, strontium fluoride dental paste and hydroxyapatite dental paste, patient-performed VAS and VAS(ice test) were measured at baseline, 1weeks and 4weeks. RESULTS: 1. In patient-performed VAS, there were significant differences reducing of tooth hypersensitivity between general fluoride toothpaste and hydroxyapatite toothpaste. 2. In operator-performed VAS(ice test), there were significant differences reducing of tooth hypersensitivity among each group, between general fluoride toothpaste and hydroxyapatite toothpaste and between general fluoride toothpaste and strontium fluoride toothpaste. CONCLUSION: In conclusion, hydroxyapatite toothpaste can be applied for control of tooth hypersensitivity.

9.Relation between the interval of supportive periodontal therapy and the prevalence of the subgingival microflora.

This study was performed to evaluate the relation between the interval of supportive periodontal therapy and the prevalence of the subgingival microflora. The subgingival plaques from 108 patients were used in the study. Control group were the patients with no periodontal treatment and test groups were assigned into 3 groups according to the period of recall check : group 1; 1-2 months, group 2; 3-4 months, group 3; 6months or more. The polymerase chain reaction (PCR) used for direct identification of periodontal pathogens (P. gingivalis, T. forsythensis, T. denticola) in subgingival plaque. The results of this study were as follows. 1. The prevalence of P. gingivalis, T. forsythensis, T. denticola in control group were 100%, 87%, 90%. 2. In clinical parameters such as plaque index, gingival index, bleeding on probing, control group was not significant different with group 1 but significant different with group 2, group 3. 3. In group 1, the majority of P. gingivalis had type II fimA. 4. When group 3 were compared with group 1, the prevalence of P. gingivalis increased. But the prevalence of P. gingivalis with type II fimA, which have the virulence factor, decreased. 5. We were unable to find the correlation between P. gingivalis with type IV fimA and periodontal disease. 6. The prevalence of T. forsythensis, T. denticola in test group were 85%, 93% or more. From the above results, we were able to find the relation between the interval of supportive periodontal therapy and the prevalence of the subgingival microflora and the need of the strict supportive periodontal therapy to prevent recurrence of periodontal disease, because there were high prevalence of periodontal pathogens.

10.Effect of high glucose on the prostaglandin E2 production in human gingival fibroblasts and periodontal ligament cells.

The purpose of this study was to evaluate the effect of high glucose on prostaglandin E2 production in human gingival fibroblasts and periodontal ligament cells in vitro. In control group, the cells(5x10(4) cells/ml) were cultured with Dulbecco's Modified Eagle's Medium contained with 10% fetal bovine serum, 45mg/dl glucose. In experimental groups, glucose was added to the above culture condition at the final glucose concentrations of 100mg/dl(Test group 1), 200mg/dl (Test group 2) and 400mg/dl(Test group 3). Then each group was tested for the cell proliferation rate, protein levels, and prostaglandin E2 production at 1/2, 1, 2, 5 days. The results were as follows ; 1. As glucose concentration increased, cell proliferation rate decreased significantly at 1, 2, 5 days in human gingival fibroblasts and periodontal ligament cells(P<0.01). 2. In human gingival fibroblasts, test group 2 and 3 showed significantly decreased protein levels as compared to control group at 5 days(P<0.01). 3. In human periodontal ligament cells, as glucose concentration increased, protein levels decreased significantly at 2 days and 5 days(P<0.01). 4. Prostaglandin E2 production in human gingival fibroblasts and human periodontal ligament cells significantly increased as glucose concentration increased(P<0.01). The results at 5 days showed obvious difference as compared to those at 2 days. From the above results, high glucose appeared to affect cellular activities including cell proliferation rate, protein levels and enhance prostaglandin E2 production. It was assumed that prostaglandin E2 production by high glucose enhances inflammatory reaction and has a toxic effect on human gingival fibroblasts and human periodontal ligament cells. This study suggests that periodontal disease in diabetic patient is related to prostaglandin E2 production.
