1.Monolimb Paralysis after Laparoscopic Appendectomy Due to Conversion Disorder.

Limb paralysis can develop for various reasons. We found a 13-year-old patient who became paralyzed in her lower extremities after laparoscopic appendectomy. Some tests, including electrodiagnostic studies and magnetic resonance imaging, were performed to evaluate the cause of lower limb paralysis. None of the tests yielded definite abnormal findings. We subsequently decided to explore the possibility of psychological problems. The patient was treated with simultaneous rehabilitation and psychological counseling. Paralysis of the patient's lower extremity improved gradually and the patient returned to normal life. Our findings indicate that psychological problems can be related to limb paralysis without organ damage in patients who have undergone laparoscopic surgical procedures.

3.Inhibitory Effects of Norwogonin, Oroxylin A, and Mosloflavone on Enterovirus 71.

Severe complications associated with EV71 infections are a common cause of neonatal death. Lack of effective therapeutic agents for these infections underlines the importance of research for the development of new antiviral compounds. In the present study, the anti-EV71 activity of norwogonin, oroxylin A, and mosloflavone from Scutellaria baicalensis Georgi was evaluated using a cytopathic effect (CPE) reduction method, which demonstrated that all three compounds possessed strong anti-EV71 activity and decreased the formation of visible CPEs. Norwogonin, oroxylin A, and mosloflavone also inhibited virus replication during the initial stage of virus infection, and they inhibited viral VP2 protein expression, thereby inhibiting viral capsid protein synthesis. However, ribavirin has a relatively weaker efficacy compared to the other drugs. Therefore, these findings provide important information that will aid in the utilization of norwogonin, oroxylin A, and mosloflavone for EV71 treatment.

4.A Study on Air Pollution and Thermal Factors in Underground Shopping Center of Pusan Area.

For the purpose of preparing the fundamental data on air pollution in underground shopping center and also contributing to the health improvement of resident, the authors measured the level of SO2, NO2, TSP, CO, CO2 and also some related factors as air temperature, air movement, relative humidity and mean radiation temperature at inside and outside of underground shopping center in Pusan from January to February and from July to August 1994. The results were as follows: 1. The mean concentration of CO within the underground shopping center was 3.1+/-1.3ppm in winter and 2.1+/-0.9 ppm in summer. There was a negative correlation (p<0.01) between inner CO concentration and temperature in summer and no correlation between inner CO concentration and outer CO concentration in underground shopping center 2. The mean concentration of C02 within the underground shopping center was 876+/-353 ppm in winter and 757+/-125 ppm in summer. There was a negative correlation (P<0.01) between inner CO2 concentration and air movement in summer and positive correlation (p<0.05) between inner CO2 oncentration and outer CO2 concentration in underground shopping center. 3. The mean concentration SO2 within a underground shopping center was 0.036+/-0.019ppm in winter and 0.040+/-0.013ppm in summer. There was a positive correlation(p<0.01) between inner SO2 concentration and positive correlation between inner SO2 concentration and outer SO2 concentration in summer and winter in Underground shopping center. 4. The mean concentration of NO2 within a underground shopping center was 0.052+/-0.038ppm in winter and 0.042+/-0.016ppm in summer. There was a no correlation between inner NO2 concentration and thermal factors in summer and winter and low correlation between inner NO2 concentration in underground shopping center. 5. The mean concentration of TSP within a underground shopping center was 430+/-214 microgram/m3 in winter, 366+/-73 microgram/m3 in summer, and very in excess of the atmospheric environmental quality standards of Korea(150 microgram/m3). There was low correlation between inner TSP concentration and temperature in summer and high correlation between inner TSP concentration and outer TSP concentration in underground shopping center.

5.Isolated Noncompaction of the Ventricular Myocardium : Clinical features and Outcomes.

PURPOSE: Isolated noncompaction of the ventricular myocardium(INVM) can present as heart failure or arrhythmias in a child. It is a rare disorder, characterized by prominent trabecular meshwork and deep intertrabecular recesses. We still know little about the diagnosis, symptoms, and clinical outcomes of INVM. METHODS: We included in our study 6 patients who showed ventricular noncompaction on echocardiography. Patients were diagnosed as INVM were excessively prominent trabeculations with deep intertrabecular recesses were found on echocardiography. Patients who had other complex heart lesions such as pulmonary atresia with intact ventricular septum in addition to ventricular noncompaction, were excluded. RESULTS: Age at presentation ranged from 1 day 7 years, with follow up being as long as 6 years. Symptoms at initial presentation were heart murmur, paroxysmal supraventricular tachycardia, cyanosis, feeding intolerance, ventricular tachycardia, and cardiomegaly at fetal screening. Prominent trabeculations and intertrabecular recesses were observed at left ventricular apex in all six patients. All patients were alive at last follow-up. One patient showed WPW syndrome on electrocardiography. Echocardiography revealed decreased systolic function in 4 patients, and decreased systolic and diastolic function in 1 patient. One patient is currently asymptomatic. CONCLUSION: Six patients were diagnosed with INVM with various symptoms at initial presentation. Echocardiography is the most important tool in the diagnosis of INVM due to its morphological characteristics. INVM can rarely be the cause of long term systolic dysfunction, and early detection by echocardiographic screening may be beneficial.

6.A Case of a Hybrid Cyst Composed of an Epidermal Cyst and a Pilomatricoma.

It is known that different parts of a pilosebaceous unit, namely the infundibulum, isthmus, sebaceous ducts, and bulbar or inferior portion, can give rise to a variety of cutaneous cysts, such as epidermal, trichilemmal, steatocystoma, or pilomatricoma. In rare instances, a cyst can arise from various portions of a hair follicle and combine to form a hybrid cyst. The pathogenesis of this lesion is uncertain. Studying the clinicopathological features of hybrid cysts helps us to understand the pathogenesis of disease which can arise from pilosebaceous units. Epidermal cysts and pilomatricoma are recognised to be clinically similar but histologically distinct entities. We present a case of a patient with a solitary nodule on the posterior neck, showing combined histologic features of both an epidermal cyst and pilomatricoma.

7.A Living Donor Liver Transplantation after Therapeutic Plasmapheresis in a Patient with Positive HLA Crossmatch.

A positive HLA crossmatch in cadevaric liver transplantation is relatively acceptable, but in living donor liver transplantation (LDLT) using relatively small sized grafts, the rejection rates were higher in positive crossmatchcases than in negative cases, as described in several previous reports. We report a case of LDLT performed with therapeutic plasmapheresis, in a recipient with a positive HLA crossmatch to donor before transplantation. The patient was a 56-year-old male patient with liver cirrhosis (UNOS status IIA, MELD score 28) caused by chronic hepatitis B. The HLA crossmatch results were 1:2 and 1:8 positive for NIH-CDC (complement dependent cytotoxicity) and AHG-CDC, respectively. The flow cytometric crossmatch (FCXM) was also positive (T-MFI ratio 9.0 and B-MFI ratio 3.4). With 5 cycles of preoperative therapeutic plasmapheresis, the HLA crossmatch converted to negative and liver transplantation was performed. The liver function of the patient was well maintained for 5 months, without any sign of hyperacute or acute rejection. However, the patient eventually died from suddenly occurred infection-associated hemophagocytic syndrome at 5 months after surgery. Therapeutic plasmapheresis can be considered as one of therapeutic options for LDLT patients with a positive HLA crossmatch to donor.

8.Change of Cerebral Blood Flow Distribution and Vascular Reserve according to Age in Koreans Measured by Tc-99m HMPAO Brain SPECT.

PURPOSE: The aim of this study was to evaluate the normal values of regional cerebral blood flow (rCBF) and cerebrovascular reserve (CVR) in normal children to aged volunteers using Tc-99m HMPAO. MATERIALS AND METHODS: Thirty four right-handed normal volunteers (20 males, 14 females, mean age 40.3+/-24.9 years, range 4 to 82 years) were underwent rest/acetazolamide (ACZ) brain SPECT using Tc-99m HMPAO and the sequential injection and subtraction method. rCBF was estimated on the basis of a semiquantitative approach by means of right/left ratio, region/cerebellum and region to whole brain ratios in frontal, parietal, temporal, and occipital lobes, basal ganglia, thalami, and cerebellum. CVR was measured by means of % perfusion increase calculated as % mean count change compared to rest rCBF in each regions. RESULTS: Mean values of right to left ratios range from 1.004 to 1.018. rCBF was highest in cerebellum and lowest in basal ganglia and thalami. Frontal and temporal rCBF decreased while occipital and thalamic rCBF increased according to age. No sexual difference of rCBF was noted. Mean CVR was 29.9+/-12.9%. Mean CVR significantly increased to late teens, and declined thereafter. After 6th decade, CVR in both frontal lobes, left parietal lobe and right basal ganglia decreased significantly with advancing age. There was no sexual difference of CVR. CONCLUSION:: Quantitative assessment of CVR was possible by ACZ Tc-99m HMPAO brain SPECT. It revealed that rCBF and CVR changed according to age in normal Korean volunteers. There was no sexual difference.

9.Multiple Granulation Tissues During Acitretin Therapy.

Acitretin, the principal and free acid metabolite of etretinate, is used to treat patients with several types of psoriasis. The common side effects encountered with acitretin therapy are cheilitis, conjunctivitis, xerosis, dryness of the nasal mucosa with minor nosebleeds, pruritis, and desquamation of the palms and soles. Granulation tissue is a well recognized side effect of oral acitretin. This reaction is characterized by ulceration, hemorrhagic crusting and excess granulation tissue. It occurs between the third and 12th weeks of therapy, most commonly affects the hands and feet. We present a case of multiple granulation tissues associated with acitretin therapy.
10.The Effect of Alloderm on Prevention of Adhesions following Tenorrhaphy in the Rabbits.

PURPOSE: Peritendinous adhesion is one of the most notorious complication after the flexor tendon injury. In this study, Alloderm(R)(LifeCell Corp., Branchburg, N.J.), which is the decellularized human dermal analogue with its intact native basement membrane components, was used for the prevention of peritendinous adhesions following flexor tendon repair. METHODS: Thirty New Zealand white male rabbits were divided equally into 3 groups. In all groups, the flexor digitorum profundus of the third finger of the right back foot was cut totally and repaired by modified Kessler suture technique. Following tendon repair, Alloderm(R) was wrapt around the repaired tendon in the first group and sodium hyaluronate gel was sprayed to the operation field in the second group. In the control group, no external material was applied. The right back foot were immobilized for 6 weeks to optimize the formation of adhesion ingrowth. After death, the third finger that repaired tendons and sheaths was removed en bloc. We checked range of motion. and studied histologically for all groups. RESULTS: The experimental groups had better range of motion than the control group. We checked that the range of motion was 73.5 degrees in Alloderm(R) group, 55.9 degrees in the hyaluronic acid group, and 38.3 degrees in the control group. in the histological study, the experimental group had less adhesions compared with the control group. CONCLUSION: This study concludes that Alloderm(R) can decrease peritendinous adhesions following flexor tendon repairs in rabbits. We think the method could be used in clinical cases.
