In complication of infective endocaditis splenomegaly and splenic infarction are not uncommon but splenic rupture is very rare. We report a case of splenic rupture complicated by infective endocarditis in 1 71-yr-old man who had been suffered from rheumatic heart disease (aortic regurgitation and stenosis and mitral regurgitation). The patient was admitted to mild fever and generalized weakness for 20 days. Diagnosis of infective endocarditis due to Staphylococcus epidermidis was made by clinical manifestaions and blood culture study.On 34th day of admissionthe patient suddenly displayed the symptoms and signs of massive intraperitoneal hemorrhage. Splenic rupture was revealed by paracentesis and radiologic studies. Rupture of spleen is an uncommon and usually fatal complication of infective endocarditis. Therefore early diagnosis and prompt treatment must be performed.

OBJECTIVE: To elucidate the location of leptin and receptors of ovary specimens obtained from patients undergoing hysterectomy by immunohistochemical staining and to determine the effect of leptin on the steroidogenesis of cultured granulosa cells. METHOD: In the culturing process of the granulosa cells, FSH (1 IU/ml)and leptin (50 ng/ml), IGF-I (50 ng/ml) was administered to each study group (Group I: FSH; Group II: FSH, leptin; Group III: FSH, IGF-I, leptin), and the levels of estradiol, progesterone, androstenedione in the culture media was measured by radioimmunoassay. Statistical analysis was conducted by one-way ANOVA with Scheffe test. RESULTS: The results showed that leptin and leptin receptors were both found to be strongly stained in granulosa and theca cells, and also in some interstitial cells. Leptin receptors were also observed in cultured granulosa cells. While there was no statistically significant difference in the androstnedione concentrations between the groups, estradiol concentrations was significantly decreased in Group IV (2202.0+/-151.14 pg/ml) compared to Group III (2859.0+/-122.6 pg/ml), and progesterone concentrations were also significantly decreased in Group II(4696.3+/-190.6 ng/ml) and Group IV (4517+/-206.78 ng/ml) compared to Group III(5546.0+/-179.5 ng/ml). CONCLUSTION: The study result of this study suggest that leptin is directly involved in the regulation of ovarian functions, in particular steroidogenesis.

PURPOSE: To report a case of subhyaloid hemorrhage which resulted from bungee jumping and to describe the clinical course. METHODS: A 21-year-old man who presented with acute diminution in vision following bungee jumping from a height of approximately 64m visited our ophthalmologic department 3 days after the jump. RESULTS: Best uncorrected visual acuity (UCVA) was right eye 1.0 and left eye F.C 10cm initially. He demonstrated a periocular contusion of about 1cm in the left lateral eyebrow. A full examination of the anterior segment was normal but his left fundus showed a subhyaloid hemorrhage of about 7 disc-diameters in size. There were surrounding superficial retinal hemorrhages and peripheral retinal edema. At 54 days follow-up, subhyaloid hemorrhage of about 5 disc-diameters in size remained and visual acuity was not improved. Trans pars plana vitrectomy was performed in his left eye and improved his vision. One month after the operation, the vision was 1.0 unaided in the left eye. The visual acuity remained as 1.0 eight months after the operation. CONCLUSIONS: It is important that the public should be educated about the potential hazard of bungee jumping and we recommend the use of safety glasses during the bungee jump.

A case of metastatic alveolar soft part sarcoma is presented with clincal, pathological and radiological features. Alveolar soft part sarcoma is a rare soft-tissue neoplasm that is malignant and invariably fatal. It was first described and named by Christopherson, et al. in 1952. Since 1952 numerous examples of this tumor have been reported and have been studied with the electron microscope, but there is still considerable uncertainty as to the exact histogenesis of the tumor. Most cases occur in young females involving the lower extrimities especially in the right side. The most initial presenting symptom is a slowgrowing painless mass and the principal metastatic sites are the lungs, followed by the brain and skeleton. Cerebral metastasis, in fact, may be the first manifestation of the disease. Treatment is radical surgical excision but radiotherapy and chemotherapy are less effective. We present the reported case of metastatic alveolar soft pat sarcoma with electron microscopic findings.

PURPOSE: To analyze the clinical course of acquired third, fourth, and sixth cranial nerve palsy at a secondary hospital. METHODS: We analyzed the medical records of 26 patients, 28 eyes who were diagnosed third, fourth, and sixth nerve palsy from March 2004 to September 2005. The clinical courses, affected cranial nerves, etiologic factors and recovery rates were examined retrospectively. RESULTS: The sixth cranial nerve was affected most frequently (12 eyes, 49.2%), followed by the third nerve (11 eyes, 17.9%). Vascular etiologic factors was most common (12 eyes, 42.9%). Only two cases were associated with neurosurgical diseases. Of the 28 eyes that were followed up for at least 3 months, 21 (75.0%) eyes showed a decrease in the angle of deviation. Of these, 14 (50%) eyes recovered completely. CONCLUSIONS: There are many vascular and undetermined etiologic causes of cranial nerve paralysis at 2nd grade hospital. So little cases are result from neurosurgical etiology. There are limitations in finding the affecting factors. Further evaluations for the factors associated with increasing recovery rates were needed.
