1.A Case of Stevens-Johnson Syndrome after Acetazolamide Use.

Erythema multiforme is an episodic, self-limited, mucocutaneous, inflammatory disorder and the disease manifests as severe form is called Stevens-Johnson syndrome. It is most commonly precipitated by herpes simplex infection or drugs and the pathogenesis of the mucocutaneous lesion is thought to be related to immune complexes, cell-mediated immunity, or both. We experienced a case of Stevens-Johnson syndrome in 23-year-old female who had systemic bullous kin lesion and diffue mucopurulent conjunctivitis after use of oral acetazolamide for control of steroid-induced increased intraocular pressure after excimer laser photorefractive keratectomy, which has not been reported in Korea. So, we report this case with a review of the literature.

3.Length determination of long bone by CT scanogram

The CT digital radiography is a new accurate technique for measuring the long bone length. Authors performed acomparative study on accuracy, time and film consumption and radiation dose between conventional spot scanogramand CT scanogram. We used two femur and two tibia specimen for materials, the Somatom II(Siemens) for CTscanogram, and Rad Check(Voctoreen) for radiation dosimetery. There was no significant difference in accuracybetween direct measurement, conventional scanogram and CT scanogram(lesser than 1% difference). The examinationtime of conventional scanogram was about 35 minutes, CT was 15 minutes, and the film consumption of spot scanogramwas 3 of 14x17'' size, but the CT scanogram need only one 8x10'' film for completion of study. The radiation doseof hip, knee and ankle joint were 220 mRad, 365 mRad, respectively, in spot scanogram, but it was lesser than 5mRad in all joints in CT scanogram. The advantages of the CT scanogram are simple, rapid and reduced radiation.

4.Remodelling of Angular Deformity in Birth-Associated Femoral Shaft Fracture.

PURPOSE: To find out the process of bone remodelling and risk factors in birth-associated femoral fracture. MATERIALS AND METHOD: We evaluated the four femoral fractures in three neonates about the obstetric and family history, and measured the angulation at the fracture site and the angle between the proximal and distal epiphysis of the femur on the radiographies taken at regular intervals. RESULTS: The incidence of birth-associated femoral fracture was 0.06%. In two cases, fracture angulation and interepiphyseal angle had been decreased. However the angular deformity was worsened in two cases, but the interepiphyseal angle had been decreased regardless to the change of fracture angulation. So the alignment of epiphyseal plate came to normal alignment of joint surface. CONCLUSION: The underlying disorder should be searched, because of its rarity. The physeal reorientation that makes joint alignment near normal irrespective of amount of angular deformity is assumed as the predominant mechanism in remodelling process of the angular deformity.

Many opthalmologists prefer to treat the eyes operated on with drops containing corticosteroids and antibiotics for a few days of a few weeks after strabismus surgery. But no controlled studies are available to support the rationale for such therapy. Therefore, we conducted a prespective double-blind masked trial to evaluate the clinical effectiveness of the drops containing antibiotics and steroid on 40 patients. There are no singnificant difference between the drug and the placebo groups.

7.An Anatomical Study on Transscleral Suture fixation of Posterior Chamber Intraocular Lens.

We performed an anatomical study on transscleral suture fixation of posterior chamber intraocular lens implantation. Results showed that the relationship of the ciliary sulcus to the overlying posterior surgical limbus differed in the vertical and horizontal meridians. Needles that pierced the ciliary sulcus after being passed perpendicularly through the sclera entered the sclera 0.88+/-0.22mm posterior to the posterior surgical limbus in the vertical meridians and 0.44+/-0.37mm in the horizontal meridians. The scleral exit site of a suture passed from the inside of the eye through ciliary sulcus was 0.90+/-0.12mm posterior to the posterior surgical limbus in the vertical meridians, 0.85+/-0.17mm in the oblique meridians, and 0.45+/-0.10mm in the horizontal meridian.

8.Primary Sarcomas of the Liver.

Primary hepatic sarcomas are uncommon, representing less than 1% of the primary malignant lesions of the liver. Three patients underwent resection for primary hepatic sarcoma at the Department of Surgery, Inje University Paik Hospital, Seoul: a 6 year-old girl with malignant mesenchmoma, a 74 year-old man with malignant fibrous histiocytoma and a 53 year-old man with rhabdomyosarcoma. Abdominal mass, fever with chills, and abdominal pain were the presenting symptoms, respectively. The patient with rhabdomyosarcoma was positive for HBsAg and had chronic active hepatitis. AFP level was elevated in this patient. CA 19-9 level was elevated in the patient with malignant fibrous histiocytoma. CEA levels were normal in all cases. All these tumors were hypodense on computed tomography. Malignant mesenchymoma was hypovascular and rhabdomyosarcoma was hypervascular on angiography. Immunohistochemical stains of the tumors were positive for vimentin but negative for epithelial markers, differentiating these lesions from other hepatic tumors. Electron microscopic examination was helpful in the diagnosis of the specific type of primary hepatic sarcoma. None had postoperative adjuvant chemotherapy or radiotherapy. The patient with malignant mesenchymoma, who underwent right hepatic trisegmentectomy, was disease free at 32 months. The patient with malignant fibrous histiocytoma, who underwent non-curative excision, died of the tumor at 2 months. The patient with rhabdomyosarcoma, who underwent posterior segmentectomy, was alive at 4 months. Resection of primary hepatic sarcoma should be executed if feasible, with potential survival measured in years, in view of the lack of other effective treatment modalities.

10.A Case of Ocular Deviation after Excinier Laser Photorefractive Keratectomy.

Patients undergoing monocular excimer laser photorefractive keratectomy (PRK) for myopic correction can develop strabismus secondary to decompensated heterophorias. We experienced a case of ocular esodeviation and diplopia that developed 8 nionths after monocular excimer laser phtorefractive keratectomy. So, we report this case with 8 review of the literature.
