1.A clinical study on the anti-hypertensive effect of fosinopril in essential hypertensive patients.

In order to investigate the efficacy and safety of oral fosinopril, a new phosphorus containing angiotensin converting enzyme inhibitor, a single dose of 10 to 20mg was administered in 23 hypertensive patients with diastotic blood pressure above 95mmHg and all other anti-hypertensive agents were not administered during 4 weeks of study. Blood pressure and heart rate were measured on the 2nd and 4th week of therapy. The complete blood count with platelet count, blood chemistry by SMA-12 and serum electrolytes were performed at the begining and 4th week of therapy. The urinalysis and electrocardiography were performed at the beginning and 4th week of therapy. Any kinds of side effects were actively questioned by the examining physicians. The following results were obtained : 1) At the beginning and 4th weeks of therapy, the average systolic and diastolic pressure were 170.0+/-17.6/101.6+/-6.1mmHg, 142.7+/-15.1/87.3+/-6.7mmHg respectively. The systolic and diastolic blood pressure were declined statistically significantly(p<0.05) throughout the period of treatment and diastolic blood pressure of all subjects except 3 patients(86%) was maintained below 90mmHg after 4th week of treatment. 2) There was no significant change in the pulse rate before and after therapy. 3) There were no significant changes in blood chemistry, serum electrolytes, hematologic findings, urinalysis and electrocardiographic findigns. 4) side effect were developed in 5 patients(23%) with dry cough, 3 patients(13%) with headache and 2 patients with facial edema but side effects were mostly mild in nature without potenitally serious episodes. These results suggested that antihypertensive therapy with onec-daily fosinopril was effective and well tolerated in essential hypertensive patients.

2.Sudden Hearing Loss in Chronic Myelogenous Leukemia Implicating Hyperviscosity Syndrome.

Sudden sensorineural hearing loss due to the initial manifestation of hematologic disease is very rare. Chronic myelogenous leukemia has been implicated as a causative factor of sudden sensorineural hearing loss. Leukemic infiltration, hemorrhage, infection, and hyperviscosity have been suggested as possible mechanisms in patients with chronic myelogenous leukemia. A 49-year-old male presented unilateral sudden sensorineural hearing loss. The patient was found to have chronic myelogenous leukemia during the work-up for the hearing loss. The WBC count upon admission was 485,100/mm(3). Hemoglobin, hematocrit, and platelet count were within the normal limits. The patient underwent three cycles of leukapheresis and chemotherapy with interferon alpha and hydroxyurea for the treatment of leukemia. The hearing threshold level was 75 dB on admission. It improved to 35 dB when the WBC count fell to 294,000/mm(3), and finally settled at 32 dB two weeks after the termination of chemotherapy when the WBC count was 125,900/mm(3). We present a case of a chronic myelogenous leukemia patient who initially presented with sudden sensorineural hearing loss. We presume that cochlear vessel occlusion as a result of elevated blood viscosity was responsible for this patient's hearing loss. Early onset of sudden deafness in a chronic myelogenous leukemia patient may be due to the hyperviscosity syndrome and be possible to reverse hearing loss through early leukapheresis.

3.Nonfluent Crossed Aphasia after Right Middle Cerebral Artery Infarction: A case report.

Crossed aphasia refers to language disturbance induced by unilateral right hemisphere (non-language dominant) injury in right-handed people who had no previous history of brain damage. Crossed aphasia occurs in less than 2 percent who developed a aphasia. We report a case of a 49-year-old right handed man with language disturbance after right middle cerebral infarction. He showed nonfluent crossed aphasia with Gerstman syndrome such as right-left disorientation, finger agnosia, acalculia and agraphia, but not with apraxia and neglect. At 7 weeks after onset, language function indicated improvement in spontaneous speech and at 19 weeks after onset, improvement in spontaneous speech, comprehension, repetition, naming and reading.

6.The Analysis of Clinical Characteristics of pT2a and pT2b Gastric Cancer.

PURPOSE: T2 gastric cancer is tumor invading proper muscle or subserosal layer. The 2002 American Joint Committee on Cancer subdivided the pT2 gastric adenocarcinoma into a type pT2a (invasion of the proper muscle) and a type pT2b (invasion of the subserosa). The aim of this study was to evaluate the differences between pT2a and pT2b gastric cancers of the clinicopathological features and the prognostic factors. METHODS: A series of curative operations were performed on 194 patients with pT2 gastric cancer from January 1999 to February 2006. We reviewed, retrospectively, the clinicopathologic characteristics, the 3-year disease free survival and the 5-year disease specific survival rates. RESULTS: There were significant clinicopathologic differences between pT2a and pT2b gastric cancer in tumor size and the number of metastatic lymph nodes. Both of the 3-year disease free survival time and the 5-year disease specific survival time were much longer for the pT2a gastric carcinoma patients than those for the pT2b gastric carcinoma patients. CONCLUSION: It is important to consider the differences between pT2a and pT2b gastric cancers for prediction of recurrence or prognosis. Furthermore, new staging of pT2a and pT2b cancer may be necessary.

7.C avernous Sinus Involvement of Adenoid Cystic Carcinoma of Lacrimal Gland.

Adenoid cystic carcinoma is slow growing, but locally aggressive and thus prone to recurrence with late distant metastasis. Intracranial involvement of adenoid cystic carcinoma is rare but most of theses cases are thought to extend directly or by perineural spread from lesions in adjacent structures, such as the paranasal sinuses, nasopharynx, salivary glands, or from the lacrimal glands. And hematogenous metastasis is rare. Adenoid cystic carcinoma of the lacrimal glands is known to comprise 30% of epithelial neoplasms of the lacrimal gland, but its cavernous sinus involvement with perineural extension has never been reported in Korea. A cavernous sinus mass was found in a 43-year-old man presenting ophthalmoplegia and facial palsy. The mass was later diagnosed as metastatic adenoid cystic carcinoma of the lacrimal gland, whose size was decreased after radiation. Thus we report the case with a brief review of literature.

8.Effect of the surface modifications and the use of washer on the reverse torque of the implant prosthetic gold retaining screw.

The screw loosening is one of the complications that happen frequently in dental implant prostheses. The purpose of this study was to evaluate the changes of reverse/loosening (opening) torque of the screw according to the surface modifications by sandblasting and 24K gold electroplating as well as to determine the possibility of the clinical use of a washer in dental implant. The reverse torque of 4 experimental conditions(control, sandblasted, use of washers, electroplasted) was measured by digital torque gauge (Model MGT50Z, Mark-10 Corp., 458 West John Street Hicksville, NY 11801 USA). Electronic torque controller (Nobel Biocare DEA 020) was used in fastening the gold screws into abutment replicas. Mixed Linear Model Analysis method was used for statistical analysis. To examine the changes of screw thread surface, microphotographs were taken by Olympus PME-3 metallurgic microscope (Olympus Optical Co. Ltd., Tokyo, Japan). Within the limitations of this study, the following results were drawn: 1. The surface modifications of the gold screws and the use of a washer have significantly affected the reverse torque value compared to the control group (p<0.01). 2. Sandblasting and electroplating treatments demonstrated significantly higher reverse torque val-ue than that of control group. 3. The use of a washer may be one of the useful clinical methods that prevent the screw loosening. However, further studies are necessary for the material selection and design of the washer.

9.A Case of Simultaneous Primary Gastric and Duodenal Mucosa-Associated Lymphoid Tissue Lymphoma after Therapeutic Endoscopy.

Primary gastrointestinal lymphoma is the most common type of extranodal non-Hodgkin's lymphoma. Most cases are defined histologically as mucosa-associated lymphoid tissue (MALT) lymphoma, while low-grade MALT lymphoma affecting the stomach and duodenum simultaneously is rare. In this case report, we describe the case of a 55-year-old female who presented to the gastroenterology clinic at our hospital for management of gastric adenoma and duodenal polyp by endoscopic mucosal resection and polypectomy. The pathology report noted MALT lymphoma with submucosal tubulovillous adenoma and hyperplastic polyps in the stomach and duodenum. She underwent eradication of Helicobacter pylori using proton pump inhibitor-based triple therapy.
