Orthostatic hypotension occurs when cardiovascular adaptive mechanisms fail to compensate the reduction in venous return that normally occurs on the upright position. A patient with orthostatic hypotension can be a challenging case for a dentist. A 78-year-old male came with idiopatic orthostatic hypotension and felt uncomfortable with his old crown restoration and discoloration on his upper teeth. Management of orthostatic hypotension can be handled by manipulating the dental chair interval movement position.

To find prevalence of orthostatic hypotension (OH) in elder persons living in community, blood pressure was measured in the supine position and after 1-2 minutes of orthostatist in 1286 elderly persons (499 men and 787 women aged 70.447.54 years). The results showed that the prevalence of OH in elderly persons living in community was 18.5%. There are not any significant difference in OH between men and women. Prevalence of OH in “very old” group (75 years old or more) was higher than in 60 - 74 years old group (30.4% vs. 13.8%). The hypertensive persons had the prevalence of OH higher than that of the normotensive persons.

Olanzapine is an atypical antipsychotic which is efficacious in the treatment of schizophrenia. The adverse effect profile for olanzapine is benign except for higher rates of metabolic events. Orthostatic hypotension is less commonly reported with olanzapine as compared to first-generation and few atypical antipsychotics. We report a case where olanzapine, in a dose dependent fashion, caused transient postural hypotension.

PURPOSE: Initial orthostatic hypotension is typically associated with active standing and normal autonomic function. Some adolescents in the present study displayed initial orthostatic hypotension during passive tilting, and we hypothesized that this condition was due to abnormal autonomic function. METHODS: The present study compared autonomic functioning in two groups of adolescents: patients with a diagnosis of initial orthostatic hypotension according to the tilt table test and patients with typical symptoms of initial orthostatic hypotension but normal tilt table test results. RESULTS: Significantly higher total composite autonomic severity scores and considerably higher adrenergic index were noted in the tilt table tested group. CONCLUSION: Initial orthostatic hypotension is possibly associated with autonomic dysfunction, especially abnormal vasoconstrictive ability, in individuals diagnosed with a tilt table test. Clinicians should take note of autonomic function when performing diagnoses of initial orthostatic hypotension using the tilt table test.

Study two cases of primary amyloidosis with the pathognomonic symptom of macroglossia , the diagnosis and treatment of primary amyloidosis. The first one presented with lesions of the tongue, peripheral neuropathy and the bone marrow aspiration revealed <30% plasmocyte with neither bone destruction nor heart, kidney injury. The second case showed diffuse infiltration of amyloid, not only caused the macroglossia but also led to the injury of the heart, pleura, kidneys, subcutaneous tissues, and the autonomic nervous system with postural hypotension

Orthostatic intolerance is the inability to tolerate an upright posture as a consequence of varying degrees of autonomic nervous system dysfunction. Orthostatic intolerance syndromes can be classified into at least 3 categories: 1) orthostatic hypotension, 2) neurally mediated (reflex) syncope, and 3) postural orthostatic tachycardia syndrome. In this review, we discuss the pathophysiology and etiologies of orthostatic hypotension and postural orthostatic tachycardia syndrome, and propose their diagnostic and therapeutic alternatives.

Orthostatic dizziness is a common type of dizziness. In general, orthostatic dizziness is provoked by standing or tilting, and subsided by supine position. The patient with orthostatic intolerance complains multiple symptoms such as dizziness, palpitation, lightheadness, fatigue and rarely syncope. Common orthostatic intolerance is orthostatic hypotension (classic, initial, transient, and delayed orthostatic hypotension) and postural orthostatic tachycardia syndrome. Transcranial Doppler is a noninvasive technique that provides real-time measurement of cerebral blood flow velocity. It can be useful for understanding the relationship between orthostatic symptoms and cerebral autoregulatory function. The reciprocal causal relationship between vestibular and autonomic dysfunction should always be kept in mind.
