Dyskinesia-hyperpyrexia syndrome(DHS)is an acute hyperpyrexia syndrome that is different from Parkinsonism-hyperpyrexia syndrome and serotonin syndrome in patients with advanced Parkinson's disease(PD),with the main symptoms of high fever, disturbance of consciousness, elevated creatine kinase, and dyskinesia. Syndrome of inappropriate secretion of antidiuretic hormone(SIADH)is a clinical syndrome caused by excessive secretion of antidiuretic hormone, which leads to the symptoms of dilutional hyponatremia, water retention, and increases in urine sodium and urine osmotic pressure. DHS has not received widespread attention in clinical practice, and there are rare reports of DHS comorbid with SIADH. This article reports the diagnosis and treatment processes of a case of DHS comorbid with SIADH and reviews the relevant literature,in order to guide the diagnosis and treatment of PD-related critical diseases.

We describe a patient with hyponatremia induced by the use of angiotensin-converting enzyme (ACE) inhibitor; imidapril HCl. Although the mechanism of severe hyponatremia due to ACE inhibitor is not clear, it is conceivable that ACE inhibitor therapy may complicate the syndrome of inappropriate secretion of antidiuretic hormone and induce hyponatremia. In addition, the possibility should be considered that hyponatremia in our patient is a presumptive interaction between oxcarbazepine and imidapril HCl.

Korean Journal of Anesthesiology 1985;18(3):297-302
Dilutional hyponatremia secondary to fluid retention in the postoperative period may be related to a persistent and inappropriate secretion of ADH, possibly orginated from surgical stress, pain, fear and others. It is also serious and over 50% mortality when it should be combined with neurological feature, especially generalized convulsion. But this phenomenon is transitory and complete curable with active management. So Anesthesiologists should be aware of this symptoms by measure of elctrolytes is postanesthetic period. We experienced 2 cases of dilutional hyponatremia with convulsion in postoperative period.
