As the use of chemotherapeutic agents increased rapidly in recent years, more patients are under the potential risk of chemotherapy related adverse reactions. Multiple regular exposures to the same drug by chemotherapy protocol may increase the risk of sensitization to a chemotherapeutic agent, which can result in hypersensitivity reactions. Once severe hypersensitivity reactions occur, causative drugs should be avoided. However, a substitute with equal efficacy is not always available. When there is no effective alternative, desensitization is a safe tool for maintenance of chemotherapeutic agents causing hypersensitivity reaction. In this review, we introduce the latest knowledge about desensitization protocol for chemotherapeutic agents which are frequently used recently.

5.Neonatal food allergy.

Food allergy is defined as abnormal immune response elicited by food intake, in which a variety of clinical symptoms will appear as a result of physiological dysfunction and/or tissue damage. Possible mechanisms for food allergy include gastrointestinal tract barrier damage, failure to induce oral immune tolerance, intrauterine sensitization, and allergen transmission during pregnancy and breastfeeding. Hereditary and environmental factors can also contribute to the disease. Gastrointestinal disorders are the main clinical manifestations of the disease. However, hypoalbuminemia, growth retardation, and even acute circulatory failure or shock may occur in severe cases. Oral food challenges are the "gold standard" for the diagnosis of food allergy. Avoidance and replacement of the responsible food are the only effective treatment options for neonatal food allergy. The use of probiotics can offer protection against the disease.

7.Munchausen Stridor-A Strong False Alarm of Anaphylaxis.

The diagnosis of anaphylaxis is often based on reported symptoms which may not be accurate and lead to major psychosocial and financial impacts. We describe two adult patients who were diagnosed as having recurrent anaphylaxis witnessed by multiple physicians based on recurrent laryngeal symptoms. The claimed cause was foods in one and drugs in the other. We questioned the diagnosis because of absent documentation of objective findings to support anaphylaxis, and the symptoms occurred during skin testing though the test sites were not reactive. Our initial skin testing with placebos reproduced the symptoms without objective findings. Subsequent skin tests with the suspected allergens were negative yet reproduced the symptoms without objective findings. Disclosing the test results markedly displeased one patient but reassured the other who subsequently tolerated the suspected allergen. In conclusion, these 2 patients' symptoms and evaluation were not supportive of their initial diagnosis of recurrent anaphylaxis. The compatible diagnosis was Munchausen stridor which requires psychiatric evaluation and behavior modification, but often rejected by patients.

8.The Hidden Culprit: A Case of Repeated Anaphylaxis to Cremophor.

Drug-induced anaphylaxis is a big pitfall in patients receiving antineoplastic chemotherapy. We report a case of lung cancer patient who experienced two near-fatal anaphylactic reactions that resulted from paclitaxel and multivitamin, seperately. Recurrent severe reactions to different agents led to further investigation to which material the patient was hypersensitive. The skin prick test revealed sensitization to cremophor, which is a commonly used emulsifying agent. This case emphasizes the importance of correctly identifying the culprit drug of anaphylaxis to avoid potentially fatal reaction.

9.Clinical assessment on application of Er:Cr:YSGG laser in dentinal hypersensitivity reduction.

OBJECTIVES: We aimed to evaluate the effectiveness of Er:Cr:YSGG laser therapy on the reduction of dentinal hypersensitivity while taking into account the length and depth of the cervical abrasion. METHODS: We included adults (age, 20-60 years) with at least 2 hypersensitive teeth. The hypersensitive teeth were stimulated with a pressure-indicating probe and an air syringe. The response of the subjects to this stimulation was quantified using a visual analog scale (VAS) and Schiff air sensitivity score. The patients were treated at baseline, immediately after laser treatment, and at 1, 2, and 4 weeks after the Er:Cr:YSGG laser therapy. The results were analyzed by repeated measure analysis of variance (ANOVA) and Bonferroni's post hoc test. RESULTS: The values of tactile test and air blast test showed statistically significant differences between the following time points: baseline and immediately after laser treatment, baseline and first week after treatment, and immediately after laser treatment and first week after treatment (P<0.05). No statistically significant differences were observed in the VAS values of the tactile test and Schiff air sensitivity score of the air blast test between the first and second, first and fourth, and second and fourth weeks after treatment (P>0.05). The depth and width of the cervical abrasion was not statistically significant in dentinal hypersensitivity reduction (P>0.05). CONCLUSIONS: The use of Er:Cr:YSGG laser therapy reduced dentinal hypersensitivity, especially immediately after laser treatment. The depth and width of the cervical abrasion is not statistically significant in dentinal hypersensitivity reduction.

10.Advance in studies on food allergy mechanism based on gut barrier.

Food allergies, as a type of adverse immune-mediated reactions to ingested food proteins, have become a serious public health issue that harms children and adults health, with increasing incidence year by year. However, without effective therapy for food allergies, doctors-have mostly advised to avoid allergens and provided symptomatic treatment. According to the findings of many studies, allergic diseases are correlated with intestinal barrier function injury, as evidenced by the significant increase in the intestinal permeability among patients with food allergies. In this paper, recent studies on correlations between food allergies and intestinal barrier functions, intestinal barrier function injury mechanisms of allergic foods and food allergy intervention strategies based on intestinal barrier functions were summarized to provide reference for laboratory researches and clinical treatment of food allergic diseases.
