1.Disseminated Epidermolytic Acanthoma.

We report a case of disseminated epidermolytic acanthoma in a 69-year-old male who presented numerous, small dark brown verrucous papules on both popliteal fossae, inguinal areas, and abdomen. Although each lesion clinically resembled seborrheic keratosis, light microscopic examination revealed features of epidermolytic hyperkeratosis.

Endometriosis is defined as the presence of endometrial tissue(gland and stroma) outside the uterus. The incidence of scar endometriosis is quite rare, and must differentiate with cellulitis and abscess. We have experienced one case of perineal endometriosis and one case of abdominal wall endometriosis at the site of postoperative wound scar. The possible pathogenesis of endometriosis and treatment were discussed.

Even though a conventional metal ceramic restoration is widely in use, its laboratory procedure is still technique-sensitive, complex, and time-consuming. A ceramic-pressed-to-metal restoration (PTM) can be a reliable alternative. However, simplified laboratory procedure for a PTM is still necessary. The article is to propose a technique that reduces time and effort to fabricate a PTM with the aid of computer-aided design, computer-aided manufacturing and selective laser sintering technologies.

6.A Digital Approach to a Definitive Immediate Denture: A Clinical Report.

Even though an immediate denture (ID) is a practical prosthesis, fabricating an ID may be challenging, as unexpected removals of periodontally compromised teeth may occur during an impression procedure. This clinical report introduces a digital approach to a maxillary ID. An intraoral scanner was applied to prevent accidental extraction. A physical cast and a resin pattern of a framework were fabricated with rapid prototyping technology. A proper border and retention was also achieved by an altered cast impression.

7.Primary Conjunctival Epithelial Cyst in the Orbit.

PURPOSE: To report a case of a primary conjunctival epithelial cyst in the orbit. CASE SUMMARY: A 60-year-old woman was referred for evaluation of proptosis of the left eye, which had developed about 2 years earlier. Upon initial examination, a movable mass was palpated in the medial aspect of the left orbit. Magnetic resonance imaging of the orbit showed a 2.5 cm-sized, ovoid, cystic mass located between the left eyeball and the medial wall of the orbit. Excisional biopsy of the orbital mass was performed. The orbital mass was a well-circumscribed cystic lesion, adherent to the medial rectus muscle. Histological examination revealed that the cyst was lined with multiple layers of cuboidal epithelium with goblet cells. A diagnosis of primary conjunctival cyst was made. CONCLUSIONS: The possibility of primary conjunctival cyst should be considered in the differential diagnosis for an orbital cystic mass.

8.Surgical Management of Colonoscopic Perforations.

PURPOSE: The purpose of this study is to evaluate the clinical features that necessitate a temporary stoma for the treatment of colonoscopic perforations. RESULTS: Between January 2000 and July 2006, 30 patients were treated for colonoscopic perforation. Based on the perforation sites, we classified these patients into the following groups: proximal colon, sigmoid colon, and rectum; we then reviewed clinical data, including the time to operation and management. RESULTS: Seventeen patients had a perforation during the diagnostic colonoscopy. Of these patients, 14 patients had sigmoid colon perforation. Six underwent an operation within 10 hours after perforation. Of these six, four were managed by primary repair or resection with anastomosis, one sigmoid colon cancer patient by anterior resection, and one rectal cancer patient by low anterior resection with diverting ileostomy. Eight patients underwent more than 12 hours after perforation. Of these eight, three were managed by resection with anastomosis and diverting ileostomy and five by resection with end colostomy. Thirteen patients had a perforation during the therapeutic colonoscopy. Of these patients, 10 patients had a proximal colon perforation. Of these 10, 3 without fever or peritonitis symptom were managed by conservative management, 6 by primary repair or resection with anastomosis, and 1 transverse colon cancer patient by right hemicolectomy. Three patients had sigmoid colon perforation. Of these three, one was managed by primary repair, one by resection with anastomosis, and one sigmoid colon cancer patient by anterior resection. CONCLUSIONS: The mechanism of perforation, the site of the perforation, and the time to operation are associated with intraperitoneal contamination and have an influence on surgical treatment.

9.Granuloma Annulare Successfully Treated with a Combined Regimen of Excimer Laser and Topical Tacrolimus.

Granuloma annulare is a relatively common benign dermatosis of unknown etiology. It is typically characterized by skin-colored to violaceous annular or arcuate lesions. The localized form of the disease usually resolves spontaneously, but treatment may be required in cases of intractable symptoms such as pruritus, or for cosmetic purposes. Herein, we report the case of a 65-year-old man diagnosed with a localized form of granuloma annulare that was refractory to various treatments but showed clinical improvement with a combined regimen of excimer laser and topical tacrolimus.

10.Distribution of CD4+CD25+ T cells and graft-versus-host disease in human hematopoietic stem cell transplantation.

PURPOSE: This study aimed to determine the frequencies of CD4+CD25+ T cells in donor graft and peripheral blood CD4+CD25+ T cells in recipients after hematopoietic stem cell transplantation (HSCT) and their association with graft-versus-host disease (GVHD). METHODS: Seventeen children who underwent HSCT were investigated. CD4+CD25+ T cells in samples from donor grafts and recipient peripheral blood were assessed by flow cytometry at 1 and 3 months after transplantation. RESULTS: CD4+CD25+ T cell frequencies in the grafts showed no significant difference between patients with and without acute GVHD (0.90% vs. 1.06%, P=0.62). Absolute CD4+CD25+ T cell number in grafts were lower in patients with acute GVHD than in those without acute GVHD (6.18x10(5)/kg vs. 25.85x10(5)/kg, P=0.09). Patients without acute GVHD showed a significant decrease in peripheral blood CD4+CD25+ T cell percentage at 3 months compared to those at 1 month after HSCT (2.11% vs. 1.43%, P=0.028). However, in patients with acute GVHD, CD4+CD25+ T cell percentage at 3 months was not different from the corresponding percentage at 1 month after HSCT (2.47% vs. 2.30%, P=0.5). CONCLUSION: The effect of frequencies of CD4+CD25+ T cells in donor grafts on acute GVHD after HSCT could not be identified, and the majority of peripheral blood CD4+CD25+ T cells in patients who underwent HSCT may be activated T cells related to acute GVHD rather than regulatory T cells. Further studies with additional markers for regulatory T cells are needed to validate our results.
