1.True Aneurysm of the Superficial Temporal Artery.

This is a case report of a 50-year-old female who presented to our clinic with headache and left temporal pulsatile mass. Aneurysm was revealed incidentally on a computed tomography angiography of her head. Open surgical resection with aneurysmal trapping was performed under local anesthesia. Histological examination demonstrated a true aneurysm of the superficial temporal artery. There was loss of normal elastin in the internal elastin layer. This case report describes the rare entity along with a review of the literature.

2.Comparison of Outcome between Percutaneous Vertebroplasty and Kyphoplasty for Osteoporotic Painful Vertebral Compression Fracture: A Preliminary Report.

STUDY DESIGN: A retrospective study. OBJECTIVES: To compare the outcome of percutaneous vertebroplasty (VP), with kyphoplasty, in the treatment of osteoporotic painful vertebral compression fractures (VCF). SUMMARY OF LITERATURE REVIEW: There is much controversy relating to the treatment of painful osteoporotic VCF. Recent analytical data exists on VP and kyphoplasty. MATERIALS AND METHODS: A consecutive group of patients, undergoing VP and kyphoplasty at our institution, between July 2000 and November 2002, were retrospectively reviewed. A total of 23 patients underwent 25 VP procedures under local anesthesia, and 8 underwent 8 kyphoplasty procedures, 3 under general and 5 under local anesthesia. A radiological assessment was achieved by the percentage of height restored, using both the preoperative and postoperative radiographs. The Visual analog scale (VAS) scores, obtained pre and postoperatively were used for the clinical assessment. The activity levels were assessed preoperatively, after discharge and at the last follow up period, by the ambulatory stati. RESULTS: The VP restored 27.62% (anterior*) and 30.26% (middle**) of the lost height. The kyphoplasty restored 35.52% (anterior*) and 53.43% (middle**) of the lost height (P=0.3334*, P=0.0264**). The postoperative pain was improved in all patients after both procedures. The postoperative VAS score was 3.826 after the VP and 2.875 after the kyphoplasty (P=0.5647). The activity levels were improved in all patients after both procedures. CONCLUSIONS: The kyphoplasty was more efficient in restoring the middle vertebral body height than the VP in the treatment of osteoporotic painful VCF. However, both procedures showed similar clinical improvements in the pain and restoration of the anterior vertebral body height in the treatment of painful osteoporotic VCF. Both kyphoplasty and VP safely increased the vertebral body height, decreased the acute back pain and quickly returned geriatric patients to higher activity levels, resulting in an increased independence and quality of life.

3.Surgical Treatment of Isthmic Spondylolisthesis: Pedicle Screw Fixation, Posterolateral Fusion, and Posterior Lumbar Interbody Fusion with Cage after Wide Decompression.

STUDY DESIGN: A retrospective study. OBJECTIVES: To verify the advantages of adding gentle reduction and posterior lumbar interbody fusion (PLIF), using a cage to the usual posterolateral fusion (PLF), with pedicle screw instrumentation, in the surgical treatment of spinal stenosis with isthmic spondylolisthesis. SUMMARY OF LITERATURE REVIEW: The stabilization of isthmic spondylolisthesis, following decompression, is difficult. The PLIF, with a cage, offers anterior column support, reduction and a broad fusion base. MATERIALS AND METHODS: 31 patients were treated with wide decompression, pedicle screws fixation, PLF and PLIF, and followed up for more than 1 year. The degrees of slippage were grades I and II in 20 and 11 patients, respectively. The grade I patients were treated with gentle reduction of the slippage in the disc space, using a leverage maneuver with a Cobb's spinal elevator. The grade II patients were treated with the insertion of a pedicle screws, fixation of rods, reduction and distraction, and then insertion of a cage. After the procedure all the patients were evaluated for the reduction of spondylolisthesis, restoration of the disc space, radiological bony union and clinical results. RESULTS: Ninety percent of the patients were rated as excellent or good. Fusion of the PLIF occurred in all patients. The average reduction in the spondylolisthesis was 42.6 and 47.8% in the grade I and II patients, respectively. The average restorations of the disc spaces were 46.9 and 100.2% in the grade I and II patients, respectively. The maintenance of the reduction and disc height were excellent in the final follow-up radiographs. CONCLUSIONS: Adding gentle reduction and PLIF, using a cage, to the usual posterolateral fusion, with pedicle screw instrumentation, in the surgical treatment of spinal stenosis, with isthmic spondylolisthesis, showed satisfactory results in the reduction of the spondylolisthesis, the restoration of the disc height, the bony union and clinically.

4.Comparison of Clinical Manifestations of RSV, Rhinovirus and Bocavirus Infections in Children with Acute Wheezing.

PURPOSE: The purpose of this study was to compare clinical manifestations of human bocavirus (hBoV), respiratory syncytial virus (RSV), and human rhinovirus (hRV) in children with acute wheezing. METHODS: We retrospectively investigated 549 virus-positive throat swabs obtained between January 2006 and December 2008 from pediatric inpatients with acute respiratory tract disease at Kwangju Christian Hospital. Among them, 109 patients, who had lower respiratory tract infections with wheezing, were enrolled in this study. The medical records of patients with positive results were reviewed for clinical data. RESULTS: The mean age of the patients with RSV was 7.15 months, 15.66 months in those with hRV, and 15.04 months in those with hBoV. The mean fever duration and frequency of patients with fever was 2.43 days and 47.9% for RSV, 2.86 days and 51.7% for hRV, and 3.75 days and 69.6% for hBoV. The frequency of patients with acute otitis media was 20.8% in the RSV, 20.7% in the hRV, and 13.0% in the hBoV groups. The frequency of lung infiltration on chest X-ray was 12.5% in the RSV, 20.7% in the hRV, and 47.8% in the hBoV groups. CONCLUSION: We compared the clinical manifestations of respiratory viral infections in infants and children with wheezing. However, further surveillance will be necessary to clarify the clinical manifestations of the viruses.

5.The Reliability of Proximal Femoral Shaft Fracture Classification.

PURPOSE: The Garden classification by which femur neck fracture is classified and the Boyd-Griffin classification by which trochanteric fracture is classified are studied on the reproducibility, repeatability, interobserver's and intraobserver's reliability and then reliability. MATERIALS AND METHODS: 56 cases in femoral neck fracture and 60 cases in trochanteric fracture who were operated from May 1999 to December 2003 were classified by three observers who are hip surgeon, orthopaedic surgeon and senior residentship doctors three times. Femur neck fracture was classified by Garden's method which used commonly and trochanteric fracture was classified by Boyd-Griffin method which is classified by the pattern of fracture and degree of comminution. We got the interobserver's and intraobserver's Kappa score using the Stata 7.0 statistically. The statistical analysis was made by Stata 7.0. RESULTS: Garden classification in femur neck fracture showed moderate agreement in intraobserver reliability and fair agreement in interobserver reliability. Boyd-Griffin classification in trochanteric fracture showed substantial agreement in intraobserver reliability and moderate agreement in interobserver reliability. CONCLUSION: Boyd-Griffin classification showed over moderate agreement but Garden classification showed fair agreement, so using Garden classification in femur neck fracture has some problem in reliability and application.

Transcatheter closure of secundum atrial septal defect(ASD) with a "buttoned" double-disk device is feasible, effective and safe method as an alternative to surgical closure. Stretched diameter of ASD, determined by balloon sizing is generally used as a guide to prediction of success or selection of device size used for transcatheter closure of ASD. To test whether other non-invasive assessment of ASD size can provide an alternative method for a stretched diameter, we studied the relationship between various measures of ASD in 22 adult patients with ASD. Although transthoracic echocardiographic horizontal or vertical diameter of ASD, the maximal diameter of ASD measured at operation and pulmonary-to systemic flow ratio(Qp : Qs), the stretched diameter had no significant correlation with other measurements. It is conclused that other assessments of ASD size can not be used as adjuncts in the estimation of the stretched ASD diameter, which in turn can be used for prediction of success or selection of device size for occlusion of the ASD.

7.The Distribution of Malassezia Yeasts on the Skin of Neonates and Infants.

BACKGROUND: Malassezia yeasts are normal skin flora of humans. But skin colonization appear to be controversial during neonate. OBJECTIVE: We prospectively studied the distribution of Malassezia yeasts on clinically normal skin of neonates and infants for providing the basic data for proving the relationship of Malassezia yeasts and pathogenesis of the diseases of neonates and infants. METHODS: A total of 200 subjects were studied using the direct smear test with 20% Parker ink/KOH solution. The numbers of the Malassezia yeasts per high power field were counted according to a bacterial index of lepra bacilli in patients of leprosy. In order to identify risk factors for the distribution of Malassezia yeasts, we compared sex, mode of delivery, gestational age, birthweight during the first week of life by statistical method of the logistic regression. RESULTS: Of the 200 neonates and infants under 12 weeks, 121 (60.5%) revealed Malassezia yeasts in at least a part of five examined sites. The prevalence of Malassezia yeasts was increased according to the age. No association was found between the incidence of Malassezia yeasts and sex, mode of delivery, gestational age, birth weight. CONCLUSION: We conclude that Malassezia yeasts colonize on the skin of neonates and infants.

8.Two Cases of Rickets that Developed as a Result of by Diet Restriction due to Atopic Dermatitis.

Nutritional rickets, which is caused by deficiency of calcium or vitamin D, is a rare disease in developed countries. However some cases have been reported recently, that developed as a result of a restricted and nutritionally imbalanced diet due to atopic dermatitis and related food allergy. We treated two infant cases of nutritional rickets. The infants had suffered from atopic dermatitis, and were fed "Sun-sik" (a powdery mixture of several grains and fruits) without receiving cow's milk or any milk products in their diet. After an adequate supply of calcium and nutritional management, they were markedly improved.

9.Perforation of Surgical Gloves during Lower Extremity Fracture Surgery and Hip Joint Replacement Surgery.

PURPOSE: To assess the frequencies and sites of surgical glove perforations in lower extremity fracture surgery and hip joint replacement (HJR) surgery. Additionally, we also studied the usefulness of an indicator system glove. MATERIALS AND METHODS: We assessed surgical glove perforations in 30 cases of lower extremity fracture surgery and 18 cases of HJR surgery conducted by one right handed 1st operator from April 2013 to July 2013. We assessed frequencies and sites of perforation in 152 gloves; 95 used in lower extremity fracture surgery and 57 used in HJR surgery. We studied the perforation rates and sites according to participants and operation types. Using the Biogel indicator system glove, which is well known as a fast indicator of glove perforation, we were also able to assess the time difference between operative participant detection of perforation and inspector nurse detection while observing in the operative field. RESULTS: There were 18 of 30 cases in lower extremity fracture surgeries and 12 of 18 cases in HJR surgeries which had more than one surgical glove perforation event. Of all 152 gloves used, perforation occurred in 15 of 57 gloves (26.3%) in HJR surgery and 23 of 95 gloves (24.2%) in lower extremity fracture surgery. Perforation occurred more frequently in operators than assistant doctors or scrub nurses. The most frequent perforation site was the second digit of the left hand. On average, the time difference between operative participant notice of perforation and inspector nurse notice of perforation was 20.6 seconds. CONCLUSION: The perforation of surgical gloves happened in approximately one out of every four persons. Importantly, we noted a 37.0% prevalence of glove perforation in 1st operators. Considering that glove perforation is a critical factor responsible for intra-operative infection, surgeons must be conscious of the risk of surgical glove perforation and use double gloving regularly. Furthermore, indicator double gloving is recommended for fast detection of outer glove perforation.

10.Clinical Outcomes of Phacoemulsification Technique with Intraocular Lens Implantation in Leprosy Patients.

PURPOSE: To evaluate the surgical results and complications in leprosy patients with cataract who underwent phacoemulsification and intraocular lens implantation retrospectively. METHODS: A total of 17 eyes of 13 leprosy patients with cataract underwent a phacoemulsification and posterior chamber intraocular lens implantation. We performed pupillary sphicterectomy or sector iridectomy for miotic pupils. RESULTS: With mean age of 73 years and a mean follow-up of 8.2 months, patients admitted for 3.2 days. The average visual acuity was 0.1 (0.04~0.3) preoperatively, 0.5 (0.04~1.0) at 1 week after operation, and 0.6 (0.1~1.0) at 2 months after operation. Intraoperative complications were posterior capsular rupture (2 eyes) and vitreous loss (1 eye). Common Postoperative complications were after cataract (4 eyes) and hyphema (2eyes). CONCLUSIONS: We thought the phacoemulsification and intraocular lens implantation was useful method for cataract of leprosy patient for increased life quality and satisfaction.
