1.Trichorhinophalangeal Syndrome , Type I.

Trichorhinophalangeal syndrome (TRPS) was first described in 1966 by Giedion. It is a rare genetic disease and divided into two groups : TRPS type I and TRPS type II. TRPS type I is an autosomal dominant inherited disorder, defined by three characteristic findings : first, sparsely distributed brittle and slow-growing scalp hair, second, a bulbous pear-shaped nose with an elongated philtrum, third brachydactyla with angular deformities. The characteristic radiological features of the hands include cone shaped epiphysis of the proximal interphalangeal joints. Most TRPS type II is sporadic and shows multiple exosteosis, mental retardation and microcephaly in addition to the three chracteristic findings of TRPS type I . A 20-year-old female visited our department for the evaluation of her sparse, slow-growing scalp hair. She also had a bulbous pearshaped nose, ulnar deviated fingers and shortened both hallux. Some relatives of patients show similar symptoms of scalp hair. A Radiologic aly, histopathologic aly and chromosomal study were done, and we established the diagnosis, TRPS type I.

2.Metopic Suture in Koreans.

We studied the incidence and morphological variation of the metopic suture in Korean. Ninety-six adult Korean dried skulls and 141 cases of antero-posterior skull roentgenograms from age of 1 month to 80years old. We divided metopic suture into complete and incomplete type. Incidence of complete type was 8.3% in dried skulls. In roentgenograms the incidence was decreased according to aging, especially around 6-8 years old, and over 20 years old age group the incidence was 4.0%. The incidence of incomplete type was 32.3%.

4.Arthroscopic Debridement in Osteoarthritis of the Knee.

A retrospective review was performed in 33 knees of 30 patients who had undergone arthroscopic debridement for degenerative osteoarthritis of the knee. The mean age of the patients at operation was 55 years and the mean follow-up after operation was 15 months. We divided the knec:s into 2 groups; meniscectomy group (23 knees) and non-meniscectomy group (10 knees). The preoperative radiographic findings were divided into 3 groups according to the classification systern by Lotke et al. The postoperative results were evaluated using the nine-point scale by Baumgaertner et al. Our study was conducted to: 1) assess the overall efficacy of arthroscopic debridement, 2) compare the results between the. Meniscectomy group and nonmeniscectomy group, and 3) identify the relationship between the preoperative radiographic findings and out- come at the final follow-up time. Overall excellent or good results were ac.hieved in 22 knees (67%) and the preoperative radiographic findings correlated with the outcome at the final follow-up time. But the results in the meniscectomy and non-meniscectomy poups showed no significant differences statistically. We believe that arthroscopic debridement is an effective option of treatments for mild I:o moderate degenerative arthritis of the knee after failure of conservative measures.

6.A Case of Neurofibromatosis Associated with Moyamoya Disease.

Neurofibromatosis produces a broad spectrum of clinical manifestations. Cafe-au-lait spots, cutaneous neurofibroma and tumors of the central and peripheral nervous system are well known manifestations. One of the more serious aspects of the disease relates to the arterial involvement. Renal arterial disease with resultant hypertension has been paticularly well documented. However, cerebrovascular lesions in neurofibromatosis are uncommon. Cerebral arterial occlusive diseases with juxtabasilar telangiectasia (moyamoya disease) associated with neurofibromatosis have been documented in about 40 patients in the world literature. We report a case of neurofibromatosis associated with the moyamoya disease.

7.Femoral Tunnel Enlargements Following Arthroscopic ACL Reconstruction

Enlargement of bone tunnels has been noted on plain X-rays following arthroscopic ACL reconstruction. The cause of this widening is unclear, but it has been hypothesized that it may be due to either mechanical or biological cases. Ishibashi et al. reported anatomical proximal fixation resulted in the most stable reconstructed knee, with increasing instability as the level of fixation moved away from the tibial plateau. The purpose of this study is to determine if any difference exists in the amount of enlargement of the femoral tunnel following arhotoscopic ACL reconstruction with position of interference screw fixation and instability, and to know the factors which affected to the enlargement of the femoral tunnels. Total 39 patients were retrospectively reviewed for tunnel measurements radiologically at one year post-operation. (27 patients received bone-patellar tendon-bone autograft, 12 patients received bone-patellar tendon-bone autograft and Kennedy LAD-ligament augmentation device). The surgery was performed using an arthroscopic single and double incision technique. AP and lateral X-rays were obtained and the tunnels were measured by two independent observers using a digital caplper. The measurements were made at the widest part of the tunnel. Correction for magnification was performed by comparing the measured width of the interference screw used for fixation of the graft with its actual width. Statistical analysis was performed using a one-way analysis of variance(ANOVA) and t-test. 1. Radiographic tunnel enlargement of femoral side was average 2.42 mm (bone-patellar tendon-bone autograft : 2.36 mm, Kennedy-LAD and autograft : 2.56 mm)(p>0.05). 2. According to the position of the interference screw, the femoral tunnel enlargement were 2.25 mm in anatomical fixation, 2.40 mm in mid-tunnel fixation, 2.62 mm in mid-tunnel fixation, 2.62 mm in outer-tunnel fixation(p < 0.05). 3. The femoral enlargement according to the overall results(Clancy, 1982) were 2.39 mm in above good result group and 2.50 mm in below fair result group(p < 0.05). Tunnel enlargement group of femoral side was related to a distance between femoral articular surface and the position of interference screw. We conclude that femoral tunnel enlargement following arthoscopic bone tendon-bone ACL reconstruction is related to the mechanical effect rather than the properties of grafts and the clinical results.

9.CLINICAL STUDY ON POLYDACTYLY OF THE FOOT.

Polydactyly is one of the most common congenital anomalies of the foot, but there are few papers on polydactyly of the foot reported in Korea. Forty-six supernummerary toes in thirty-semen patients were operated at Department of Plastic and Reconstructive Surgery, Korea University Guro Hospital, from May 1985 to May 1995. A clinical review was performed on these cases and the results were as follows : 1. Of the 37 patients, the proportion of unilateral involvement to bilateral was 28(76%) to 9 (24%). 2. Of the 46 toes, postaxial type was a majority involving 35 toes(76%) followed by the preaxial type 7 (15%) and central type 4(9%). 3. In anatomical pattern, Wassel's type IV and Stelling and Turek's type II were the most common ones in 7 toes of preaxial type and 39 toes of central and postaxial types. 4. The excellent results could be made by individualized reconstructive methods performed before walking age.

10.A Case of Cutaneous Horn Arising from Actinic Keratosis.

Cutaneous horn is the clinical term for a circumscribed, conical, markedly hyperkeratotic lesion. We report a case of cutaneous horn occuring on the face in a 81 year old women. An about 1.2 X1.3 cm sized dark brownish conical horn was developed on her face 1 year before. Histopathologic findings showed marked hyperkeratosis, parakeratosis, and papillornatosis of the epidermis, and some cells of straturn mslpighii showed pleornorphism and nuclesr atypis. This case was considered to be cutaneous horn arising from actinic keratosis, and we performed minimal local excision.
