The tricuspid valve is not a simple but a complex structure, tricuspid valve complex. This complex is composed of tricuspid orifice, annulus, valve leaflet, chordae tendineae and papillary muscles. There are flew articles about these structures. The authors studied tricuspid valve complex in 53 cases of normal korean adults, such as circumference of tricuspid annulus, the maximum diameter of the tricuspid orifice, height and breadth of the cusps, including commissures, the ratio of rough to clear zone, the number of scallops of posterior cusp, the number, length and pattern of arrangement of the several types of chordae, the number and morphological characteristics of papillary muscles.

2.A Case of Stenotrophomonas Maltophilia Endophthalmitis after Cataract Operation.

It has been reported that the cause of the bacterial endophthalmitis following cataract extraction and intraocular lens implantation was G(+) organisms in most cases. However, the endophthalmitis caused by G(-) organisms has worse prognosis than that by G(+)organisms. We experienced a case of Stenotrophomonas maltophilia endophthalmitis which has not yet been reported in Korea. A 77-year-old female presented with blurred vision, conjunctival injection and ocular pain 5 days after cataract extraction and intraocular lens implantation. Ocular findings such as corneal edema, hypopyon and thick posterior capsular opacity were consistent with infectious endophthalmitis. Culture of vitreous aspirate isolated Stenotrophomonas maltophilia 15 days after cataract extraction.

3.Two Cases of Congenital Giant Melanocytic Nevus (CGMN) with Vitiligo.

Congenital giant melanocytic nevi (CGMN) are lesions of more than 20 cm in diameter, often in the distribution of a garment. Depigmented zones may appear around several types of acquired lesions such as dermal, junctional, and compound nevi, Spitz nevus, blue nevus, and malignant melanoma. The depigmentation may occur within the lesion, around it, or at a distant site. In contrast, congenital nevi rarely develop depigmentation. Vitiligo-like leukoderma distant from the nevus has developed in only three cases with a CGMN. We report two cases of CGMN with vitiligo which is located away from primary CGMN lesion.

6.Percutaneous Pedicle Screw Fixation in Thoracolumbar Fractures: Comparison of Results According to Implant Removal Time

BACKGROUND: The purpose of this study was to determine whether restoration of range of motion (ROM) could be achieved by implant removal after natural bone healing and consolidation of fractured vertebrae and examine whether early removal of the implant could maximize restoration of ROM. METHODS: This study included 30 cases of thoracolumbar fractures without neurological deficit requiring surgery (nine cases of flexion-distraction injuries and 21 cases of burst fractures). Percutaneous pedicle screw fixation (PPSF) was performed at the fractured vertebrae and one level above and one level below the fracture level. Pedicle screws were removed at an average of 12 months after surgery upon healing of fractured vertebrae. The following radiological and clinical findings were evaluated: restoration of anterior vertebral height ratio (AVHR), Cobb angle (CA), ROM, and complications. Sixteen patients who were checked for ROM were divided into two groups based on the time of implant removal: nine patients within 12 months and seven patients after 12 months. Restoration of vertebral height loss and ROM were compared between the two groups. RESULTS: At the final follow-up, significant pain relief and restoration of AVHR and CA were achieved in patients who underwent PPSF. Patients who had implant removed within 12 months after surgery had better ROM recovery than those who had implant removed after 12 months postoperatively. There were no significant differences in AVHR and CA between the two groups. CONCLUSIONS: PPSF followed by implant removal after healing of fractured body appears to be effective in achieving restoration of ROM. In our study, early removal of implant within 12 months after surgery was associated with better achievement of ROM than removal after 12 months. In addition, there were no significant differences in restoration of vertebral height between the two groups.

7.A Study of Standards and Norms Used for Electrodiagnosis in Korea.

In clinical settings, electrodiagnosis is used for the differential diagnosis of neuropathy and myopathy, as well as detremining severity and localization of lesions in the neuromuscular system. By many authors, various methods of the study and factors influencing the results were verified so far. However, the results vary according to methods or influencing factors during electrodiagnostic studies. Since there has been no standardization in methods of the study and study environment, we sometimes feel difficulties in interpretation of study results and in exchage of findings of study among laboratories. In this study, we have collected standards and norms used by different electrodiagnostic laboratories in Korea, hoping that we can come up with one nationwide standards and norms in Korea.

9.Direct Ureteric Length Measurement Using Intravenous Pyelography.

PURPOSE: The ureteric length is the most important factor in determining the ideal length of a ureteral stent. In most literature, the ureteric length has been estimated according to the patient height. The ureteric length and reliability of the patient's height were both investigated. MATERIALS AND METHODS: The actual ureteral trace(AUT) and linear distance(LD) from the ureterorenal junction to the ureterovesical junction were measured on the 15 minute intravenous pyelography(IVP) view. A total of 203 patients(100 men, 103 women and 406 ureters), with normal findings, were studied. The AUT, LD and height were all measured. RESULTS: The mean+/-SD height and AUT of the right and left ureters of the patients were 164.3+/-8.3, 23.4+/-1.9 and 24.4+/-2.0cm, respectively. Mean LD of right and left ureters were 22.1+/-1.9 and 22.9+/-2.0cm, respectively. The patient's height showed significant correlations with both the AUT and LD, but these correlations were not linear (R2=0.024 [Rt], 0.059 [Lt]). However, the AUT and LD had a linear correlation (R2=0.879 [Rt], 0.884 [Lt]). Two formulae for estimating the AUT from measurement of the LD were suggested: Right AUT=0.94xright LD+2.6. Left AUT=0.96xleft LD+2.4. CONCLUSIONS: Our results have demonstrated that a patient's height was not reliable for estimating the ureteric length. The LD on IVP is preferable to the patient's height in estimating the ureteric length.

10.Incidence and Risk Factors of Tuberculosis in Human Immunodeficiency Virus-Infected Patients in Korea.

BACKGROUND: Tuberculosis is the most frequent opportunistic infection in HIV-infected patients in Korea. We examined the incidence and risk factors for tuberculosis in HIV-infected patients. METHODS: We reviewed the medical records of 143 HIV-infected patients between January 1988 and June 1997 at the Seoul National University Hospita The incidence of tuberculosis was determined by the number of new tuberculosis per 100 patients divided by a total duration of follow-up (cases/100 person- years). The incidence of tuberculosis was analyzed with respect to CD4+ lymphocyte count, status of tuberculin skin reaction, and status of delayed hypersensitivity skin reaction. RESULTS: The total duration of follow-up was 229.6 person-years. During follow-up, 22 patients developed tuberculosis [9.6 cases per 100 person-years (95% CI, 6.0 to 14.5)]. The incidence of tuberculosis according to sex and transmission route showed no significant difference. Tuberculosis occurred more frequently in patients with minimum CD4+ T lympho-cyte counts of less than 200 cells/mm3 (14.2 cases/100 person-years) than in those with higher T lymphocyte counts (3.9 cases/100 person-years) [relative risk, 4.02 (95% CI 1.32 to 12.5), P=0.009]. The incidence of tuberculosis was higher among tuberculin- positive patients [17.0 cases/100 person-years (95% CI, 9.0 to 29.0)] than among tuberculin-negative [8.0 cases/100 person-years (95% CI, 3.2 to 16.6)], but the difference was not statistically significant [relative risk, 2.35 (95% CI, 0.88 to 6.26, P=0.078)]. The rate of tuberculosis in patients with negative delayed hypersensitivity test was not significantly different from that in patients with positive results (P=0.510). CONCLUSION: The incidence of tuberculosis was 9.6 cases per 100 person-years. CD4+ lymphocyte count (<200 cells/mm3) was the most useful predictor for the development of tuberculosis in HIV patients in Korea.
