1.Asbestosis Epidemics Caused by Non-occupational Neighborhood Exposure.

Asbestos is a naturally occurring mineral in the Earth's crust, and it is not confined to the historic and current asbestos mining areas, but rather quite commonly encountered in certain geological environments across the world. The middle-west side of the Korean Peninsula (Chungnam province) has been composed of numerous talc and asbestos ore deposits derived from serpentine and dolomite. These areas have experienced epidemics of malignant mesothelioma and pleural plaques as a result of non-occupational "neighborhood" exposure and naturally occurring asbestos (NOA) exposure. However, asbestosis epidemics from environmental exposure were rare because asbestosis needs relatively high concentration of asbestos exposure. This review summarizes asbestosis epidemics which occurred in the residents who have lived past or current asbestos mining areas and natural in-place asbestos deposits (or asbestos-like mineral deposits) in the world, including Korean asbestosis endemics that occurred in Chungnam province, which has many historic asbestos mining. Also, this manuscript will describe effective public policies for managing NOA, minimizing potential hazards, and protecting public health from asbestos exposure.

2.Epidemiologic Characteristics of Malignant Mesothelioma in Korea.

Malignant mesothelioma is an aggressive tumor occurring in mesothelial cells of the pleura and peritoneum. This cancer is attributable to asbestos in 80~90% of the patients. Asbestos has been widely used in Korea since the 1970s, therefore, a large number of Korean workers, especially who engaged in the industries of asbestos textiles, shipbuilding, or construction, have been exposed to asbestos occupationally. Korea has several kinds of statistics showing mesothelioma mortality and morbidity, which include cancer registry data, mortality data, and surveillance data. According to the annual report of the Korea Central Cancer Registry (KCCR), fatal malignant mesothelioma occurred in around 40~50 cases a year during the recent 10 years (1993~2002). According to the surveillance data, about 34 cases have been reported annually since 2001. It has also been revealed that about 60% of malignant mesothelioma patients have experiences to be exposed to asbestos in the past. The number of mesothelioma patients in Korea is smaller than that of some developed countries such as United Kingdom, Germany, and Japan. However, mesothelioma has increased greatly in recent years in Korea, and it is expected to increase continuously considering asbestos consumption, as it happened in other countries which used large amounts of asbestos. Therefore, active policies to prevent asbestos-related malignant mesothelioma and to compensate patients suffering from it have to be made in Korea.

3.The effect of restorative materials on the stress distribution of class V composite resin restorations: a 3D finite element investigation.

The purpose of this study was to analyze the stress distribution aspect of unrestored and restored combined shape (wedge shape occlusally and saucer shape gingivally) class V cavity, which found frequently in clinical cases. A maxillary second premolar restored with a combined shape class V composite restorations were modeled using the three dimensional finite element method. Static occlusal load of 170 N was applied on lingual incline of buccal cusp at the angle of 45degrees with the longitudinal axis of the tooth. And three dimensional finite element analysis was taken by ANSYS (Version 6.0, Swanson Analysis System Co., Houston, U.S.A) program which represent the stress distribution on unrestored and restored cavity wall and margin. The conclusions were as follows. 1. Compared to the unrestored cavity, Von Mises stress at the cementoenamel junction and line angle of the cavity base were reduced and in restored cavity. 2. Von Mises stress at the occlusal and cervical cavity margin and wall were increased in restored cavity in comparison with the unrestored cavity. 3. In the hybrid and hybrid/flowable composite resin restoration, Von Mises stress at the cementoenamel junction and line angle of the cavity base were reduced more than in the flowable restoration. 4. In the hybrid and hybrid/flowable composite resin restoration, Von Mises stress at the occlusal and cervical cavity margin and wall were increased more than in the flowable restoration.

4.The Relationship between Hospital Selection by Employer and Disabilities in Occupational Accidents in Korea

Background:
In the event of an industrial accident, the appropriate choice of hospital is important for worker health and prognosis. This study investigates whether the choice of hospital by the employer in the case of industrial accidents affects the prognosis of injured employees.
Methods:
Data from the 2018 Panel Study of Workers’ Compensation Insurance in Korea were used in an unmatched case-controlled study. The exposure variable is “hospital selection by an employer,” and the outcome variable is ‘worker’s disability.” Odds ratios (ORs) were estimated by modified Poisson regression and adjusted for age, gender, underlying disease, injury severity, and workplace size and stratified by industrial classification. The group at increased risk was analyzed and stratified by age, gender, and area.
Results:
In the construction industry, hospital selection by the employer was significantly associated with increased risk of disability (adjusted OR 1.26; 95% confidence interval [CI]: 1.20–1.32) and severe disability (adjusted OR 1.38; 95% CI: 1.08–1.76) among the injured. Female and younger workers not living in the Seoul capital area were more at risk of disability and severe disability than those living in the Seoul capital area.
Conclusions
Hospital selection by employers affects the prognosis of workers injured in an industrial accident. For protecting workers’ health and safety, workplace emergency medical systems should be improved, and the selection of appropriate hospitals to supply treatment should be reviewed.
5.Comparison of work environment and occupational injury in direct and indirect employment in Korea and Europe

BACKGROUND: To investigate the risk of injury for indirect employment in Korea, we compared work environment, workplace exposure, and injury risk according to the type of employment contract between Korea and European Union (EU) countries. METHODS: We analyzed data of blue-collar workers from the fourth Korean Working Conditions Survey (2014) and the sixth European Working Conditions Survey (2015) and compared workplace risk factors and preventive factors for occupational injuries. Multiple logistic regression analysis was conducted to identify the relationship between occupational injury and the type of employment contract after adjusting for age and sex. RESULTS: For descriptive characteristics, a relatively older age, lower income, lower proportion of full-time work, greater proportion in small-sized companies, and fewer unionizations were noted in indirect employment in Korea than in direct employment. Workplace exposure was significantly higher in indirect employment in both Korea and Europe. Among safety-related factors, indirect employment in both Korea and Europe lacked the support of company education, coworkers, and supervisors. Indirectly employed blue-collar workers had a significantly higher risk of occupational injury than those directly employed in Korea (odds ratio [OR]: 1.876), whereas there was no significant difference between directly and indirectly employed workers in EU countries (OR: 1.038). CONCLUSIONS: Indirectly employed blue-collar workers have an increased risk of occupational injury in Korea.

6.Epidemiologic Characteristics Revealed with a Malignant Mesothelioma Surveillance System in Korea.

OBJECTIVES: This study estimated the magnitude of malignant mesothelioma and its epidemiologic features in Korea. We collected data on 160 cases for the 5-year period from 2001 to 2005. METHODS: We established a surveillance system for malignant mesothelioma in 2001. The important participants were pathologists, and respiratory and occupational physicians. We reclassified these cases according to the date of diagnosis and calculated fatality by comparing National Mortality Data. We actively surveyed 18 cases which were diagnosed in 2004 and 2005 to evaluate their work-relatedness. RESULTS: Among 160 cases, 18 were initially diagnosed before 1995, 57 from 1996 to 2000, 11 in 2001, 18 in 2002, 11 in 2003, 29 in 2004 and 16 in 2005. After 2001, 17cases were reported annually. Among the 86 cases which were diagnosed before 2003, 60 cases (69.8%) had died and 46(53.5%) had died within 1 year. Among the 18 cases which we surveyed, 5 were related with the construction industry, 2 with asbestos textile manufacturing industries and 1 with asbestos painting. There was no evident work relatedness in the other 10 cases, but 6 were related to environmental exposure. CONCLUSIONS: Although this surveillance system revealed the magnitude of malignant mesothelioma in Korea, it needs to be supplemented by an active surveillance system using death certificate data and cancer registry data etc.
